OBJECTIVE: To explore a surgical treatment for obstructive sleep apnea hypopnea syndrome (OSAHS).
METHODS: 12 cases were treated during the period from Jan 1998 to Aug 2006. Partial soft palate was resected in rhombus shape from the middle to shorten the soft palate and enlarge the pharyngeal cavity. The uvula was reserved.
RESULTS: The patients were followed up for six months to five years. There was no complication. Good results were achieved in 9 patients. 2 cases got some kind of improvement. No improvement happened in one case who received a partial tongue resection later.

OBJECTIVE: It was aimed to investigate the effect of plasma radiofrequency ablation on obstructive sleep apnea hypopnea syndrome.
METHOD: Fifty-four patients with OSAHS were treated with plasma radiofrequency ablation on soft palate, tonsil, uvula and root of tongue in light of needs.
RESULT: All these cases were reevaluated with PSG by the end of postoperative period lasted for three month. Among them, 6 were cured, 24 had significant therapeutic effect, 19 had effective outcome, and 5 had no effective, with a total effective rate of 90.74%. No serious complications occurred.

Initially described in 1981, uvulopalatopharyngoplasty was the first surgical procedure specially designed at the palatal level for snoring and obstructive sleep apnea (OSA). To date, palatal surgery remains the most commonly used surgery for OSA. The advancement of this surgery over the past 30 years has been a process of evolution in concepts, examination, and technique to increase safety and effectiveness in the treatment of OSA. Concept changes have emerged in the disease etiology, purpose of surgery, treatment priorities, staging of operations, integration therapy, and surgical endpoints.

A case with palatal presentation of scleroma is presented due to its rarity and unusual manifestations. The patient presented with only palatal symptoms. Nasal endoscopy revealed signs of all three stages of scleroma: catarrhal, granulomatous and sclerotic stages. Histopathology was vital in the diagnosis. The patient was treated by medical therapy, and palatal symptoms were relieved by uvulopalatopharyngoplasty.

The author presents the case of a male patient with obstructive sleep apnoea syndrome, in which the use of classic CPAP therapy after performing an uvulo-pharyngoplasty (UVPP) led to very good results. The pressure levels determined initially after a polysomnography with Apnoe Check were compared with the results of the measurements with another device, AutoSet, used for the first time in our country. This allowed the tapering of pressure levels for CPAP and also revealed other advantages of the device, reviewed here.

Although uvulopalatopharyngoplasty has been proposed as a treatment for obstructive sleep apnea, there is limited objective data concerning its efficacy. We have examined 40 patients with moderate to severe obstructive sleep apnea before and 3 months following uvulopalatopharyngoplasty. A standardized questionnaire and detailed overnight sleep study were performed before and after surgery. In addition, 15 patients were retested 1 year postoperatively. This article presents the first documented case of recurrent isolated uvular angioedema associated with intranasal cocaine use.
