Perioperative lesions of the facial nerve: follow-up investigations using transcranial magnetic stimulation.

Peripheral facial palsy can occur after aural surgery and neurosurgery. Routine neurophysiological investigation (utilizing electrical stimulation and the blink reflex) does not allow the direct assessment of the site of a lesion. In the present study transcranial magnetic stimulation (TMS) was applied in order to evaluate the usefulness of this method for prognosis. Twenty-three patients with postoperative facial pareses (after removals of an acoustic neuroma in 12 patients and parotid tumors in 11) were investigated. Ipsilateral short-latency and contralateral long-latency responses (after c

Electrical stimulation of the amygdala increases the amplitude of elicited ponto-geniculo-occipital waves.

The amygdala projects massively via its central nucleus (CNA) into brain stem regions involved in alerting and ponto-geniculo-occipital (PGO) wave generation. Electrical stimulation of CNA is known to enhance the acoustic startle response (ASR) and influence spontaneous PGO waves. The role of the amygdala in the modulation of ASR and elicited PGO waves (PGOE) was investigated in albino rats. Electrically stimulating CNA within 25 ms prior to an auditory stimulus enhanced ASR and PGOE amplitude in a similar way, with the largest response occurring when the electrical and auditory stimuli were g

Evidence for tinnitus-related plasticity in the auditory and limbic system, demonstrated by arg3.1 and c-fos immunocytochemistry.

Distributions of arg3.1 and c-fos immunoreactive neurons (IRN) in gerbil auditory cortex (AC) and amygdala showed characteristic differences when comparing systemic application of the tinnitus-eliciting drug salicylate with acoustic stimulation or saline injections. In AC, arg3.1 IRN induced by stimulation focused in regions corresponding to the frequency content of the stimulus. Injections of salicylate (350 mg/kg body weight) led to accumulation of arg3.1 IRN in the high frequency domain, while saline injections produced a diffuse distribution. After all treatments, c-fos IRN outnumbered arg…

Amplitude Modulated Noise for Tinnitus Suppression in Tonal and Noise-Like Tinnitus.

BACKGROUND: The phenomenon of short-term tinnitus suppression by different forms of acoustic stimulation is referred to as residual inhibition (RI). RI can be triggered in the majority of tinnitus cases and was found to be depending on the used intensity, length or types of sounds. Past research already stressed the impact of noise stimulation as well as the superiority of amplitude modulated (AM) pure tones at the individual tinnitus frequency for RI in tonal tinnitus. Recently a novel approach for the determination of noise-like tinnitus characteristics was proposed. OBJECTIVES: The aim of …

Analysis of the Effect of Musical Stimulation on Cortical Auditory Evoked Potentials.

Introduction  Cortical auditory evoked potentials (CAEPs) are bioelectric responses that occur from acoustic stimulations, and they assess the functionality of the central auditory system. Objective  The objective of the present study was to analyze the effect of musical stimulation on CAEPs. Methods  The sample consisted of 42 healthy female subjects, aged between 18 and 24 years, divided into two groups - G1: without musical stimulation prior to the CAEP examination; and G2: with stimulation prior to the examination. In both groups, as a pre-collection procedure, the complete basic audiologi…

Inhibition of intracranial self-stimulation in brain stem-transected cats--a proposed mechanism of aversive effects produced by brain stimulation.

Effects of intracranial self-stimulation of central 'punishment areas' were studied on an operant conditioning of vertical eye movements in the midpontine pretrigeminal cats as well as in the encéphale isolé cats. In 36 pretrigeminal cats, the ventromedial hypothalamus (VMH), basal amygdaloid nuclei (AMY), dorsal central gray (CG) of the midbrain and the thalamic nuclei such as the ventralis posteromedialis (VPM) and ventralis posterolateralis (VPL) were tested. No suppression of eye movements indicating a passive avoidance conditioning from stimulation of these 'punishment areas' was obtained

Focused ultrasound to bilateral thalamic nuclei significantly improves human cognitive attention in a frequency- and intensity-dependent manner.

