Vitamin D deficiency in veterans with chronic spinal cord injury.

Chronic spinal cord injury (SCI) is associated with osteopenia, increasing the prevalence of long-bone fractures. Although disuse may be the primary cause of osteopenia, identification of any additional mechanisms of bone loss may lead to potential therapeutic interventions. We investigated the relationships of serum calcium (Ca), phosphorus (PO4), albumin, alkaline phosphatase (Alk P), and parathyroid hormone (PTH) with serum 25-hydroxyvitamin D [25(OH)D] in 100 subjects with chronic SCI and 50 control subjects. in a subgroup of 50 subjects with SCI and 50 control subjects, we correlated thes

Leishmaniasis among Desert Storm veterans: a diagnostic and therapeutic dilemma.

Troops stationed in the Middle East during Operations Desert Shield/Storm were potentially exposed to leishmaniasis, a parasitic infection transmitted by sandflies. In this region, infection primarily causes cutaneous and, less often, visceral disease. Visceral leishmaniasis, which typically has an incubation period of several months, can be a difficult diagnosis as it presents with a wide range of symptoms and there are no non-invasive, reliable diagnostic tests. Cutaneous leishmaniasis is more easily diagnosed using culture and stained smears of biopsy and aspirate samples from skin lesions.

CD-ROM prototype applications within the Department of Veterans Affairs.

The Department of Veterans Affairs (VA) has been actively involved in examining the benefits of Compact Disc Read Only Memory (CD-ROM) to its operations. Approximately twenty VA titles either have been produced or are scheduled for production by Summer of 1991. Both textual and database CDs have been prototyped. Several of the CDs have been integrated directly into the VA's MUMPS-based hospital information systems. Titles which contain patient information employ the Data Encryption Standard (DES) for sensitive information. A related project involves the installation of a system which allows co…

The Veterans Administration Cooperative Study of stable angina: current status.

The current status of the Veterans Administration Cooperative Study of the effect of surgery on survival in patients with stable angina is presented. The outcome in 686 adult males randomly allocated to medical or surgical treatment groups in 1972-1974 was studied in subgroups of patients classified by invasive (arteriographic) and noninvasive risk factors. In 91 patients with left main lesions reducing the luminal diameter 50% or more, surgery significantly improved survival in the two-thirds characterized as middle or high risk by four simple noninvasive predictors of prognosis (New York Hea…

Choosing appropriate comparison group participants in studies of veterans: Characteristics of orthopedically injured and uninjured Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn veterans.

INTRODUCTION: Research addressing deployment-related traumatic brain injury (TBI) is fairly complex due to a high prevalence of comorbid conditions, multiple exposures, and the lack of acute medical records. Therefore, there is a need for a well-defined, matching comparison group. This study compared deployment-related characteristics, everyday functioning, and cognitive performance in recently deployed veterans who had not sustained any injuries with those who had orthopedic injuries during deployment, but who were without a history of TBI. METHOD: Participants included 45 individuals who ha…

Trade-Offs Between Patient Experience and Health Care Access: Comparing Veterans Health Administration Versus Community Care by Veteran Race and Ethnicity.

BACKGROUND: Racial and ethnic disparities in patient-reported experiences exist in the Veterans Health Administration (VA) and in VA-paid health care from non-VA providers ("community care"), but how these disparities compare is unknown. OBJECTIVES: We compared racial and ethnic disparities in patient-reported experiences between VA primary care and community care, and calculated differences in VA versus community care by patient race and ethnicity. METHODS: Using cross-sectional data from the 2020 to 2023 VA Survey of Healthcare Experiences of Patients-VA Primary Care and Community Care sur…

Examining bias in the award of Veterans Affairs (VA) disability benefits for posttraumatic stress disorder in women veterans: Analysis of evaluation reports and VA decisions.

Studies have raised concerns about possible inequities in the U.S. Department of Veterans Affairs (VA)'s awards of disability for posttraumatic stress disorder (PTSD) to women. However, the diagnoses and opinions made by disability examiners have not been studied. A sample of 270 initial PTSD examination reports and corresponding VA decisions were studied. Compared to men, women veterans were as likely to be diagnosed with a service-related mental disorder, χ2(1, N = 270) = 2.31, p = .129, odds ratio (OR) = 1.79, 95% CI [0.84, 3.80], and be granted service-connection, χ2(1, N = 270) = 0.49, p …

Well-being of Canadian Veterans during the COVID-19 pandemic: cross-sectional results from the COVID-19 Veteran well-being study.

BACKGROUND: The impacts of the COVID-19 pandemic have disproportionally affected different population groups. Veterans are more likely to have pre-existing mental health conditions compared to the general Canadian population, experience compounded stressors resulting from disruptions to familial, social, and occupational domains, and were faced with changes in health-care delivery (e.g. telehealth). The objectives of this study are to assess (a) the mental health impact of COVID-19 and related life changes on the well-being of Veterans and (b) perceptions of and satisfaction with changes in he…

Receipt of Overlapping Opioid and Benzodiazepine Prescriptions Among Veterans Dually Enrolled in Medicare Part D and the Department of Veterans Affairs: A Cross-sectional Study.

