Localized Delayed T-cell Mediated Hypersensitivity After mRNA SARS-CoV-2 Vaccination.

Since the emergency authorization of SARS-CoV-2 vaccines, the medical literature has been investigating the management of allergic reactions to the vaccines. Anaphylaxis has been reported among a minority of vaccinated individuals, and many trials monitoring the safety profile of the vaccines have identified cases of benign cutaneous reactions. Typical features of delayed benign cutaneous hypersensitivity reactions include localized erythema, pruritus, and rash. However, reports have described rare cases of rash and atopy at sites apart from the injection site following vaccine delivery. We wi

Long-term cefuroxime axetil in subacute cutaneous lupus erythematosus. A report of three cases.

BACKGROUND: Subacute cutaneous lupus erythematosus (SCLE) is a subset of lupus erythematosus characterized mainly by prominent photoaggravated cutaneous manifestations. Standard therapies for SCLE include topical or systemic steroids and antimalarial drugs. Both methods show limited efficacy in clearing cutaneous lesions and occasionally produce serious side effects. AIM: To assess the efficacy of cefuroxime axetil, an oral cephalosporin with antibacterial and immunosuppressive activity, in patients with SCLE. METHODS: Three patients with SCLE were treated with cefuroxime axetil at a daily d

Tetrodotoxin Delivery Pen Safely Uses Potent Natural Neurotoxin to Manage Severe Cutaneous Pain.

Clinically available therapies often inadequately address severe chronic cutaneous pain due to short anesthetic duration, insufficient intensity, or side effects. This study introduces a pen device delivering tetrodotoxin (TTX), a potent neurotoxin targeting nerve voltage-gated sodium channels, as a safe and effective topical anesthetic to treat severe chronic cutaneous pain. Chemical permeation enhancers, such as sodium dodecyl sulfate (SDS) and limonene (LIM), are incorporated to enhance TTX skin permeability. The device ensures precise TTX dosing down to the nanogram level, essential to avo

Systemic Platelet-Activating Factor-Receptor Agonism Enhances Non-Melanoma Skin Cancer Growth.

Platelet-activating factor-receptor (PAF-R) agonists are pleiotropic lipid factors that influence multiple biological processes, including the induction and resolution of inflammation as well as immunosuppression. PAF-R agonists have been shown to modulate tumorigenesis and/or tumor growth in various skin cancer models by suppressing either cutaneous inflammation and/or anti-tumoral adaptive immunity. We have previously shown that a chronic systemic PAF-R agonist administration of mice enhances the growth of subcutaneously implanted melanoma tumors. Conversely, chronic topical applications of

Glutathione metabolism in mice is enhanced more with hapten-induced allergic contact dermatitis than with irritant contact dermatitis.

Cutaneous inflammation induced by electrophilic compounds involves irritant contact dermatitis (ICD) and allergic contact dermatitis (ACD). Reduced glutathione (GSH) and related thiols have been postulated to play important roles in detoxification of electrophilic xenobiotics, protection of tissues against reactive oxygen species, and modulation of immunologic functions in normal and diseased subjects. The dynamic aspects of GSH metabolism, however, and its significance in patients with ICD and ACD remain to be clarified. The current study was carried out to elucidate the pathogenesis and poss

New amphotericin B-gamma cyclodextrin formulation for topical use with synergistic activity against diverse fungal species and Leishmania spp.

Amphotericin B (AmB) has a broad antifungal and leishmanicidal activity with low incidence of clinical resistance. Its parenteral administration has high risk of nephrotoxicity that limits its use. In order to treat cutaneous infections, AmB topical administration is a safer therapy because of the low systemic absorption of the drug across mucous membranes. Moreover, in some developing countries both fungal topical infections and cutaneous leishmaniasis are an important health problem. The aim of this work is to formulate a topical amphotericin preparation and test its in vitro antifungal (aga

Efficacy of topical 2% carteolol in the treatment of infantile hemangiomas: a dermoscopic and clinical evaluation.

