Dynamics of cochlear synaptopathy after acoustic overexposure.

Recent work shows that acoustic overexposures causing only transient threshold elevation, and no hair cell loss, nevertheless can cause irreversible loss of the synapses between inner hair cells and cochlear nerve fibers (Kujawa and Liberman 2009). This cochlear synaptopathy, which is selective for the subset of sensory fibers with high thresholds and low spontaneous rates (Furman et al. 2013), appeared fully developed at 24-h post-exposure and showed no recovery by 8 weeks. However, prior studies of this synaptopathy counted only pre-synaptic ribbons, did not examine post-exposure times less

Beyond auscultation: acoustic cardiography in clinical practice.

Cardiac auscultation by stethoscope is widely used but limited by low sensitivity and accuracy. Phonocardiogram was developed in an attempt to provide quantitative and qualitative information of heart sounds and murmurs by transforming acoustic signal into visual wavelet. Although phonocardiogram provides objective heart sound information and holds diagnostic potentials of different heart problems, its examination procedure is time-consuming and it requires specially trained technicians to operate the device. Acoustic cardiography (AUDICOR, Inovise Medical, Inc., Portland, OR, USA) is a major

Audiological correlates of tumor parameters in acoustic neuromas.

The objective of this study was undertaken to evaluate a correlation of pure tone audiometry, speech audiometry with tumor size, the presence of intrameatal extension, contralateral pure tone audiometry, and contralateral speech audiometry results at acoustic neuroma (AN) patients. Seventy-six patients with a unilateral-cerebellopontine angel tumor with/without internal acoustic canal (IAC) were included in this study. Data on pure tone audiometry, speech discrimination scores (SDS), speech reception thresholds (SRT), tumour size, and extension to IAC were analyzed retrospectively. As a result

Role of tobacco use in the etiology of acoustic neuroma.

Two previous studies suggest that cigarette smoking reduces acoustic neuroma risk; however, an association between use of snuff tobacco and acoustic neuroma has not been investigated previously. The authors conducted a case-control study in Sweden from 2002 to 2007, in which 451 cases and 710 population-based controls completed questionnaires. Cases and controls were matched on gender, region, and age within 5 years. The authors estimated odds ratios using conditional logistic regression analyses, adjusted for education and tobacco use (snuff use in the smoking analysis and smoking in the snuf

Gait instability in patients with small acoustic neuroma.

BACKGROUND: Small acoustic neuromas seldom result in typical vestibular symptoms, despite the tumor arising from the vestibular nerve. In this study, we have shown that abnormal gait in eleven patients with small acoustic neuroma could be detected in gait analysis by the use of tactile sensor. Patients displayed no oculomotor abnormality and had tumors less than 10 mm from the porus acoustics. METHODS: Gait related parameters including the coefficients of variations (CV) of stance, swing, double support, area ratio of trajectories of center of force (TCOF), in addition to the foot pressure di…

Acoustic and physiologic aspects of bone conduction hearing.

Bone conduction (BC) is the way sound energy is transmitted by the skull bones to the cochlea causing a sound perception. Even if the BC sound transmission involves several pathways including sound pressure induced in the ear canal, inertial forces acting on the middle ear ossicles and cochlear fluids, alteration of the cochlear space, and pressure transmission through the 3rd window of the cochlea, the BC sound ultimately produces a wave motion on the basilar membrane similar to that of air-conducted sound. The efficiency of the BC stimulation is largely dependent on the skull bone where the

Locus equations are an acoustic expression of articulator synergy.

The study investigated the articulatory basis of locus equations, regression lines relating F2 at the start of a Consonant-Vowel (CV) transition to F2 at the middle of the vowel, with C fixed and V varying. Several studies have shown that consonants of different places of articulation have locus equation slopes that descend from labial to velar to alveolar, and intercept magnitudes that increase in the opposite order. Using formulas from the theory of bivariate regression that express regression slopes and intercepts in terms of standard deviations and averages of the variables, it is shown th

A generalized smoothness criterion for acoustic-to-articulatory inversion.

The many-to-one mapping from representations in the speech articulatory space to acoustic space renders the associated acoustic-to-articulatory inverse mapping non-unique. Among various techniques, imposing smoothness constraints on the articulator trajectories is one of the common approaches to handle the non-uniqueness in the acoustic-to-articulatory inversion problem. This is because, articulators typically move smoothly during speech production. A standard smoothness constraint is to minimize the energy of the difference of the articulatory position sequence so that the articulator traject

Herpes and acoustic neuromas: is there a cause and effect to observe?

Acoustic neuromas are a result of damage to the affected nerve function and can potentially press surrounding tissues. Although some sources suggest that observation is the treatment of choice for only those over 65 years of age and those unable to tolerate undergoing surgery or radiosurgery, most affected individuals should strongly consider not doing any aggressive therapies. Herpes has already been shown to mimic acoustic neuroma clinically, but growing evidence suggests that it is likely the cause of most cases of this entity.

Magnetic resonance imaging screening in acoustic neuroma.

OBJECTIVES: Magnetic resonance imaging (MRI) is the definitive investigation for detection of an acoustic neuroma. It is however an expensive resource, and pick-up rate of a tumor can be as low as 1% of all patients scanned. This study aims to examine referral patterns for MRI screening for patients presenting with asymmetrical sensorineural hearing loss (ASHL). A second aim was to suggest appropriate screening criteria. METHOD: All 132 MRI scans performed for ASHL in the year 2005 were reviewed retrospectively along with their case records and audiograms. In addition, MRI scans and case reco

Surgical management of large acoustic neuromas: a review.

Acoutic neuromas operated at UMMC from 2001 to 2006 were retrospectively reviewed. There were a total of 27 cases. All tumors were large, measuring more than 2 cm. Hearing loss was the most common presenting symptom (63%), followed by headache (52%), dysequilibrium (30%), facial numbness (30%), tinnitus (26%) and gait disturbances (15%). Eleven (41%) of patients had hydrocephalus at the time of presentation, for which a shunt procedure was required. The translabrynthine (TL) approach was used for 12 patients and the retrosigmoid (RS) with or without presigmoid clearance for the remaining 15. M

Translabyrinthine approach for acoustic tumor removal. 1992.

The translabyrinthine approach is the most direct route to the cerebellopontine angle. It is the preferred approach for removal of all tumors in patients with poor hearing and for large tumors when the likelihood of hearing preservation is slight. This approach offers the advantages of minimum cerebellar retraction, identification of the facial nerve proximally and medially, and the ability to repair immediately the facial nerve if it is severed during acoustic tumor removal. This approach has the lowest morbidity with regard to spinal fluid leaks and also postoperative headaches.

Occupational noise exposure and risk of acoustic neuroma.

A small number of prior epidemiologic studies of occupational noise exposure based on self-report have suggested an association with acoustic neuroma. The goal of the present study was to further examine the association between noise exposure and acoustic neuroma by using an objective measure of exposure in the form of a job exposure matrix. A total of 793 acoustic neuroma cases aged 21-84 years were identified between 1987 and 1999 from the Swedish Cancer Registry. The 101,756 controls randomly selected from the study base were frequency matched to cases on age, sex, and calendar year of diag
