[Injuries of the toes].

Toe injuries are common in clinical practice and cover a wide spectrum, from simple fractures and dislocations to complex tendon and soft tissue injuries. While most injuries can be conservatively treated, displaced fractures, open injuries or relevant loss of function require a differentiated surgical approach. Rare but prognostically important entities, such as décollement injuries or proximal tendon ruptures pose a particular challenge. A structured clinical examination supported, if necessary, by imaging is essential for making the correct diagnosis and deciding on the appropriate treatmen

Trends and epidemiology of knee extensor mechanism injuries presenting to United States emergency departments from 2001 to 2020.

OBJECTIVES: Injuries to the native extensor mechanism (EM) of the knee are potentially disabling and often require surgical treatment. Large-scale, updated epidemiological data on these injuries is lacking. The objective of the current study was to examine recent trends in EM injuries presenting to United States (US) Emergency Departments (EDs) over the last 20 years using a nationally representative sample. METHODS: This study retrospectively analyzed the National Electronic Injury Surveillance System (NEISS) database to identify cases of EM injuries (defined as either a quadriceps tendon ru

Combined Ipsilateral Revision Quadriceps Tendon Repair and Patellar Tendon Allograft Reconstruction Technique for Floating Patella.

Extensor mechanism disruptions of the knee are significant injuries that have the potential to result in pain and dysfunction unless surgically repaired in the acute setting. Subacute or chronic injuries and revision surgery result in added complexity. The floating patella is a rare injury involving an ipsilateral bifocal disruption of the extensor mechanism. In this article, we present our surgical technique for ipsilateral revision quadriceps tendon repair and patellar tendon reconstruction to address a recurrent tear after revision quadriceps tendon repair with a concomitant subacute patell

Creb3l1 Overexpression Improves Flexor Tendon Repair in Aged Rats.

Aging increases tendon injury incidence and poor repair outcomes, but age-related differences in tendon mechanical properties and underlying mechanisms remain unclear. We first analyzed data from the Global Burden of Disease (GBD) database to characterize the associations between aging and population-level changes in tendon injury burden. We found the age-standardized incidence rate (ASIR) of tendon injuries caused by mechanical force decreased with advancing age, whereas the age-standardized prevalence rate (ASPR) increased with age, and the years lived with disability (YLDs) due to tendon in…

Traumatic Triceps and Medial Ulnar Collateral Ligament Rupture in an Adolescent Mogul Skier: A Case Report.

Triceps tendon ruptures are uncommon injuries that account for less than 1% of all upper extremity tendon injuries. Medial ulnar collateral ligament injury (mUCL), while common in overhead athletes as a result of valgus forces during the throwing mechanics, has scarcely been reported in non-overhead, throwing individuals. Traumatic assault to the elbow may result in the rupture of the triceps tendon with concomitant mUCL injury. As such an injury pattern typically presents in middle-aged males, weightlifters, or American football players from eccentric overloading of the elbow. We present an a…

Treatment of chronic extensor tendons lesions of the fingers.

Chronic finger extensor apparatus injuries are the result of the initial acute treatment having failed or being flawed. Because of their chronic nature, these injuries present various amounts of tendon retraction, tendon callus lengthening, peritendinous scar adhesions, static and dynamic imbalances with the flexor apparatus and intrinsic muscles, and joint contractures. This article will review the anatomy of the extensor mechanism and then will outline by location, the various clinical pictures that are secondary to chronic tendon injury. The clinical presentation of these injuries can be hi

MR imaging of the tendons of the foot and ankle.

Hindfoot pain from tendon pathology is common and seen in a wide range of patients from young athletes to older sedentary individuals. Magnetic resonance (MR) is an excellent technique for imaging tendons and for identifying injuries that can be treated with surgery. MR also demonstrates the presence of bone marrow edema, which is a cause of pain and often a marker for adjacent tendon injury. Finally, MR can reveal other etiologies that cause similar hindfoot pain, such as osteochondral injuries and impingement. In this article, we review the normal MR appearance of the ankle tendons and tendo

Associated injuries found in chronic lateral ankle instability.

Sixty-one patients underwent a primary ankle lateral ligament reconstruction for chronic instability between 1989 and 1996. In addition to the ligament reconstruction, all patients had evaluation of the peroneal retinaculum, peroneal tendon inspection by routine opening of the tendon sheath, and ankle joint inspection by arthrotomy. A retrospective review of the clinical history, physical exam, MRI examination, and intraoperative findings was conducted on these 61 patients. The purpose was to determine the type and frequency of associated injuries found at surgery and during the preoperative e

Extensor pollicis longus rupture after distal radius fracture: results of reconstruction by transposition of the extensor indicis tendon and postoperative dynamic splinting.

