An atypical presentation of CNS tuberculosis manifesting with meningoencephalitis and tuberculoma in a New York taxi driver.

Mycobacterium Tuberculous (MTb) meningitis is a rare manifestation of extrapulmonary tuberculosis (Tb) but remains the most common form of Central Nervous System (CNS) manifestation of tuberculosis. It is associated with significant morbidity and mortality yet difficult to diagnose given the low sensitivity and specificity of diagnostic testing with cerebral spinal fluid (CSF) analysis which typically shows CSF findings of lymphocytic pleocytosis, elevated protein, and low glucose and is confirmed by acid fast bacillus (AFB) culture. Here, we describe a case of severe meningoencephalopathy in

[Bone tuberculosis: when consider this diagnosis?].

The most common presentation of bone tuberculosis (TB) is called spondylodiscitis, or "Pott's disease", which is a difficult diagnosis due to its low prevalence in Switzerland. It should be considered in patients with persistent back pain, who are at high risk, such as migrant population and immunocompromised patients. Diagnosis is based on imaging and the detection of M. tuberculosis in biopsy of affected vertebra orparaspinal abscess, or even if active tuberculosis is proven in any other site. It's essential to initiate appropriate treatment as quickly as possible in order to avoid neurologi

Multifocal Tuberculous Spondylitis and Extensive Extraspinal Tubercular Osteomyelitis without Immunocompromise: Case Report and Literature Review.

Multifocal extensive spinal and extraspinal tuberculosis is very rare. So far, fewer than 10 cases have been reported. We hereby report two such cases to highlight their rarity, the diagnostic and therapeutic challenges they presented, and the overall prognosis of the condition. The two patients (a 19-year-old woman and another 20-year-old woman) had multifocal extensive tuberculosis involving spine and appendicular skeleton with neurological deficit. Both patients presented with back and neck pain and gradual neurological deficit. The insidious onset and malignancy-like spread pattern mimicke

[Radiology in tuberculosis].

The radiological manifestations of pulmonary tuberculosis such as primary and secondary tuberculosis, micronodular tuberculosis and tuberculous pneumonia as well as postinfectious residual scarring, are described. Simultaneously the radiology of the extrapulmonary manifestations of tuberculosis such as tuberculous pleurisy, spondylitis, different forms of abdominal tuberculosis and renal tuberculosis is discussed. Pathophysiological mechanisms are used to classify the different manifestations of tuberculosis, to explain the localizations typical of pulmonary tuberculosis as well as of extrapul

Simultaneous Presentation of Relapsed Diffuse Large B-cell Lymphoma and Extrapulmonary Tuberculosis in a Patient With HIV: A Case Report.

Human immunodeficiency virus (HIV) infection is a strong risk factor for diffuse large B-cell lymphoma (DLBL) and tuberculosis. Both DLBCL and tuberculosis can have remarkably similar clinical presentations, proving to be a diagnostic and therapeutic challenge. We report the only known case of an HIV-infected individual who presented simultaneously with relapsed DLBCL in the form of spinal cord involvement and tuberculosis of the mesenteric lymph nodes. This case highlights the possibility of multiple co-existing diagnoses in HIV, and the need for a low threshold to obtain confirmation via bio

The incidence of Mucormycosis superinfection: A challenge to current Covid-19 pandemic.

OBJECTIVE: Spinal tuberculosis, though destructive, can be cured in many patients by chemotherapy, though surgery is often necessary for decompression and deformity correction. Our aim of this study was to investigate the clinical efficacy of posterior debridement joint graft fixation therapy for lumbar vertebral fractures in patients with spinal tuberculosis with a compression fracture. PATIENTS AND METHODS: We prospectively included 48 patients diagnosed with spinal tuberculosis and lumbar compression fracture in our hospital from June 2010 to June 2013. The patients were randomly divided i

Revisiting the CNS tuberculosis with emphasis on giant tuberculomas and introducing the "outer rim excrescence sign".

Central nervous system tuberculosis is a leading cause of morbidity and mortality in developing countries with Mycobacterium tuberculosis as the main etiological agent. Classical imaging findings pose no problem for diagnosis of CNS tuberculosis. Cases of giant or solitary ring enhancing lesions are increasing which mimic other infectious and noninfectious pathologies. This is a retrospective study of 40 patients (28 male and 12 female) of either surgically or clinically proven CNS tuberculosis. Solitary tuberculomas (ring or solid enhancing) were seen in 19 patients. Four patients showed pres

An analysis of clinicoradiological and histopathological correlation in tuberculosis of spine.

