OBJECTIVE: To investigate factors that predict walking with a prosthesis after lower limb amputation.
DESIGN: Systematic literature review.
METHODS: A computer-aided literature search of MEDLINE, EMBASE, CINAHL and the Cochrane Library was performed to identify studies published up to August 2007 that investigated factors that predicted walking ability after lower limb amputation.
RESULTS: A total of 57 studies were selected. Predictors of good walking ability following lower limb amputation include cognition, fitness, ability to stand on one leg, independence in activities of daily living

Seventeen great toes, amputated at the distal phalangeal to the level of the MTP joint, were replanted between 1990 and 1998, at Izmir Hand and Microsurgery Hospital. Replantation in five out of six complete amputations, and seven out of eleven incomplete amputations were successful, and the overall survival rate was 76.4%. In failed replantations, the base of the proximal phalanx of the great toe was preserved during closing of the stump. Nine of 17 patients were available for review in the follow-up period of mean 3.5 years (range 1-6.5 years). Clinical and biomechanical evaluations of the o

Understanding the biomechanical interaction between residual limbs and prosthetic sockets is essential for optimizing prosthesis design and minimizing tissue injury. Existing cadaveric stance simulators have not been specifically designed to examine socket-residual limb biomechanics. This study designed, constructed, and evaluated a cadaveric stance simulator capable of applying physiologically representative compressive forces through residual limbs. The long-term objective is to utilize dynamic stereo x-ray (DSX) to quantify 3D skin strain at the socket-residual limb interface, for which thi

BACKGROUND: Relative movement between the socket and residual limb can impair function in prosthesis users. It is plausible that, in addition to its mechanical effect, the integrity of the socket-residuum interface influences the ability of an individual to sense tactile cues through the prosthesis. Vacuum-assisted suspension (VAS) has been shown to reduce relative movement at this interface, providing a means to test this premise. The purpose of this pilot study was to assess the effects of altering socket-residuum interface integrity through the VAS pressure level on the thresholds of percep

The wear comfort of a prosthesis is of great importance for amputee patients. The wear comfort can be affected by changes in the interface between the residual limb and prosthesis socket, which can be caused by time-dependent volume fluctuations of the tissue, leading to unwanted local pressure marks. The basis to ensure time-independent wear comfort of a prosthesis is to identify these changes. Common techniques for identifying these variations have a negative impact on the sensitive interface between the residual limb and prosthesis. The following paper contains a proof of concept for the de

PURPOSE: This work presents unique designs for prosthetic restoration of the distal finger. We first discuss fitting a prosthetic nail in order to restore the cosmetic deficit caused by partial or complete nail injury. This concept is inspired from snap fit and lanced sheet metal technology. We also discuss new approaches to designing and fitting a full fingertip prosthetic with a special suspension and a socket for more complete cosmetic fingertip restoration.
METHODS: The designs utilize the compliance and higher strain level of hinges to fit the prosthesis with either the residual nail or

Symptomatic neuromas are a common cause of postamputation pain that can lead to significant disability. Regenerative peripheral nerve interface surgery is performed to treat symptomatic neuromas and prevent the development of neuromas. This review delineates the clinical problem of postamputation pain, describes the limitations of the available treatment methods, and highlights the need for an effective treatment strategy that leverages the biologic processes of nerve regeneration and muscle reinnervation. The evidence supporting use of regenerative peripheral nerve interface surgery to mitiga

People with a transtibial amputation worldwide rely on their prosthetic socket to regain their mobility. Patient comfort is largely affected by the weight and strength of these prosthetic sockets. The use of additive manufacturing could give the prosthetist a range of new design possibilities when designing a prosthetic socket. These new design possibilities can in turn lead to improved socket designs and more comfortable prosthetic sockets. This new way of designing and producing prosthetic sockets radically differs from the manual traditional production process. This makes it difficult for p

While considering the importance of the interface between amputees and prosthesis sockets, we study an anthropomorphic prosthesis socket whose size can be dynamically changed according to the requirements of the residual limb. First, we introduce the structure and function of the anthropomorphic prosthesis socket. Second, we study the dynamic model of the prosthesis system and analyze the dynamic characteristics of the prosthesis socket system, the inputs of an oscillometric pump, and the control mechanism of force-sensitive resistor (FSR) pressure signals. Experiments on 10 healthy subjects u

Accurate, noninvasive testing for deep venous thrombosis (DVT) by conventional methods is often not possible in the rehabilitation patient. Lower extremity amputation, a cast or bandage, or skin problems present obstacles to standard diagnostic methods. This report describes the use of duplex ultrasound (US) scanning for noninvasive diagnosis of DVT in a seventy-year-old man with a below-knee amputation, on whom Doppler and plethysmography examinations could not be performed. As experience is gained with this technique, the use of venography for diagnosis of DVT becomes more difficult to ratio

Painful stump neuromas in the postamputation patient are difficult to diagnose and treat. We report a case in which magnetic resonance allowed precise preoperative diagnosis and localization of such a lesion. Magnetic resonance, with its ability for long axis imaging in the extremity, is ideally suited for the workup of stump neuromas.

Fourteen artificial limbs in eight adult patients with below the knee amputations were evaluated for prosthetic fit by several radiological parameters. The best objective means of evaluation of prosthetic adequacy is piston action.

The process of fabricating polymerized metal sockets for above-knee and below-knee prostheses is described. The technique is based on pulse stamping of metal blanks over a matrix imitating the negative mould of the stump and subsequent polymeric coating by vibro-vortex spraying. The monitoring of more than 500 patients fitted with metal-polymer sockets since 1978 is reported.

This paper describes a socket design and manufacturing technique for through-knee stumps, based on 14 years experience with 290 prostheses. The bulbous shape of the stump and its full end-bearing quality requires a socket which has very little resemblance to above-knee sockets. The requirements of the through-knee socket are listed and the manufacturing technique is described and illustrated in detail. Emphasis is put on the quality of the negative plaster mould. The socket is manufactured according to the double wall technique commonly used in below-knee sockets. It provides a maximum of comf
