Generalized dissociative amnesia: episodic, semantic and procedural memories lost and found.

OBJECTIVE: This review tests Ribot's classic twofold categorization of generalized amnesia (GA) into Type I, total loss of episodic memory, and Type II, additional more or less extensive loss of semantic and/or procedural memory. It also explores his law of regression, according to which, cast in modern terms, recovery of lost procedural and semantic memories precedes recovery of episodic memory, as well as reported aetiological factors. METHOD: Clinically and formally assessed cases of GA, published since 1845, were surveyed and further analysed. RESULTS: Over and above authentic episodic m

[Ictal EEG pattern of transient epileptic amnesia in acute phase of non-herpetic limbic encephalitis].

A 60-year-old, right-handed woman was admitted to our hospital for amnesia as the only neurological abnormal findings following the autonomic symptoms and transient episodes of loss of awareness. EEG during the amnesia showed rhythmic alpha activity arising from the left mid-temporal region. Although this ictal activity showed evolution in the frequency and amplitude, the location was limited in the bilateral temporal areas. After the EEG evaluation, her amnesia was resolved immediately, suggesting that her presentation was transient epileptic amnesia (TEA). Meanwhile, given the clinical cours

[Amnesic presentations of the compulsive obsessional confusions (about 3 patients appearing in a consultation of memory)].

Disorders or complaints of memory are a frequent cause of consultation in depression, major anxiety and psychiatry disease with personality disorders. We report 3 patients with obsessive compulsive disorder (OCD), without diagnosis and treatment, examined in a specialized memory consultation. They always had OCD with cognitive checking. Diagnosis of transient global amnesia and temporal complex seizure were discussed in 2 cases. Psychometric impairment only was observed in first free recall of a verbal memory task and was no specific. Behavioural during testing seemed to be very important to a

[Migraine accompagnée in childhood and adolescence].

The term migraine accompagnée or complicated migraine is applied to those cases of migraine in whom a neurological deficit, which is transient in most instances, occurs. The three main varieties are hemiplegic (unilateral sensory loss, hemiplegia, aphasia), basilar (brain stem dysfunction) and ophthalmoplegic migraine. The clinical symptoms of migraine accompagnée in children and adolescents are discussed on the basis of the observations in 26 patients (11 boys, 15 girls) and the results of EEG-, CT- and angiographic investigations are referred. A special case of "migraine without headache" an

High-resolution SPECT to assess hippocampal perfusion in neuropsychiatric diseases.

UNLABELLED: The purpose of this study is to clarify the changes of hippocampal perfusion in neuropsychiatric diseases, including dementia, compared with control subjects, and to correlate hippocampal perfusion with the dementia rating scale and the severity of memory disturbance in patients with these diseases. METHODS: A total of 45 right-handed patients were investigated (13 with dementia of Alzheimer type, 6 with multi-infarct dementia, 4 with progressive dementia with motor neuron disease (MND), 3 with transient global amnesia, 5 with other diseases and 14 control subjects). Regional cere

Acute confusional migraine: our knowledge to date.

Acute confusional migraine (ACM) is a rare migraine variant, affecting children and adolescents, as well as adults. Between 0.45 and 7.8% of children with migraine present with ACM, but the disorder may well be underdiagnosed. ACM is an exclusion diagnosis and some dangerous causes of confusion (e.g., epilepsy, ischemia, hemorrhagia, neoplasm, intoxication and encephalitis) should be ruled out. The confusional state often manifests with a wide diversity of cortical dysfunctions, such as speech difficulties, increased alertness, agitation and amnesia. Exact history taking, clinical examination,

Paroxysmal limbic disorders in neuropsychiatry.

Patients who experience episodes with a transient disturbance in perception, memory, mood, and level of anxiety are frequently encountered by neurologists, psychiatrists, and psychologists. The possibilities for differential diagnosis of these episodes include partial complex epilepsy, psychogenic seizures, panic disorder, dissociative states, migraine, and transient global amnesia, among others. The clinical history may be vague and confusing, making it difficult to determine definitively the etiology of the events. This article describes a spectrum of paroxysmal disorders involving limbic st

Anti-glutamic Acid decarboxylase antibody associated limbic encephalitis in a child: expanding the spectrum of pediatric inflammatory brain diseases.

Anti-glutamic acid decarboxylase directed antibodies are a rare cause of autoimmune limbic encephalitis that is relatively resistant to immunotherapy. Here we report a 15-year-old boy with nonparaneoplastic, anti-glutamic acid decarboxylase limbic encephalitis presenting with subacute headache, memory disturbance, psychiatric symptoms, and seizures. At onset, his memory disturbance manifested as transient global amnesia-like episodes. Clinical remission was achieved with rituximab, intravenous immunoglobulin, and corticosteroids.

Transient epileptic amnesia mistaken for mild cognitive impairment? A high-density EEG study.

Mild cognitive impairment (MCI) converts to Alzheimer's disease within a few years of diagnosis in up to 80% of patients. The identification among such a population of a rare form of epilepsy (transient epileptic amnesia [TEA]), characterized by mixed anterograde and retrograde amnesia with apparent preservation of other cognitive functions, excessively rapid decay of newly acquired memories, and loss of memories for salient personal events of the remote past, strongly affects prognosis and medical treatment. Our aim was to define the clinical utility of routine high-density electroencephalogr

A woman lost in the cemetery: A case of time-limited amnesia.

