Pathological diagnosis and cytoreduction of cardiophrenic lymph node and pleural metastasis in ovarian cancer patients using video-assisted thoracic surgery.

BACKGROUND: The aim of this study was to assess the benefit of video-assisted thoracic surgery (VATS) in pathological diagnosis and intrathoracic cytoreduction of cardiophrenic lymph node (CPLN) and pleural metastasis on computed tomography (CT) in patients with ovarian cancer. METHODS: We reviewed a database of ovarian cancer patients who underwent VATS from June 2007 to November 2008. The criterion for suspicion of malignancy on CT was either a CPLN with short-axis dimension >5 mm or multinodular pleural thickening. Seven patients with primary ovarian cancer and five patients with recurrent

Is video-assisted thoracoscopic surgery a feasible approach for clinical N0 and postoperatively pathological N2 non-small cell lung cancer?

OBJECTIVE: It remains controversial whether video-assisted thoracoscopic surgery (VATS) major pulmonary resection (VMPR) with systematic node dissection (SND) is a feasible approach for clinical N0 and pathological N2 non-small cell lung cancer (cN0-pN2 NSCLC). We compared the clinical outcome of patients who underwent VMPR with SND for cN0-pN2 NSCLC with the outcome of patients who underwent MPR with SND by thoracotomy. We conducted this study to determine the feasibility of VMPR for cN0 and pN2 NSCLC patients and intraoperative node staging by node sampling. METHODS: Between 1997 and 2006,

Transcutaneous Electrical Acupoint Stimulation to Prevent Ipsilateral Shoulder Pain After Video-Assisted Thoracoscopic Surgery: A Multicenter Randomized Controlled Trial Protocol.

BACKGROUND: Ipsilateral shoulder pain (ISP) after video-assisted thoracoscopic (VATS) surgery is common, distinct from incisional pain, and frequently overlooked in clinical practice. Transcutaneous electrical acupoint stimulation (TEAS) is a non-invasive, safe non-pharmacological analgesic modality. This trial protocol aims to evaluate the potential preventive effect of TEAS on ISP after VATS surgery. PATIENTS AND METHODS: We will conduct a multicenter, single-blind randomized trial of 216 patients undergoing unilateral VATS surgery. Patients will be randomized 1:1 to receive either TEAS or

Indocyanine green-guided video-assisted thoracoscopic surgery thoracic duct ligation and talc pleurodesis for refractory chylothorax after Ivor Lewis oesophagectomy.

Refractory chylothorax following oesophagectomy is a challenging complication, often requiring surgical re-intervention. We describe a standardized technique for thoracic duct ligation using indocyanine green (ICG) fluorescence guidance. A 62-year-old male presented with high-output chylothorax 40 days after a minimally invasive Ivor Lewis oesophagectomy, refractory to conservative management. The procedure involved pre-operative bilateral inguinal lymph node injection of ICG. A right-sided three-port VATS approach was utilized. Intra-operatively, a provocative fatty meal test (cream and olive

Perioperative immersive virtual reality intervention for acute and persistent postoperative pain after video-assisted thoracoscopic surgery: A randomized controlled trial.

BACKGROUND: Postoperative pain remains a major barrier to recovery after video-assisted thoracoscopic surgery (VATS) and may progress to persistent postoperative pain. We evaluated whether perioperative immersive virtual reality (VR) could reduce acute and longer-term pain after VATS. METHODS: A total of 120 patients undergoing elective VATS were randomized 1:1 to a VR group or a control group. The VR group received immersive VR from 1 day before surgery to postoperative days (POD) 1-3, followed by a 2-week home-based intervention. The primary outcome was pain scores during coughing over POD