BACKGROUND: The centromedian nucleus of the thalamus (CMT) is a core arousal center with potential for enhancing attention and alertness. While emergent low intensity focused ultrasound (LIFU) can reach this area, the stimulation paradigms which may enhance function in humans remain unknown. METHODS: In a within-subject, double-blind, sham-controlled paradigm, we administered bilateral stimulation of the CMT in N = 16 healthy adult participants using a novel wearable focused ultrasound neuromodulation device and 3 distinct pulse frequencies (1Hz, 5Hz, and 25Hz) and a diffuse unfocused sham st

Prognostic significance of intraoperative facial nerve stimulus thresholds.

OBJECTIVE: Intraoperative facial nerve monitoring has reduced the incidence of facial nerve paralysis associated with acoustic neuroma surgery, but poor facial nerve outcomes continue to occur. Intraoperative prediction of facial nerve outcome would be advantageous in patient management and counseling. This study seeks to evaluate intraoperative facial nerve stimulus thresholds as a tool for predicting postoperative facial nerve outcome. STUDY DESIGN: This study is a prospective clinical study of the prognostic value of intraoperative stimulus thresholds. SETTING: The study was performed at

LN-PMUT ultrasonic transducer enables instantaneous blood pressure reduction via renal hilar implantation.

Blood pressure (BP) is a fundamental vital sign, yet no implantable device for BP modulation is currently available. Here, we present a miniaturized ultrasound probe based on lead-free lithium niobate (LN) piezoelectric micromachined ultrasonic transducer (PMUT) technology. The device features a dual-electrode design offering enhanced emission sensitivity and acoustic output, while maintaining a compact size (3 × 3 mm2, 3 mg), wide bandwidth (∼32%), and 2.5 MHz resonant frequency. Unilateral placement and vibratory stimulation at the renal hilum induced an immediate reduction in BP. Daily stim

Effect of Aging on Middle Ear Absorbance Characteristics Across Frequencies.

PURPOSE: Although age-related auditory changes are well recognized, limited studies have examined how aging affects middle ear mechanics, which can be assessed using wideband tympanometry (WBT). Unlike conventional tympanometry, WBT offers frequency-specific insights into middle ear acoustic transfer function. Ethnic differences in the anatomy of the external and middle ear such as canal length, ossicular dimensions, and stapes configuration may influence acoustic transmission. This study aimed to investigate the effect of aging on middle ear absorbance across frequencies using WBT. METHOD: A

The Influence of Vowel Identity, Vowel Production Variability, and Consonant Environment on Envelope Following Responses.

OBJECTIVES: The vowel-evoked envelope following response (EFR) is a useful tool for studying brainstem processing of speech in natural consonant-vowel productions. Previous work, however, demonstrates that the amplitude of EFRs is highly variable across vowels. To clarify factors contributing to the variability observed, the objectives of the present study were to evaluate: (1) the influence of vowel identity and the consonant context surrounding each vowel on EFR amplitude and (2) the effect of variations in repeated productions of a vowel on EFR amplitude while controlling for the consonant

Audiological evaluation of patients with cleidocranial dysplasia (CCD).

OBJECTIVE: The description of clinical manifestation of hearing problems in cleidocranial dysplasia (CCD) remains limited and incomplete, since CCD constitutes a rare congenital disorder. The study aims to provide a complex panel analysis of the auditory system in patients suffering from the disease. PATIENTS AND METHODS: The study group consisted of 4 children with CCD (aged: 12-15), who underwent orthodontic treatment. A full panel analysis of their auditory systems was performed, including high-frequency audiometry and a new method of middle ear assessment - WBT (Wideband Tympanometry). R

Auditory brainstem response.

