Laryngeal tuberculosis in the United States of America: a forgotten disease.

Laryngeal tuberculosis (TB) was a common manifestation of TB in the early twentieth century, but now represents only 1% of all cases. Most modern case series of laryngeal TB originate outside the USA. We report a case of laryngeal TB from our institution and review other US cases published between 1970 and 2012. One hundred twenty-seven cases were identified. The mean patient age was 49 y and 28% were female. The mean duration of symptoms was 19 weeks. Dysphonia and weight loss were the most common manifestations, seen in 96% and 47% of cases, respectively. These symptoms were usually attribut

To live and die in America: labor in the time of cholera and cancer.

A popular explanation of the epidemiological transition is that the germs that caused infectious disease mortality were defeated by the "magic bullets" of mainstream medicine over the course of the 20th century, permitting the population to get old enough to get the chronic diseases of heart disease and cancer. This explanation is false. The most important causes of infectious disease were the political and economic structures that favored capital at the expense of labor so blatantly that it left a large portion of the working population virtually at death's door. This was remedied only when r

Socioeconomic determinants of cervical cancer screening in Latin America.

OBJECTIVE: To assess the impact of health care access and socioeconomic determinants on Pap smear screening in Latin America. METHODS: Individual-level data was collected from the Demographic and Health Surveys in Bolivia, Brazil, Dominican Republic, Ecuador, Guatemala, Nicaragua, Peru, and Trinidad and Tobago between 1987 and 2008. Multivariate logistic regression analyses were used to identify socioeconomic and health care determinants of two outcomes: knowledge of Pap smears and recent Pap smear screening. RESULTS: In all countries, the proportion of women with a recent Pap smear screenin

Laryngectomy rehabilitation: a perspective from the United States of America.

PURPOSE OF REVIEW: It is important on a periodic basis to evaluate and re-evaluate the standard of care in your area and to compare it to the care provided in other parts of the world. This can result in modifications and improvement in the patient's outcome. RECENT FINDINGS: Over the last year, additional weight has been added to the medical literature regarding the value of moisture exchangers, chronic low-risk medications, the relationship between vocal rehabilitation and esophageal reflux disease, as well as the chronic concern regarding tracheo-esophageal puncture site enlargement. Multi

[The cholera epidemic in Latin America: reemergence, morbidity, and mortality].

OBJECTIVE: To determine morbidity and mortality from cholera during different segments of the period 1991-2011 in the countries of Latin America. METHODS: Using information sources from a nonsystematic literature search for works on cholera epidemics, a mixed ecological study was conducted aimed at a time series analysis of morbidity, mortality, and case-fatality in cholera-related health events between 1991 and 2011 in 18 Latin American countries. RESULTS: During the period 1991-2011, 1 839 037 cases of cholera were reported in Latin America, with 19 538 deaths and a case fatality rate of 1

Contemporary results for proximal aortic replacement in North America.

OBJECTIVES: The purpose of this study was to characterize operative outcomes for ascending aorta and arch replacement on a national scale and to develop risk models for mortality and major morbidity. BACKGROUND: Contemporary outcomes for ascending aorta and arch replacement in North America are unknown. METHODS: We queried the Society of Thoracic Surgeons Database for patients undergoing ascending aorta (with or without root) with or without arch replacement from 2004 to 2009. The database captured 45,894 cases, including 12,702 root, 22,048 supracoronary ascending alone, 6,786 ascending plu

Dengue: an escalating public health problem in Latin America.

Dengue infection is a significant and escalating public health problem in Latin America. Its re-emergence and subsequent rise in the region over the past 50 years has largely been caused by a combination of a lack of political will, the radical growth of urban populations, migration flow and insufficient financial resources. Its increased incidence has been compounded by climate change, poor sanitation and extreme poverty, which lead to more breeding sites of the mosquito vector Aedes aegypti. In order to control dengue effectively, an integrated approach incorporating vector management and en

[Mayaro: a re-emerging Arbovirus in Venezuela and Latin America].

Mayaro virus produces nonspecific, sublethal disease symptoms, often confused with dengue, but with symptoms of arthalgias that can cause incapacitating disability. Outbreaks have been localized and sporadic in the Pan-Amazonia forest since its first isolation in 1954 (Trinidad and Tobago). The literature available is scarce, diverse and dispersed. Mayaro virus is an alphavirus, phylogenetically related to the Semliki forest antigenic complex. UNA and Mayaro viruses are the only viruses of this complex that have been isolated in the New World. Mayaro consists of single-stranded RNA of positive

Venezuelan equine encephalitis in a teenager visiting Central America.

