Adherence to osteoporosis pharmacotherapy one year after osteoporotic fracture - a Swiss trauma center secondary prevention project.

AIMS OF THE STUDY: According to current evidence, one out of ten fracture patients with osteoporosis does not sustain another fracture if he or she is on adequate medication. However, epidemiological surveys show that only about 15 to 30% of affected patients avail themselves of the treatments. This cohort study investigated how many fracture patients with a recommendation for antiosteoporotic therapy effectively received treatment and the possible reasons why the treatment was not implemented. METHODS: As part of a quality improvement programme in a Swiss trauma centre, fracture patients wer

Abdominal wall reconstruction following severe loss of domain: the R Adams Cowley Shock Trauma Center algorithm.

BACKGROUND: Large, complex, posttraumatic and recurrent abdominal hernias present a reconstructive challenge. Multiple techniques have been described to restore the integrity of the abdominal wall, although the indications and applications can be difficult to navigate. The authors propose an algorithm that facilitates the assessment and treatment of secondary large ventral defects. METHODS: The algorithm described involves a systematic approach to abdominal wall reconstruction and was applied to 23 consecutive patients at the R Adams Cowley Shock Trauma Center. Data collected from the chart r

Injury prevention strategies to promote helmet use decrease severe head injuries at a level I trauma center.

Head injuries (HIs) remain a major contributor to trauma mortality, with many deaths occurring despite optimal use of available therapy. Injury prevention is vital to decrease the impact of HIs. Helmets can decrease the severity of HIs in both bicycle crashes (BCs) and motorcycle crashes (MCCs). A major challenge is to increase helmet use. A mandatory motorcycle helmet law in 1990 and information campaigns aimed at bicyclists have increased the percentage of riders wearing helmets in Washington State. We hypothesized that there would be an associated decrease in the proportion of severe HIs in

Drug use in patients admitted to a university trauma center: results of limited (rather than comprehensive) toxicology screening.

The results of rapid toxicology screening of paired blood and urine samples from 936 patients admitted to a university trauma center during the calendar year 1988 were reviewed in order to determine the drugs detected by use of a relatively limited (rather than comprehensive) screening procedure and to define the demographic characteristics of the positive population. Of the 936 cases, 65% (610) were positive for one or more drugs: ethanol (436), cocaine and/or benzoylecgonine (164), sympathomimetic amines (142), phencyclidine (40), narcotic analgesics (36), salicylates (17), and barbiturates

Efficacy of primary repair for retroperitoneal rectal injuries among patients with combined military trauma: tertiary single-center experience.

BACKGROUND: Retroperitoneal rectal injuries (RRIs) caused by combined military trauma represent a complex clinical challenge, especially in modern armed conflicts. Traditional management protocols have evolved significantly since World War II, emphasizing fecal diversion and presacral drainage. However, emerging evidence supports primary repair as a viable alternative for select cases. This study evaluates the efficacy of primary repair for RRIs among military trauma patients treated at a tertiary center. METHODS: A retrospective cohort study was conducted at the National Cancer Institute, Ky

Contemporary management pathways in liver trauma: predominant and successful use of non-operative strategies at a high-volume tertiary center.

BACKGROUND: Non-operative management (NOM) has become the default for haemodynamically stable liver trauma, yet pathway-level performance and bedside predictors of mortality warrant contemporary appraisal in University trauma centres. METHODS: We conducted a single-centre retrospective cohort study of consecutive adults (≥ 18 years) with blunt or penetrating hepatic injury managed within a protocolised, physiology- and CT-guided pathway (1 Jan 2018–30 Apr 2025). The primary endpoint was in-hospital mortality. Secondary endpoints included NOM failure, liver-specific complications, and era-level

Selective transarterial embolization for vascular complications after hip fracture surgery: An eleven-year experience in a tertiary trauma center.

BACKGROUND: Arterial injury following hip fracture surgery is an uncommon but potentially life-threatening complication. Delayed recognition may result in persistent bleeding, severe anemia, and hemodynamic instability. Selective transarterial embolization (TAE) has emerged as a minimally invasive treatment. The aim of this study was to evaluate the clinical presentation, management, and outcomes of postoperative arterial bleeding treated with TAE after hip fracture surgery. METHODS: A retrospective study was conducted including 14 consecutive patients (11 women, 3 men) who underwent TAE for

Nighttime intramedullary nailing of femoral fractures is not associated with worse outcomes: a 13-year Level I trauma center cohort.

BACKGROUND: Whether nighttime orthopedic trauma surgery adversely affects outcomes remains controversial. Evidence specifically addressing intramedullary fixation across femoral fracture locations remains limited. This study evaluated whether nighttime surgery was associated with a higher rate of recorded postoperative complications than daytime surgery. METHODS: This retrospective observational cohort included adult patients with nonperiprosthetic femoral fractures treated with intramedullary fixation between January 2010 and January 2023 at a single Level I trauma center. The Birmingham Eye Trauma Terminology - Extension from Lebanon (BETT-XL) classification: A multi-center validation in modern warfare.

BACKGROUND: In modern warfare, ocular injuries frequently result in severe injuries that challenge existing classification systems. The standardized Birmingham Eye Trauma Terminology (BETT) does not adequately capture the extensive globe or periocular damage encountered in conflict settings. We aimed to develop an extension to BETT capable of classifying the full spectrum of injuries. We further sought to validate the extended framework using a real-world cohort from the 2024 pagers explosions in Lebanon. METHODS: Phase I included a targeted literature review of severe ocular trauma cases unc
