Inflammatory vitiligo in Vogt-Koyanagi-Harada disease.

Vogt-Koyanagi-Harada disease is a rare disease characterized by uveitis, meningitis, dysacusis, alopecia, poliosis, and vitiligo. We describe a 48-year-old patient with Vogt-Koyanagi-Harada disease associated with thin inflammatory raised erythema and plaque-type inflammatory erythema superimposed on vitiligo. Interestingly, inflammatory raised erythema was separated from the perfect vitiligo, and the incomplete vitiligo lay between them initially. Thereafter, incomplete vitiligo became completely depigmented with diminution of inflammatory raised erythema. This is the second case of vitiligo …

Orbital myositis, vitiligo, and giant cell myocarditis.

Giant cell myocarditis is a rare idiopathic inflammatory heart disease characterized histologically by multinucleated giant cells, and clinically by rapid progressive heart failure, arrhythmias, or sudden death, often within hours to days of initial symptoms. There are two previously reported cases of giant cell myocarditis with idiopathic orbital myositis. We report a similar case in a patient who also had vitiligo, a diagnostic endomyocardial biopsy, and survival because of a cardiac transplant. Giant cell myocarditis should be monitored for in the course of inflammatory orbital myopathy bec…

Vitiligo patterns simulating autoimmune and rheumatic diseases.

Several variants of generalised vitiligo can be recognised by their cutaneous distribution. In some, certain anatomical regions--e.g., hands--are predominantly affected while the cutaneous depigmentation in other variants shows a similar anatomical distribution to that of the internal structures affected in some of the autoimmune and rheumatic diseases. Five patients have been seen, and three described by others, with vitiligo affecting skin of eyelids and lower front of neck, reminiscent of the anatomical distribution of lesions in thyrotoxicosis with exophthalmos: two of these patients have …

Prescribing and using vitiligo treatments: lessons from a nested process evaluation within the HI-Light vitiligo randomized controlled trial.

BACKGROUND: The HI-Light Trial demonstrated that for active, limited vitiligo, combination treatment with potent topical corticosteroid (TCS) and handheld narrowband ultraviolet B offers a better treatment response than potent TCS alone. However, it is unclear how to implement these findings. AIM: We sought to answer three questions: (i) Can combination treatment be used safely and effectively by people with vitiligo?; (ii) Should combination treatment be made available as routine clinical care?; and (iii) Can combination treatment be integrated within current healthcare provision? METHODS: …

Report of a case of radiation-induced new-onset vitiligo with collective review of cases in the literature of radiation-related vitiligo.

Radiation-induced hypopigmentation consistent vitiligo has been reported in a few case reports. We report herewith a case of vitiligo at the site of radiation delivery after a lag of several months in a patient with preexisting hypothyroidism without a previous or family history of vitiligo, and review the cases reported in the literature collectively.

Future research into the treatment of vitiligo: where should our priorities lie? Results of the vitiligo priority setting partnership.

BACKGROUND: Vitiligo is the most frequent depigmentation disorder of the skin and is cosmetically and psychologically devastating. A recently updated Cochrane systematic review 'Interventions for vitiligo' showed that the research evidence for treatment of vitiligo is poor, making it difficult to make firm recommendations for clinical practice. OBJECTIVES: To stimulate and steer future research in the field of vitiligo treatment, by identifying the 10 most important research areas for patients and clinicians. METHODS: A vitiligo priority setting partnership was established including patients…

Vitiligo Successfully Treated With Roflumilast Cream 0.3.

Vitiligo is an autoimmune cutaneous depigmenting disorder that can have significant psychological impacts on affected patients, posing a burden to daily life, which can be even more apparent in patients with deeper Fitzpatrick skin tones. Here we report the case of an adult patient with skin of color treated with roflumilast cream 0.3% following an unsatisfactory response to typical treatments. Treatment resulted in almost full repigmentation on the forehead and cheeks. This case suggests that roflumilast cream 0.3% may have a positive therapeutic effect for patients with vitiligo who are unre…

Deciphering Depigmentation: Mouse Models for Vitiligo Research.

Vitiligo is an autoimmune skin disease characterized by melanocyte loss and skin depigmentation. Although significant advances have been made in understanding the pathogenesis of vitiligo, the development of novel treatments has depended and will likely continue to depend on preclinical animal model research. This review provides a comprehensive overview of established mouse models for vitiligo, categorizing them on the basis of their mechanisms of melanocyte loss, immune involvement, and relevance to human disease. Finally, we explore emerging areas in vitiligo research, emphasizing the need …
