Squaring the round: An unusual progressive graphomotor impairment with post-mortem findings.

Investigations of neurodegenerative disorders may reveal functional relationships in the cognitive system. C.S. was a 63-year-old right-handed man with post-mortem confirmed Pick's disease with a range of progressive impairments including non-fluent aphasia, speech, limb, oculomotor, and buccofacial apraxia, but mostly intact intelligence, perception, orientation, memory, semantics, and phonology. During progression, agrammatism in writing with impairments in syntactic comprehension emerged in parallel with an unusual graphomotor deficit in drawing and writing, with an increasing deterioration

Significance of finger forces and kinematics during handwriting in writer's cramp.

Muscular hyperactivity during handwriting, irregular and jerky scripts, as well as awkward and slowed pen movements are the cardinal symptoms of writer's cramp. Accordingly, impaired kinematics and increased force have been reported in writer's cramp. However, the relationship between these symptoms has rarely been investigated. In addition, measurements of finger forces have been restricted to the vertical pen pressure. In the present study, the pen of a graphic tablet was equipped with a force sensor matrix to measure also the grip force produced against the pen barrel despite highly variabl

Task Specific Tremors.

A patient reported bilateral hand tremors when writing but not with other tasks. These "task specific" tremors are considered subcategories of essential tremor. Primary writing tremor, in which the tremor occurs only with writing, is probably the most common. The important teaching point is that the "standard" tremor assessment, watching the patient holding a sustained posture and touching his finger to the examiner's and then back to the nose is not adequate. Patients should be tested doing the activity that causes them the most difficulty.

Pilot Randomized Controlled Trial of a Syndemics Intervention with HIV-Positive, Cocaine-Using Women.

This pilot randomized controlled trial examined the feasibility and acceptability of a Syndemics intervention targeting the intersection of stimulant use, trauma, and difficulties with HIV disease management in cocaine-using women. All participants received contingency management (CM) for 3 months with financial incentives for stimulant abstinence during thrice-weekly urine screening and refilling antiretroviral medications monthly. Sixteen participants were randomized to complete four expressive writing (n = 9) or four neutral writing (n = 7) sessions delivered during the CM intervention peri

Orthographic influences on division of labor in learning to read Chinese and English: Insights from computational modeling.

Learning to read any language requires learning to map among print, sound and meaning. Writing systems differ in a number of factors that influence both the ease and rate with which reading skill can be acquired, as well as the eventual division of labor between phonological and semantic processes. Further, developmental reading disability manifests differently across writing systems, and may be related to different deficits in constitutive processes. Here we simulate some aspects of reading acquisition in Chinese and English using the same model architecture for both writing systems. The cont

Frontal pure agraphia for kanji or kana: dissociation between morphology and phonology.

We present two patients with frontal pure agraphia more impaired for either kanji or kana (two separate writing systems for the Japanese language). The lesion of patient 1 (preferentially disturbed for kanji) was restricted to the foot of the middle frontal gyrus and the adjacent anterior precentral gyrus, whereas the lesion of patient 2 (preferentially disturbed for kana) included the posterior two thirds of the middle frontal gyrus. Both patients made agraphic errors (impaired recall) for kanji and agraphic or paragraphic errors (changing into other symbols) for kana. The double dissociation

[Neurolinguistic features during recovery of a Chinese patient with pure alexia].

OBJECTIVE: To analyze the neurolinguistic features of a Chinese patient with pure alexia in acute and convalescent stages. METHODS: We assessed the reading and writing abilities of the patient with the Aphasia Battery of Chinese (ABC), the reading examination of Chinese characters (1999, Lin) and the Chinese agraphia battery (CAB). RESULTS: In the ABC examination in the acute phase, the patient performed well in oral expression and comprehension, and the prominent linguistic abnormalities were alexia and merging agraphia; in the convalescent phase, the recovery of alexia was better than that

Dysgraphia in patients with primary lateral sclerosis: a speech-based rehearsal deficit?

The present study aims to demonstrate that errors when writing are more common than expected in patients affected by primary lateral sclerosis (PLS) with severe dysarthria or complete mutism, independent of spasticity. Sixteen patients meeting Pringle's et al. [34] criteria for PLS underwent standard neuropsychological tasks and evaluation of writing. We assessed writing abilities in spelling through dictation in which a set of words, non-words and short phrases were presented orally and by composing words using a set of preformed letters. Finally, a written copying task was performed with the

Effect of subject state on auditory brainstem response threshold using Kalman-weighted averaging.