A 43-year-old woman one day experienced a dissociative fugue which she could not recall. She was married, nulliparous, with no history of dissociative disorder or other psychiatric disorders. She had been sexually abused during late childhood-early adolescence. She was examined thoroughly from both psychiatric and medical standpoints to exclude organic causes for her condition. Magnetic Resonance Imaging showed only some non-specific abnormalities. On personality tests, a histrionic structure of personality emerged, with obsessive and narcissistic traits accompanied by rigidity and anxiety, dy

"Broken Heart" and "Broken Brain": Which Connection?

The interconnections between brain and heart are increasingly recognized. Takotsubo cardiomyopathy, also known as "broken heart syndrome", is characterized by a cardiovascular dysfunction provoked by an emotional or stressful situation. Similar events can trigger a neurological pathology called transient global amnesia. These conditions can occur simultaneously, although their precise connection is not well understood. We aim to present the case of a patient who experienced them and to review the relevant literature.

Neurologic complications of antiphospholipid antibodies.

Antibodies directed against phospholipids are highly associated with venous and arterial thrombotic episodes, which are often recurrent. There seems to be a skewed frequency of cerebral and ocular events when the arterial circulation is affected. Other neurologic syndromes, including dementia, migraine, chorea, transverse myelopathy, Guillain Barré syndrome, transient global amnesia, seizures, motor neuron disease, myasthenia gravis, and depression, have also been described in association with aPL. Although some of them (for example, dementia, chorea, seizures, and transient global amnesia) co

Prolonged reversible amnesia: a case report.

A 55-year-old woman sustained an acute amnesia that lasted for 8 days. The results of a cerebral magnetic resonance imaging scan and an electroencephalogram were normal. A Wechsler Memory Scale-Revised (WMS-R) test performed 6 days after the event showed global impairment in every domain, except for attention/concentration. A single photon emission computed tomography (SPECT) scan taken 8 days after the event showed hypoperfusion in the left temporal and frontal areas. The results of a follow-up SPECT scan on the 40th day after the episode were normal. A second WMS-R test showed some improveme

Memory for Public Events in Mild Cognitive Impairment and Alzheimer's Disease: The Importance of Rehearsal.

Ribot's law refers to the better preservation of remote memories compared with recent ones that presumably characterizes retrograde amnesia. Even if Ribot-type temporal gradient has been extensively studied in retrograde amnesia, particularly in Alzheimer's disease (AD), this pattern has not been consistently found. One explanation for these results may be that rehearsal frequency rather than remoteness accounts for the better preservation of these memories. Thus, the aim of present study was to address this question by studying retrograde semantic memory in subjects with amnestic mild cogniti

The Man Who Mistook a Hat for His Wife: Case Report of Aortic Dissection Presenting With Acute Hyperfamiliarity for Faces.

Acute aortic dissection is a challenging diagnosis for emergency physicians because of its high mortality and wide range of clinical presentations. We report a case of a previously healthy man who presented with hyperfamiliarity for faces syndrome as the predominant symptom of a large type A aortic dissection diagnosed by computed tomography angiography in the emergency department. Different elements of his presentation suggested possible other diagnoses, including transient global amnesia and transient ischemic attack. We discuss how evaluating these other diagnostic possibilities led to the

Differential diagnosis of seizures.

Distinguishing epileptic events from nonepileptic paroxysmal neurologic events represents a common diagnostic challenge. Syncope, either cardiac or noncardiac, can appear similar to atonic and even convulsive seizures. Breath holding and benign paroxysmal vertigo in children may be confused with epilepsy. Classic migraine, transient global amnesia, and transient ischemic attacks may resemble epileptic seizures. Sleep disorders, including nocturnal movements, parasomnias, and narcolepsy also may resemble epileptic seizures. Most movement disorders are distinguished easily from epilepsy; however

Recurrent orthostatic global amnesia in a patient with postoperative hyperfibrinogenemia.

Transient global amnesia (TGA) is characterized by sudden, temporary dysfunction of antegrade and recent retrograde memory without other neurologic deficits. Although there is sometimes a precipitating event, the origin of TGA remains controversial. We encountered a patient who developed recurrent TGA when upright, in whom the symptom promptly and regularly resolved when supine. Symptoms began about a week after cardiac surgery concurrent with marked hyperfibrinogenemia and acceleration of the erythrocyte sedimentation rate, and abated without recurrence when these laboratory abnormalities wer

[Migraine and cortical spreading depression].

Migraine is an episodic headache disorder accompanied by various neurological, gastrointestinal and autonomic changes. In one fifth of the migraineurs, a neurological disturbance (visual, sensory or motor) appears during or before the development of the headache called migraine aura. Cortical spreading depression (CSD) is a transient neuronal depolarization that spreads across unilateral hemisphere from a focus and is followed by a long-lasting depression of neuronal activity. CSD was proposed to be the underlying phenomenon of the migraine aura as it propagates at a similar velocity with visu

Unusual neurologic manifestations associated with Epstein-Barr virus infection.

We describe two patients who displayed unusual neurologic manifestations associated with reactivation of latent Epstein-Barr virus (EBV) infection. The patients included a 34-year-old woman who suffered from meningoencephalitis presenting with symptoms identical to those of transient global amnesia, and a 57-year-old man with jumbling phenomenon and gait ataxia. The protean neurologic manifestations of EBV infection underscore the need to suspect this infection in any acute neurologic disorders of possible infectious origin irrespective of the age of the patient.