BACKGROUND: Overlapping use of opioids and benzodiazepines is associated with increased risk for overdose. Veterans receiving medications concurrently from the U.S. Department of Veterans Affairs (VA) and Medicare may be at higher risk for such overlap. OBJECTIVE: To assess the association between dual use of VA and Medicare drug benefits and receipt of overlapping opioid and benzodiazepine prescriptions. DESIGN: Cross-sectional. SETTING: VA and Medicare. PARTICIPANTS: All veterans enrolled in VA and Medicare Part D who filled at least 2 opioid prescriptions in 2013 (n = 368 891). MEASURE…

Substance use over the military-veteran life course: an analysis of a sample of OEF/OIF veterans returning to low-income predominately minority communities.

This paper presents an overview of substance use patterns of recent veterans returning to low-income predominately minority communities over four periods of the military-veteran career. Respondent driven sampling (RDS) was used so that unbiased estimates could be obtained for the characteristics of the target population. The majority of participants had used marijuana but no other illegal drugs. In the military, marijuana use was substantially lower and alcohol was the drug of choice; the majority were binge drinkers and nearly half were heavy drinkers. While deployed, alcohol and marijuana us…

Leveraging Natural Language Processing to Identify Veterans Who Inject Drugs to Assess Preexposure Prophylaxis and Sexually Transmitted Infection Testing Services at the Veterans Health Administration.

BACKGROUND: People who inject drugs (PWID) face disproportionate risks for infectious diseases yet remain difficult to identify within health care systems. Natural language processing (NLP) offers potential solutions for identifying PWID to improve access to harm reduction services. METHODS: We evaluated an NLP dashboard designed to identify Veterans with evidence of injection drug use across 6 Veterans Health Administration facilities between August and October 2024. Four independent reviewers assessed electronic health records to confirm recent injection drug use and evaluated preventive ca…

The Role of Military Sexual Trauma History and Frequency of Use of Veterans Health Administration Homeless and Justice Program Services Among Male and Female Homeless Veterans.

Military sexual trauma (MST) is a prevalent experience associated with deleterious chronic health and psychiatric conditions. The Department of Veterans Affairs (VA) offers MST-related care free of charge; however, MST history can impact perceptions of trust and safety, affecting willingness to access such care. MST is also more prevalent among Veterans with a history of homelessness and criminal legal involvement. These Veterans are often clinically complex and may benefit from higher rates of VA service use to meet their health and social care needs. No research to date has examined the pote…

Screening rates for hepatitis B and C among low-income US veterans: Data from the National Veteran Homeless and Other Poverty Experiences Study.

Screening for viral hepatitis is considered a high-priority area in the Veterans Health Administration (VHA). Yet, few studies have examined viral hepatitis screening test use among low-income veterans who are considered high-risk with limited healthcare access. Using cross-sectional data from 933 participants in the 2021-2022 National Veteran Homeless and Other Poverty Experiences (NV-HOPE) study, we examined rates and correlates of lifetime screening for hepatitis B (HBV) and hepatitis C (HCV) infections. Multivariable logistic regression models evaluated characteristics associated with HBV/…

Association of Medication-Assisted Treatment and Age With Treatment Completion Among Veterans Seeking Non-VA Treatment for Opioid use Disorder at Non-Veterans Health Administration Facilities.

BACKGROUND: Medication-assisted treatment has been shown to be effective in treating opioid use disorder among both older adults and veterans of U.S. Armed Forces. However, limited evidence exists on MAT's differential effect on treatment completion across age groups. This study aims to ascertain the role of MAT and age in treatment completion among veterans seeking treatment in non-Department of Veterans Affairs healthcare facilities for opioid use disorder. METHODS: We used the Treatment Episode Data Set-Discharges (TEDS-D; 2006-2017) to examine trends in treatment and MAT usage over time a…

Antipsychotic initiation and new diagnoses excluded from quality-measure reporting among Veterans in community nursing homes contracted by the Veterans Health Administration in the United States.

OBJECTIVES: To assess whether prevailing antipsychotic use rates in community nursing homes (CNH) influence new initiation of antipsychotics and diagnosis with antipsychotic indications among Veterans. METHODS: We used linked 2013-2016 Veterans Administration (VA) data, Medicare claims, Nursing Home Compare, and Minimum Data Set (MDS) assessments. The exposure was the proportion (in quintiles) of all CNH residents prescribed antipsychotics in the quarter preceding a Veteran's admission date. Using adjusted logistic regression, we analyzed two outcomes measured using MDS: antipsychotic initiat…