Two hundred and sixty-seven consecutive patients with clinical and radiological features suggestive of tuberculosis of the spine treated over a period of five years were analysed : (1) To assess how dependable clinical and radiological features are in establishing a diagnosis of spinal tuberculosis and (2) to identify other conditions which may have clinical and radiological features similar to those seen in patients with spinal tuberculosis. This retrospective case series analysis was done in a university teaching hospital in south India. The clinical presentation, radiological features and t

[Clinical features, diagnosis and treatment of solitary vertebral arch tuberculosis].

OBJECTIVE: To explore clinical features of vertebral arch tuberculosis, and to discuss the diagnosis and treatment of this illness. METHOD: Seventeen of 3,825 cases of spinal tuberculosis who were hospitalized and treated from 1956 to 1996 were reviewed, and their clinical features, diagnosis and treatment were analyzed and reported. RESULT: Sixteen cases of vertebral arch tuberculosis were in adolescence and prime of life, and another one was a 2-year-old child. Their symptoms in early stages were untypical, including local fixed pain and tenderness, and later, abscess, fistula and symptoms

Preparation and in vitro sustained release performance of thermosensitive gel microsphere composite loaded with Anti-Tuberculosis drug PaMZ/BMP-2.

BACKGROUND: To meet the needs of local treatment for spinal tuberculosis, this study developed a thermosensitive gel microsphere composite with both anti-tuberculosis and osteogenesis-promoting functions for efficient drug sustained release. The composite consists of anti-tuberculosis drugs (Pretomanid, Pa-824; Moxifloxacin, M; Pyrazinamide, Z) and bone morphogenetic protein-2 (BMP-2) to optimize drug release kinetics and enhance local therapeutic effects. METHODS: PaMZ/BMP-2 microspheres were prepared using microfluidics and characterized by SEM, particle size analysis, and XRD. LC-MS and EL

Cervical Spine Tuberculosis: A Systematic Review and Meta-Analysis of Epidemiology, Clinical Presentation, Diagnosis, Management, and Outcomes.

BACKGROUND: Spinal tuberculosis is the most common form of extra pulmonary tuberculosis. While cervical involvement, known as cervical Pott's disease, has traditionally been considered rare, its incidence is increasing. However, comprehensive data on its clinical presentation, diagnostic methods, treatment options, and outcomes remain limited. This review consolidates evidence on cervical spine tuberculosis, offering insights into its epidemiology, therapeutic strategies, and outcomes. METHODS: A systematic search of PubMed/MEDLINE and Google Scholar was conducted following the Preferred Repo

The rare manifestations in tuberculous meningoencephalitis: a review of available literature.

Aim: Tuberculous meningitis is an infectious disease of the central nervous system caused by Mycobacterium tuberculosis (M. tuberculosis). It mainly involves the meninges and brain parenchyma, as well as the spinal cord and meninges; Disability and mortality rates are high. In recent years, due to the increase of drug-resistant tuberculosis patients, population mobility and the prevalence of acquired immune deficiency syndrome, the incidence rate of tuberculosis has increased significantly, and tuberculous meningitis has also increased. Methods: At present, tuberculosis is still a worldwide inf

Multiple Intracranial Tuberculomas with an Intra-medullary Spinal Cord Tuberculoma in a Pediatric Patient.

Central nervous system (CNS) tuberculosis (TB), caused by Mycobacterium tuberculosis (MT), is a severe form of TB, which presents as meningitis, cerebritis, abscesses, spinal tuberculous arachnoiditis, and rarely tuberculomas. CNS TB is prevalent in the underdeveloped or developing world and is common in malnourished, alcoholics, children, young adults, immunocompromised, and cancer patients. Intracranial tuberculomas (ICT) can present with symptoms and signs of focal neurological deficits with or without systemic manifestations. ICT is the least common presentation of CNS TB. TUBERCULOSIS INFECTION MIGHT INCREASE THE RISK OF INVASIVE CANDIDIASIS IN AN IMMUNOCOMPETENT PATIENT.

Deep Candida infections commonly occur in immunosuppressed patients. A rare case of a multiple deep organ infection with Candida albicans and spinal tuberculosis was reported in a healthy young man. The 19-year-old man complained of month-long fever and lower back pain. He also had a history of scalded mouth syndrome. Coinfection with Mycobacterium tuberculosis and Candida albicans was diagnosed using the culture of aspirates from different regions. Symptoms improved considerably after antifungal and antituberculous therapy. This case illustrates that infection with tuberculosis might impair t