Infantile hemangiomas (IH) are the most common benign vascular tumors in infants, often requiring treatment in 10-15% of cases due to complications. While systemic propranolol is the first-line treatment, its potential for systemic side effects raises interest in topical therapies. This study investigates the efficacy of topical 2% carteolol in treating IH and its dermoscopic changes over time. A total of 15 patients with IH were treated with topical 2% carteolol drops applied twice daily for 3 months. Clinical and dermoscopic images were captured at baseline, 1 month, and 3 months. Dermoscopi

An update on nanocarriers for follicular-targeted drug delivery for androgenetic alopecia topical treatment.

INTRODUCTION: Androgenic alopecia is a multifactorial disease with a high incidence and a great psychological burden on patients. The current FDA-approved treatment is topical minoxidil or oral finasteride. However, both present significant limitations. While the systemic absorption of finasteride causes serious sexual side effects, minoxidil's low solubility imposes a challenge in obtaining a non-irritative and effective formulation. One way to solve such limitations is by using nanocarriers targeting the drug delivery to the hair follicles upon topical application. AREAS COVERED: Here, we r

Contact Dermatitis Caused by Topical Minoxidil: Allergy or Just Irritation.

Topical minoxidil is the mainstay of treatment for androgenic alopecia and is also used off-label for other forms of hair loss. Despite its efficacy and favourable safety profile, the use of minoxidil is associated with various side effects, the most commonly reported of which is contact dermatitis. A clear distinction between allergic contact dermatitis and irritant contact dermatitis to minoxidil is critical for management of androgenic alopecia. This article presents a systematic review of the current literature, evaluating minoxidil-induced allergic contact dermatitis. Of the 251 records i

Topical delivery of insulin using novel organogel formulations: An approach for the management of diabetic wounds.

Diabetes mellitus is a growing chronic form of diabetes, with lengthy health implications. It is predicted as poor diabetic wound recovery affects roughly 25% of all diabetes mellitus patients, frequently resulting in lower traumatic injury and severe external factors and emotional expenses. The insulin-resistant condition increases biofilm development, making diabetic wounds harder to treat. Nowadays, medical treatment and management of diabetic wounds, which have a significant amputation rate, a high-frequency rate, and a high death rate, have become a global concern. Topical formulations ha

Patiens with psoriasis and mild to moderate skin disease can be treated with topical agents. This review finds that the most effective topical treatment is calcipotriol and betamethasone combination formulations. Pure topical corticosteroids continue to have a role in the treatment of psoriasis, as they are available in many strengths and formulations that facilitate ease of use. In addition, the price is often lower than for the combination formulations. Topical calcineurin inhibitors can treat facial and intertriginous psoriasis to reduce the risk of skin atrophy.

The clinical effects of zinc as a topical or oral agent on the clinical response and pathophysiologic mechanisms of acne: a systematic review of the literature.

This article reviews the published literature about the efficacy of oral and topical zinc as treatments for acne vulgaris. The medical literature was systematically reviewed to identify relevant articles. Each published study was assessed for pathophysiologic results and the quality of the clinical evidence the study provided based on Strength of Recommendation Taxonomy (SORT) criteria. Finally, the body of evidence for using oral or topical zinc in the treatment of acne was assessed, again using SORT criteria. A SORT strength of recommendation of B (inconsistent or limited-quality patient-ori

Interventions for actinic keratoses.

BACKGROUND: Actinic keratoses are a skin disease caused by long-term sun exposure, and their lesions have the potential to develop into squamous cell carcinoma. Treatments for actinic keratoses are sought for cosmetic reasons, for the relief of associated symptoms, or for the prevention of skin cancer development. Detectable lesions are often associated with alteration of the surrounding skin (field) where subclinical lesions might be present. The interventions available for the treatment of actinic keratoses include individual lesion-based (e.g. cryotherapy) or field-directed (e.g. topical) t