Injury of the extensor pollicis longus tendon is one of the commonest extensor tendon injuries after distal radius fracture. In 2003-2005 we performed extensor indicis transfer in 31 patients with loss of function of the EPL tendon and postoperative dynamic splinting. In 25 patients, a distal radius fracture managed surgically led to the EPL rupture (wire internal fixation in 23 patients, plate internal fixation in 2 patients). In 6 patients, the cause was a distal radius fracture treated conservatively. Out of our 25 followed-up patients, 9 (36%) had a very good and 15 (60%) a good result in …

Elbow, forearm and wrist injuries in the athlete.

Competitive and recreational athletes sustain a wide variety of soft tissue, bone, ligament, tendon and nerve damage to their upper extremities. Most such injuries are related to direct trauma or repetitive stress, and account for a significant amount of 'down time' for athletes participating in a wide range of sports, particularly those in which the arm is utilised for throwing, catching or swinging. Overuse injuries to the elbow include musculotendinous injuries, ulnar nerve injuries and ligamentous injuries. Osteochondrol lesions of the capitellum and posterior impingement injuries in the j

Modified Ultrasonographic Classification of Tendinous Mallet Injuries Based on a Combination with Wang Classification Highlighting Lateral Band Involvement.

Background: Although ultrasonography is useful for assessing soft-tissue injuries, its role in classifying tendinous mallet injuries remains underdefined. This study aimed to propose a modified ultrasonographic classification system that combines Wang's classification with additional subtypes emphasising lateral band involvement to improve diagnostic accuracy and prognostic assessment. Methods: A retrospective cross-sectional study was performed on 36 patients with acute closed tendinous mallet injuries of the fingers. All patients underwent ultrasonographic evaluation. Based on the modified c

A novel minimally invasive nonanatomical single-bundle medial collateral ligament reconstruction technique with a short isometric construct Achilles tendon allograft: A surgical description with clinical and radiological outcomes in multiligament knee injury patients.

PURPOSE: Multiligament knee injuries (MLKIs) involve various ligaments in the knee. Current double-bundle anatomical reconstructions of the medial collateral ligament (MCL) increase the level of technical complexity, often resulting in the establishment of numerous bone tunnels and different fixation points with additional hardware. To overcome these limitations, we proposed a novel minimally invasive nonanatomical MCL reconstruction with one tibial tunnel in the metaphysis using Achilles allograft in the MLKI setting. METHODS: In a retrospective study, we enroled 24 patients with MLKIs who u

Technique for Complete Extensor Mechanism Reconstruction Using a Fresh Quadriceps Tendon-Patella-Patellar Tendon-Tibial Tubercle Allograft in a Native Knee.

Chronic extensor mechanism injuries can cause severe functional impairment and require extensive surgical intervention. Current techniques involve reconstruction of the extensor mechanism using allograft, autograft, or synthetic mesh. This Technical Note describes an extensor mechanism reconstruction utilizing a fresh complete quadriceps tendon-patella-patellar tendon-tibial tubercle allograft for patients with chronic, recurring extensor mechanism injuries.

Case of Acute Concomitant Rupture of Anterior Cruciate Ligament and Patellar Tendon of Knee: Surgical Decision Making and Outcome.

INTRODUCTION: Acute concomitant rupture of both anterior cruciate ligament (ACL) and patellar tendon is a rare injury. The diagnosis of either of the injuries may be missed on clinical examination in the emergency room. A high index of suspicion is necessary when active extension is absent, and necessary investigations like ultrasound or magnetic resonance imaging (MRI) should be done to confirm the diagnosis. There are no fixed treatment protocols in these injuries. The options include immediate repair of patellar tendon with either simultaneous or delayed reconstruction of ACL. CASE REPORT:

Hypocalcemic-induced tetany that causes triceps and bilateral quadriceps tendon ruptures.

Traumatic ruptures of the triceps are very rare among tendon injuries. A unique case of hypocalcemic tetany that induced spontaneous triceps tendon avulsion with associated bilateral quadriceps tendon ruptures is reported. The current orthopaedic literature is reviewed, and a new application of an existing surgical treatment for repair of the major tendon ruptures is described.

[Transposition of the extensor indicis tendon in reconstruction of thumb extension after rupture of the extensor pollicis longus tendon].

During a period of 13 years we performed 56 extensor indicis proprius (EIP) transpositions for reconstruction of the ruptured or severed extensor pollicis longus tendon. The open injuries (n = 9) involved failed primary repair or untreated tendon injuries. The subcutaneous ruptures occurred after distal radius fractures (n = 17) or other closed injuries of the wrist (n = 10), without trauma in 14 patients and in 6 patients by rheumatoid synovialitis or collagenosis. 35 patients returned for follow-up examination 8 months to 10.5 years after operation. According to the evaluation scheme suggest…
