It is suggested in certain circumstances, that a divided palmaris longus tendon should be repaired, as it is an ideal choice for a tendon graft, or could be of use as a tendon transfer. The musculotendinous unit may be of use as an autogenous free graft.

Many modifications of tendon transfers have been described in the treatment of radial nerve palsy. Controversy persists over the muscles of choice to get the best results in motion of wrist and fingers. Patients with radial nerve palsy and treated by the method of Merle d'Aubigne in neurophysiological tests showed that the pronator teres did not work very well for extending the wrist. Thinking about the neurophysiologic principles it seemed that the flexor digitorum superficialis of the middle and ring fingers would make a better substitute for the wrist extension. From 1994 to 1997 four patie…

A reconstructive technique and physical therapy protocol is presented for the treatment of extensor hallucis longus (EHL) lacerations with critical size defects caused by tendon retraction. The primary goal of treatment was to restore EHL structure and function without the use of a bridging allograft or tendon transfer. The technique is performed by split lengthening the distal segment of the lacerated EHL and rotating the lengthened segment proximally 180° to bridge the tendon defect. The lengthened tendon is then sutured to the proximal segment of the EHL. The EHL is then tubularized with an…

INTRODUCTION: Chronic Achilles tendon rupture (CATR) presents a therapeutic challenge due to tendon retraction, degeneration, and muscle atrophy that typically occur beyond four weeks after injury, complicating delayed repair. To address the resultant tendon defect and restore functional continuity, several reconstructive strategies have been described, including turndown flap augmentation and flexor hallucis longus (FHL) tendon transfer.
OBJECTIVE: This study evaluated functional outcomes and complications following combined turndown flap augmentation and FHL tendon transfer for CATR.

BACKGROUND: Footdrop, or the inability to actively dorsiflex the foot, may result from numerous pathologic conditions, including poliomyelitis and cerebral palsy. Although the gait of patients with footdrop can be improved by performing an extensor hallucis longus (EHL) to tibialis anterior (TA) tendon transfer, the success rate of this procedure is relatively low.
METHODS: Seven paralytic patients with footdrop were surgically treated using a new buttonhole-type technique that involves passing a loop of the EHL through a TA split using umbilical tape and suturing at the four corners of the E…

Combined flexor hallucis longus tendon transfer and bone-tendon allograft transplantation is a reasonable option for advanced distal-segment Achilles tendinopathy. This procedure provides anatomic restoration and improved function of the posterior muscle group without sacrificing the regional anatomy. Allograft transplantation is safe and does not require immunosuppressive therapy. The soft tissue envelope should be healthy because wound complications can be an issue. This procedure is especially helpful in patients with significant disability.

Reports have demonstrated that peroneal tendon tears can be successfully treated by direct repair or debridement and tubularization, based on the extent and nature of the pathology. Irreparable peroneal tendon tears or completely unsalvageable tendons after failure of previously attempted repairs are rare, and as a result there is a lack of high-level evidence to guide the management of these complex injuries. When irreparable tears are encountered the salvage options include tenodesis, bridging of the defect using allografts or autografts, and tendon transfers. The choice of treatment strateg…

We present an interesting and rare case of a sequential bilateral traumatic rupture of the extensor pollicis brevis tendons distal to the first extensor compartment in a patient with no underlying systemic condition or pathology around the wrist. Satisfactory thumb function was restored by bilateral transfer of the extensor indicis proprius.

Much discussion exists about the best operative technique to treat congenital convex pes valgus. In this article a table of surgical approaches and an algorithm, based upon literature review, are presented. In our opinion the technique of choice in a child younger than 2 years of age is extensive release with lengthening of tendons and fixation procedures. In a child over 2 years of age, extensive release with tendon transfer is the preferred procedure. When this procedure has failed, naviculectomy with extensive release and tendon transfer, or subtalar / triple arthrodesis must be considered.

Tendon transfer of the flexor digitorum longus tendon (FDLT) or the flexor hallucis longus tendon (FHLT) into the tibialis posterior tendon is carried out in patients with tibialis posterior dysfunction. FDLT and FHLT are connected in the region of the knot of Henry. The present study has investigated the anatomical variations of this tendinous interconnection. The results could be used to determine which of the two tendons should be transected proximal to the region of the knot of Henry in the surgical treatment of tibialis posterior dysfunction. In over two-thirds of cadaver specimens invest…

Achilles tendinitis is a common condition of overuse seen in active individuals. It is multifactorial, usually due to a combination of anatomic and biomechanical characteristics along with poor training technique. Pathologic changes can involve the surrounding tissue (bursae, paratenon) or the tendon itself and can occur both at its insertion into the calcaneous and the critical hypovascular zone. A clear understanding of the cause and stage of the condition is critical for successful treatment, be it conservative or surgical. Recent advances in surgical technique and postoperative rehabilitat

Achilles tendinitis is a problem encountered frequently. There are certain anatomical and biomechanical principles that help explain the etiology of this entity. We prefer to separate our thinking into "insertional" and "noninsertional" Achilles tendinitis. This is helpful because it allows nonoperative and operative treatment to be problem specific and systematic.
