Short- and medium-term outcomes after transcatheter pulmonary valve placement in the expanded multicenter US melody valve trial.

BACKGROUND: Transcatheter pulmonary valve placement is an emerging therapy for pulmonary regurgitation and right ventricular outflow tract obstruction in selected patients. The Melody valve was recently approved in the United States for placement in dysfunctional right ventricular outflow tract conduits. METHODS AND RESULTS: From January 2007 to August 2009, 136 patients (median age, 19 years) underwent catheterization for intended Melody valve implantation at 5 centers. Implantation was attempted in 124 patients; in the other 12, transcatheter pulmonary valve placement was not attempted beca

The Ross-Konno procedure in children: outcomes, autograft and allograft function, and reoperations.

BACKGROUND: The Ross aortic valve replacement with a modified Konno-type enlargement Ross-Konno procedure of the aortic annulus and subannular region allows an autograft aortic valve replacement for children with significant annular and subannular hypoplasia. The potential for growth and the proven durability of the autograft make the Ross-Konno procedure an ideal aortic valve replacement for this subgroup with multilevel left ventricular outflow tract obstruction. We reviewed our institutional midterm experience to assess autograft and homograft hemodynamics, and management after a Ross-Konno

Instantaneous subaortic outflow obstruction after volume reduction in hearts with univentricular atrioventricular connection and discordant ventriculoarterial connection.

OBJECTIVE: To study the phenomenon of potential subaortic outflow obstruction after surgical volume unloading of the heart in patients with univentricular atrioventricular connection, discordant ventriculoarterial connection, and bulboventricular foramen (BVF)-dependent systemic flow. MATERIAL AND METHODS: Intraoperative transesophageal echocardiography was used in five patients with tricuspid atresia (N = 3) or double-inlet left ventricle (N = 2) with rudimentary right ventricle and BVF who were scheduled to undergo a bidirectional cavopulmonary anastomosis (N = 3) or completion of the Fonta

Late presentation of pulmonary valve stenosis confirmed by cardiovascular magnetic resonance.

We describe the case of a 70-year-old man who presented with increasing exertional dyspnea. He was found to have an ejection systolic murmur and evidence of right ventricular outflow tract obstruction, with a peak velocity of 4.5 m/s recorded by transthoracic Doppler echocardiography. Cardiovascular magnetic resonance showed right ventricular hypertrophy, pulmonary valve stenosis, peak recorded velocity 4.2 m/s, with thickened pulmonary valve leaflets of reduced mobility, and poststenotic dilatation of the main pulmonary artery. The case illustrates that severe pulmonary valve stenosis can pre

Pulmonary valve-moderator band distance and association with development of double-chambered right ventricle.

Double-chambered right ventricle (DCRV), a form of right ventricular outflow obstruction that sometimes accompanies a ventricular septal defect (VSD), is associated with superior and rightward displacement of the septal insertion of the moderator band. It was hypothesized that this superior displacement is present and identifiable by echocardiography in patients with a VSD even before right ventricular outflow tract obstruction develops. Eight patients who had a previous echocardiographic study showing a VSD alone were echocardiographically diagnosed as having DCRV. Their initial echocardiogra…

Surgery for biventricular obstruction in hypertrophic cardiomyopathy in children and young adults: technique and outcomes†.

OBJECTIVES: Biventricular obstruction in hypertrophic cardiomyopathy (HCM) is uncommon, and some clinicians believe that, when symptoms are refractory to medical treatment, this severe form of HCM is best treated by transplantation. We describe our conventional surgical approach and outcomes to treat biventricular obstruction in HCM. METHODS: From 1993 to 2013, we treated 11 symptomatic patients with biventricular outflow obstruction. Relief of left ventricular (LV) obstruction was obtained by performing a transaortic extended septal myectomy and/or a left apical ventriculotomy. Right ventric…

A rare finding of giant accessory mitral valve tissue: a case report.

BACKGROUND: Accessory mitral valve tissue (AMVT) is a rare anomaly that can be detected in the first decade. It is associated with other congenital cardiac abnormalities, such as ventricular septal defect. When detected in adulthood, it is usually an incidental finding on echocardiography. Symptomatic individuals can present with breathlessness, syncope, and features of distal tissue embolization. Cardiac surgery is indicated in those with significant left ventricular outflow tract obstruction. CASE SUMMARY: A 45-year-old man without any significant medical history was referred due to an abno

Mavacamten for Obstructive HCM in Malignancy With Life-Threatening Symptoms.

BACKGROUND: A 79-year-old woman admitted to critical care with obstructive hypertrophic cardiomyopathy and metastatic pancreatic neuroendocrine tumor presented with recurrent pulmonary edema and hemodynamic compromise from dynamic left ventricular outflow tract obstruction. CASE SUMMARY: Mavacamten was initiated, leading to rapid symptomatic improvement. However, during the course of treatment, the patient developed marked liver enzyme elevation, raising concern for drug-induced liver injury or an alternate oncological diagnosis. DISCUSSION: Multidisciplinary discussion was essential for rea

Severe Mitral Annular Calcification in Two Cases of Hypertrophic Obstructive Cardiomyopathy.

Management of patients with hypertrophic obstructive cardiomyopathy and severe mitral annular calcification can be challenging. Our cases highlight the importance of addressing all elements contributing to left ventricular outflow tract obstruction in cases of hypertrophic obstructive cardiomyopathy: hypertrophic basal interventricular septum, abnormal papillary muscles, and systolic anterior motion of the anterior mitral valve leaflet. Addressing mitral valve repair through aortotomy by performing a septal myectomy, papillary muscle realignment, and resection of aberrant chordae tendineae all

Long-term results of the modified Konno procedure in high-risk children with obstructive hypertrophic cardiomyopathy.