OBJECTIVES: This study aims to explore the impact of a subject's testing state on auditory brainstem response (ABR) thresholds using a novel ABR system (Vivosonic Integrity™), which incorporates Kalman-weighted averaging and bluetooth electrical isolation to address the limitation of conventional ABR limitation to obtain a stable result under non-sedated conditions, especially for infants and children. METHOD: Twenty-four adults (18-34 years old, 48 ears) with normal hearing were enrolled for ABR testing under three different states (lying quietly in the supine position or sleeping-lying; wat

Not all prolific letter writers are alike.

Concerns have recently been raised that AI-assisted "letter-bombing" may distort medical publishing. Although I share these concerns, prolific Letter writing can arise for legitimate reasons. Using my own publication history as an example, I illustrate how retirement, loss of access to primary data, and broader academic interests naturally shifted my publications from original articles toward Letters. The problem lies not in writing many Letters, but in inappropriate motivations and misuse of AI.

Anatomizing the pulse: Edmund King's analogy, observation and conception of the tubular body.

In an unpublished anatomical treatise written around 1670, the English anatomist and fellow of the Royal Society of London Edmund King proposed that the human body was ultimately an assemblage of tubes and contained liquids. Without literally seeing every of its constituents to be tubular, how did King come to posit a tubular body? This article tackles the question by examining King's inquiry about the pulse against his framing of the circulatory system into a universally tubular model. Asking how King registered this model despite the limited visibility of vascularity in practice, I discuss t

Generalizability of pause times in sentence production to distinguish between adult writers.

Researchers often decide on the number of trials included in an experiment without adhering to an empirical method or framework. This might compromise generalizability and unnecessarily increase participant burden. In this article we want to put forward generalizability theory as a guide for task reduction. We will use a sentence production task to demonstrate how a generalizability and a decision study can help researchers to estimate the minimum number of trials and of items per trial that are necessary to generalize over trials. We obtained writing process data for 116 participants. Each of

A Case of Clinical Scholarship.

A 32-year-old child and adolescent psychiatry resident with a history of presenting an abstract at a local psychiatric conference during residency and fourth authorship on an immunotherapy paper from a summer internship during medical school presents with a new-onset desire to write a case report. She has just come off her consultation liaison rotation, during which she consulted on a 12-year-old boy with attention-deficit/hyperactivity disorder, oppositional defiant disorder, disruptive mood dysregulation disorder, and low depressive state hospitalized for pseudoseizures, a new-onset inabilit

Assessment of whether use of a professional medical writer is associated with time to publication of papers reporting phase III oncology clinical trials in two high-impact general medicine journals.

OBJECTIVE: Professional medical writers assist authors with ethical and accurate publication of scientific research. We hypothesized that the support of a medical writer would enable timely publication of research and explored their association using papers reporting phase III oncology trials from 2019-2023. Given the time frame of this research, we also assessed the impact of the coronavirus disease 2019 (Covid-19) pandemic on publication volume as a secondary objective. METHODS: We searched PubMed for phase III clinical trials published in New England Journal of Medicine (NEJM) and the Lanc

Dyslexia and dysgraphia of primary progressive aphasia in Chinese: A systematic review.

INTRODUCTION: Currently, little is known about Chinese-speaking primary progressive aphasia (PPA) patients compared to patients who speak Indo-European languages. We examined the demographics and clinical manifestations, particularly reading and writing characteristics, of Chinese patients with PPA over the last two decades to establish a comprehensive profile and improve diagnosis and care. METHODS: We reviewed the demographic features, clinical manifestations, and radiological features of Chinese-speaking PPA patients from 56 articles published since 1994. We then summarized the specific re

Agraphia caused by acute right parietal infarction.

Injury in the dominant language hemisphere typically leads to agraphia, however we report a patient with agraphia after injury to the right angular gyrus. A 71-year-old Korean woman presented with the complaint of an inability to write for the last 7 days. The patient had been illiterate for most of her life, but had started learning to write Hangul, the Korean alphabet, at a welfare center 3 years ago. On language screening she was unable to write although she could read, and other language functions showed no abnormalities. Brain MRI showed acute infarction in the right angular gyrus. Her wr
