This prospective randomized study was performed in order to investigate the effects of interventive treatment or counselling on pregnancy rates in infertile couples in whose male partners a varicocele was diagnosed. Varicocele, a varicosity of the pampiniform plexus, usually on the left side, is a common urologic problem. It may be associated with symptoms of local discomfort or abnormal spermatogenesis. The value of intraoperative anterograde spermatic phlebography in the treatment of varicocele is maintained. The knowledge of the vascular anatomy making it possible to adapt surgical tactics to the needs of the particular patient. Varicocelectomy in the adult is followed by a relatively low recurrence rate but children treated by conventional techniques seem prone to have more frequent recurrences. A new method of non-surgical treatment of varicocele syndrome is described: it consists in sclerotherapy of spermatic vein by trans-femoral percutaneous catheterization with balloon-catheters. In 8 cases venous thrombosis has been induced by direct electric clotting. The techniques and a 6 months follow-up are discussed. It is pointed out that this procedure should be considered as the method of choice for tubular lesions and sub-fertility prophylaxis in young people and in childhood.

A quantitative analysis of Leydig cells was performed in 23 testicular biopsies of men with left varicocele and sperm count ranging from zero to 95,000 sperm/mm3. The oligozoospermic patients had more Leydig cells and higher FSH and LH serum levels than the patient group with more than 10,000 sperm/mm3. The results of a total of 102 varicocele operations, 83 of which were performed because of subfertility and a desire to have children, are presented. Spermatological examinations were performed on 61 patients before and after varicocelectomy. Sperm morphology remained unchanged, but the sperm density increased in 67.2% cases and in 54.1% the sperm motility rose over a period of up to 18 months following the treatment.