OBJECTIVE: Transaortic septal myectomy is the gold standard surgery in obstructive hypertrophic cardiomyopathy, but it is not optimal for children aged less than 5 years and with right ventricular outflow tract obstruction. We evaluated outcomes with the modified Konno procedure in children with severe forms of obstructive hypertrophic cardiomyopathy. METHODS: A total of 79 consecutive children who underwent the modified Konno procedure in our center between 1991 and 2016 were included. RESULTS: Clinical features included age less than 5 years (38%), maximal septal thickness 25 mm or more (3

Asymptomatic Left Ventricular Myxoma in a 12-Year-Old Male.

Cardiac myxoma is the most common cardiac tumor in patients of all ages; the majority are encountered as single left atrial tumors. Left ventricular myxomas are exceedingly rare, having been recorded in a small number of case reports involving children worldwide. We report a case of a left ventricular myxoma with left ventricular outflow tract obstruction in a previously healthy, asymptomatic adolescent black male. Transthoracic echocardiograms revealed a single, large (2.5 × 5-cm), lobulated, mobile mass within the left ventricular cavity that oscillated into the outflow tract, thereby causin

Echocardiographic assessment of adults with tetralogy of Fallot.

Advances in medical and surgical management of congenital heart disease in the last three decades have resulted in a great many survivors to adulthood. Proper care requires intimate knowledge of the basic malformations and their surgical anatomy and results. Tetralogy of Fallot, the most common cyanotic malformation in adults, represents a spectrum from mild right ventricular outflow tract obstruction to complete pulmonary atresia. Evaluation of surgical residua and sequelae includes imaging of aortic-to-pulmonary arterial palliative shunts, detection of residual ventricular septal defect patc

Complete left bundle branch block resolution during treatment with Mavacamten: a case report of a first-in-class scenario.

BACKGROUND: Hypertrophic cardiomyopathy is a common form of genetic heart disease and in two-thirds of cases is associated with left ventricular outflow tract obstruction. Mavacamten, a selective cardiac myosin ATPase inhibitor, has emerged as the first disease-specific approach able to target the molecular mechanism responsible for this clinical phenotype. CASE SUMMARY: A 69-year-old woman, admitted for a presyncopal episode, was diagnosed with obstructive hypertrophic cardiomyopathy and treated with beta-blockers at maximal tolerated dose. At follow-up, she was still symptomatic with clinic

Trapezoidal resection of an elongated anterior mitral leaflet and Alfieri stitch in hypertrophic cardiomyopathy.

BACKGROUND: In individuals with hypertrophic obstructive cardiomyopathy, elongated anterior mitral leaflets are commonly associated with systolic anterior motion. In patients with mild septal hypertrophy, a myectomy is considered insufficient to relieve systolic anterior motion and left ventricular outflow tract obstruction. CASE PRESENTATION: In the patient, who had relatively mild septal hypertrophy, the section of the anterior leaflet protruding into the left ventricular outflow tract was resected, concomitant with septal myectomy and the relocation of the papillary muscles. [Multiple lentiginosis and hypertrophic obstructive cardiomyopathy].

Two patients with Multiple Lentiginosis (ML) had hypertrophic cardiomyopathy with severe obstruction of the right and left ventricular outflow tracts. ML is a rare syndrome in which there is a typical skin pigmentation disorder together with various other abnormalities like slight mental retardation, sensorineural deafness, genital and skeletal anomalies and sometimes a characteristic facies. Obstruction of the right ventricular outflow tract is frequent in such patients. The association of ML and obstructive hypertrophic cardiomyopathy has been recently reported. A critical examination of the

Factors Associated with Prolonged Mechanical Ventilation in Late Repair of Tetralogy of Fallot.

Research establishing factors associated with duration of mechanical ventilation after Tetralogy of Fallot repair, is mainly based on population presenting at early infancy. There are fewer reports regarding repair after infancy, during childhood and preadolescence. To compare two groups of late TOF repair based on post-operative invasive mechanical ventilation duration and explore associations with pre-operative clinical markers of severity of right ventricular outflow tract obstruction. Pharmacotherapy for the treatment of obstructive hypertrophic cardiomyopathy.

Introduction: Hypertrophic cardiomyopathy (HCM) is one of the most common genetic heart diseases and represents a leading cause of sudden cardiac death as well as a prevalent cause of heart failure and stroke. HCM is characterized by a very complex pathophysiology, consisting of heterogeneous clinical manifestations and natural history. Left ventricular outflow tract (LVOT) obstruction has been considered the most knowable feature of HCM since the initial clinical descriptions of the disease.Areas covered: In this review, the authors discuss the most recent reports on the pharmacological treat

The case for surgery in obstructive hypertrophic cardiomyopathy.

Relief of left ventricular (LV) outflow obstruction in patients with hypertrophic cardiomyopathy (HCM) and disabling symptoms refractory to maximum medical management has historically been a surgical problem. Surgical septal myectomy permanently abolishes systolic anterior motion of the mitral valve and mitral regurgitation, while normalizing LV pressures and wall stress. Also, these salutary goals are achieved without encumbering patients with post-procedural devices (e.g., pacemakers or defibrillators) or creating potentially arrhythmogenic substrates, as may occur with alcohol septal ablati