The auditory brainstem response (ABR), consisting of five to six vertex-positive peaks with separation of about 0.8ms, is very sensitive to factors that affect conduction velocity and hence ABR wave latencies in the brainstem auditory pathways. In addition, disorders causing dissynchronization of neural activity result in an amplitude decrease or disappearance of ABR peaks. The opposite effects occur in the maturation process, which takes about 2 years postterm; here conduction velocity increases quickly to its adult value, but synaptic delays being sensitive to synchronous release of transmit

Objective measurement of subjective tinnitus using the acoustic change complex.

At present, there is no objective method for diagnosing subjective sensorineural tinnitus. Recently, the acoustic change complex (ACC) has been used to evaluate neural detection of sounds. Thus, the present study aimed to examine whether the ACC can reflect cortical detection and discrimination of sounds matched with tinnitus frequencies. We hypothesized that the ACC to change stimuli matched with tinnitus frequencies would be decreased in tinnitus patients because the tinnitus interferes with the perception of acoustic changes. To test the hypothesis, 96 ears of normal-hearing (NH) tinnitus p

Voluntary eardrum movement: a marker for tensor tympani contraction?

HYPOTHESIS: Voluntary eardrum movement (VEM) and resultant tympanometric changes reflect tensor tympani (TT) contraction. BACKGROUND: TT contraction has been hypothesized to cause symptoms of aural fullness, tinnitus, clicking, and even vertigo despite the lack of understanding of how it functions or what causes it to contract. Identifying tympanometric changes unique to TT contraction can provide a diagnostic tool for identifying its role in pathologic conditions. METHODS: Various tympanometric measurements were performed on human subjects who could voluntarily move their eardrums. These we

Audiogram of the chicken (Gallus gallus domesticus) from 2 Hz to 9 kHz.

The pure-tone thresholds of four domestic female chickens were determined from 2 Hz to 9 kHz using the method of conditioned suppression/avoidance. At a level of 60 dB sound pressure level (re 20 μN/m(2)), their hearing range extends from 9.1 Hz to 7.2 kHz, with a best sensitivity of 2.6 dB at 2 kHz. Chickens have better sensitivity than humans for frequencies below 64 Hz; indeed, their sensitivity to infrasound exceeds that of the homing pigeon. However, when threshold testing moved to the lower frequencies, the animals required additional training before their final thresholds were obtained,

Input and output compensation for the cochlear traveling wave delay in wide-band ABR recordings: implications for small acoustic tumor detection.

BACKGROUND: The Stacked ABR (auditory brainstem response) attempts at the output of the auditory periphery to compensate for the temporal dispersion of neural activation caused by the cochlear traveling wave in response to click stimulation. Compensation can also be made at the input by using a chirp stimulus. It has been demonstrated that the Stacked ABR is sensitive to small tumors that are often missed by standard ABR latency measures. PURPOSE: Because a chirp stimulus requires only a single data acquisition run whereas the Stacked ABR requires six, we try to evaluate some indirect evidenc

Wideband energy reflectance measurements of ossicular chain discontinuity and repair in human temporal bone.

OBJECTIVE: Recent studies have suggested the potential of wideband energy reflectance (ER) for the assessment of middle ear disorders. The purpose of this study was to examine the use of wideband ER in the evaluation of an ossicular discontinuity and its repair in human cadaver temporal bones. DESIGN: ER measurements at ambient pressure were made on five human cadaver ears in three conditions: (1) baseline with intact ossicular chain, (2) with the ossicular chain cut using an argon laser, and (3) with the ossicular chain repaired. Measurement of the distribution of medial olivocochlear acoustic reflex strengths across normal-hearing individuals via otoacoustic emissions.

A clinical test for the strength of the medial olivocochlear reflex (MOCR) might be valuable as a predictor of individuals at risk for acoustic trauma or for explaining why some people have trouble understanding speech in noise. A first step in developing a clinical test for MOCR strength is to determine the range and variation of MOCR strength in a research setting. A measure of MOCR strength near 1 kHz was made across a normal-hearing population (N = 25) by monitoring stimulus-frequency otoacoustic emissions (SFOAEs) while activating the MOCR with 60 dB SPL wideband contralateral noise.
