A clinical study of 38 patients (28 men and 10 women) with tuberculous pleurisy was conducted. The age of these patients ranged from 19 to 92 years, with an average age of 48.9 years. In 30 patients, the chief complaint was fever, and other common complaints included chest pain, dyspnea, and coughing. Bacillus tuberculosis was found in the pleural fluid of 7.9% of the patients. The concentration of gamma interferon (IFN gamma) and adenosine deaminase (ADA) activity were measured in the pleural fluid of 162 patients to compare their diagnostic significance and to establish a possible correlation between both tests. Between January 1981 and December 1986, thoracoscopies were carried out on 152 patients in order to evaluate the diagnostic accuracy of the technique in cases of pleural effusion. All patients had previously undergone a thoracentesis together with an unguided pleural biopsy but had remained undiagnosed. Of them, 74 cases were confirmed as malignant; the others were benign. Eosinophilic pleuritis are inflammatory effusions containing more than 10% of eosinocytes. More frequent in men, the course of illness is acute our subacute but pleural sequelae are seldom. They can be due to all inflammatory process, thoracic trauma, tuberculosis and viral diseases are predominant. Unexplained etiology represents 25% of cases, so "called autonomous pleuritis". Pleural eosinophilia could be explained by eosinophilotactic action of red cells or by activating products induced during hypersensibility reaction.