BACKGROUND: Stupor, coma, and other alterations of consciousness are among the most serious life-threatening emergencies faced by the emergency department physician. When a patient arrives with altered mentation from Central or South America, the usual causes that occur in the United States must be considered; however, other unusual tropical disease must be excluded, such as Venezuelan equine encephalitis (VEE). OBJECTIVE: This study aimed to review the clinical features of VEE. CASE: A 17-year-old female traveled to Belize and developed vomiting, diarrhea, fever, headaches, and myalgias. Ov

Epidemiology of endemic systemic fungal infections in Latin America.

Although endemic mycoses are a frequent health problem in Latin American countries, clinical and epidemiological data remain scarce and fragmentary. These mycoses have a significant impact on public health, and early diagnosis and appropriate treatment remain important. The target population for endemic disease in Latin America is mostly represented by low-income rural workers with limited access to a public or private health system. Unfortunately, diagnostic tools are not widely available in medical centers in Latin America; consequently, by the time patients are diagnosed with fungal infecti

Placentation in Sigmodontinae: a rodent taxon native to South America.

BACKGROUND: Sigmodontinae, known as "New World rats and mice," is a large subfamily of Cricetidae for which we herein provide the first comprehensive investigation of the placenta. METHODS: Placentas of various gestational ages ranging from early pregnancy to near term were obtained for five genera, i.e. Necromys, Euryoryzomys, Cerradomys, Hylaeamys, and Oligoryzomys. They were investigated by means of histology, immunohistochemistry, a proliferation marker, DBA-lectin staining and transmission electron microscopy. RESULTS: The chorioallantoic placenta was organized in a labyrinthine zone, s

[Congenital malformations in Latin America in the period 1995-2008].

BACKGROUND: The Latin American Study of Congenital Malformations (ECLAMC) has performed an epidemiological surveillance of congenital malformations since 1967. This allows to detect any unexpected change in the incidence of malformations, possibly caused by a new environmental teratogenic agent. AIM: To report a summary of the results thus far obtained in this study. MATERIAL AND METHODS: The ECLAMC database was analyzed and all live births and stillbirths of more than 500 grams in the period 1995-2008, were analyzed. RESULTS: There were 2,409,407 births in the nine participant countries. O

Patient safety in obstetrics--the Hospital Corporation of America experience.

We report an update on obstetric patient safety efforts and results in the nation's largest obstetric health care delivery system. The application of principles advocated by the Institute of Medicine a decade ago has resulted in reduced adverse outcomes, as reflected by claims experience. Particular progress has been made in standardization and documentation of critical processes, establishment of national quality benchmarks, reduction in elective deliveries <39 weeks' gestation, and reduction in fatal postcesarean pulmonary embolism. Our experience provides a useful blueprint for similar prog

Malaria in selected non-Amazonian countries of Latin America.

Approximately 170 million inhabitants of the American continent live at risk of malaria transmission. Although the continent's contribution to the global malaria burden is small, at least 1-1.2 million malaria cases are reported annually. Sixty percent of the malaria cases occur in Brazil and the other 40% are distributed in 20 other countries of Central and South America. Plasmodium vivax is the predominant species (74.2%) followed by P. falciparum (25.7%) and P. malariae (0.1%), and no less than 10 Anopheles species have been identified as primary or secondary malaria vectors. Rapid deforest

Control of visceral leishmaniasis in latin america-a systematic review.

BACKGROUND: While three countries in South Asia decided to eliminate anthroponotic visceral leishmaniasis (VL) by 2015, its control in other regions seems fraught with difficulties. Is there a scope for more effective VL control in the Americas where transmission is zoonotic? We reviewed the evidence on VL control strategies in Latin America-diagnosis, treatment, veterinary interventions, vector control-with respect to entomological and clinical outcomes. METHODOLOGY/PRINCIPAL FINDINGS: We searched the electronic databases of MEDLINE, LILACS, and the Cochrane Central Register of Controlled Tr…

Canine leishmaniasis in North America: emerging or newly recognized?

Canine leishmaniasis is a fatal zoonotic visceralizing disease usually associated with tropical areas. The etiologic agent is an obligate intracellular protozoan, Leishmania infantum. In 1999, an outbreak of a canine leishmaniasis was reported in a Foxhound kennel in New York, and since that report, several other outbreaks have occurred across the United States in additional Foxhound kennels. Because of the high mortality and transmissibility associated with these outbreaks, it is essential that clinicians be aware of this disease to permit its rapid recognition and institution of control meas
