Effect of Intravenous Ondansetron on Spinal Anesthesia-Induced Hypotension and Bradycardia: A Randomized Controlled Double-Blinded Study.

BACKGROUND: Spinal anesthesia is a safe anesthetic technique commonly practiced. However, it is associated with hypotension (33%), bradycardia (13%), and shivering which are induced by hypovolemia, sympathetic blockade, and Bezold-Jarisch reflex through intracardiac serotonin (5HT3) receptors and vagus nerve. AIM: To study the effect of intravenous (i.v.) ondansetron on hypotension and bradycardia induced by spinal anesthesia. SETTING AND DESIGN: This was a randomized controlled double-blinded study done in a tertiary care teaching hospital. METHODS: Of 140 patients, 70 in Group A received

Delayed diagnosis of cauda eqina syndrome with perineural cyst after combined spinal-epidural anesthesia in hemodialysis patient.

Symptomatic Tarlov (perineural cysts) are uncommon. In the following hemodialysis case, cauda equina syndrome was not detected after combined spinal-epidural anesthesia untilthe patient reported a lack of sensation in the perianal area 14 days postoperatively. She had normal motor function of her extremities. A laminectomy and cyst irrigation was performed. After the operation, her sphincter disturbance subsided gradually and her symptoms had disappeared.

Prophylactic [corrected] granisetron does not prevent postdelivery nausea and vomiting during elective cesarean delivery under spinal anesthesia.

BACKGROUND: Intraoperative nausea and vomiting (IONV) during cesarean delivery (CD) under regional anesthesia has multiple etiologies, and the role of prophylactic antiemetics for prevention of IONV remains controversial. In this trial we sought to determine the efficacy of granisetron for prevention of IONV during elective CD under spinal anesthesia with strict control of the causative factors. METHODS: The study was conducted as a randomized, double-blind, placebo-controlled trial in 176 parturients. After administration of 10 mL/kg of lactated Ringer's solution, spinal anesthesia was admin

[Anesthesia for scoliosis surgery: preoperative assessment and risk screening of patients undergoing surgery to correct spinal deformity].

Spinal column deformity is associated with potentially serious alterations of respiratory and cardiac function. Such alterations, in association with the underlying disease that caused the deformity--such as neuromuscular disease--are risk factors that affect the prognosis for scoliosis patients undergoing anesthesia and surgery. It is important for the anesthesiologist to carry out a detailed preoperative assessment to identify patients at risk with the aim of decreasing morbidity and mortality related to surgical correction of deformities. Of paramount importance is awareness of the risk of

Spinal anesthesia in MELAS syndrome: a case with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes.

MELAS syndrome (mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes) is one of the classic mitochondrial encephalomyopathies with variable clinical presentation and multisystem involvement. Enhanced sensitivity to neuromuscular blockade or anesthetic agents and susceptibility to malignant hyperthermia in these patients have ever been reported, all of which complicate the management of general anesthesia. To avoid these appalling troubles in general anesthesia, we chose spinal anesthesia for a patient with MELAS syndrome receiving appendectomy. The patient obtained

[Does intrathecal clonidine prolong the effect of spinal anesthesia with hyperbaric mepivacaine? A randomized double-blind study].

BACKGROUND: Clonidine, an alpha 2-receptor agonist, has been reported to prolong the blocking actions of local anaesthetics. The aim of this study was to investigate the effects of spinally injected clonidine on the duration of spinal anaesthesia by mepivacaine and on the postoperative demand for analgesics. METHODS: Forty-five patients who had transurethral urological surgery under spinal anaesthesia were randomized to 3 groups (n = 15 each) and studied in a double-blind fashion. Group I received 80 mg mepivacaine 4% only, while in group II mepivacaine was combined with 75 micrograms clonidi

Bilateral central scotoma with preservation of central vision in 2 patients following caesarean section under spinal anesthesia.

Two female patients are described with acute bilateral central scotomata with preservation of central vision following spinal anesthesia. Both patients had a temporary drop in systemic blood pressure and underwent caesarean section with moderate blood loss. In order to explain the visual field defects extensive ophthalmological examination and CT-scanning with nuclear magnetic resonance imaging were performed. The hypothetical explanation for the visual field defects is transient macular ischemia.

Hemodynamic Changes During Cesarean Section Under Spinal Anesthesia in Normotensive and Hypertensive Pregnant Women-A Narrative Review.

Data on cardiac and hemodynamic parameters associated with preeclampsia (PE), particularly changes occurring in the immediate perioperative period, remain scarce. These changes are clinically important for the management of patients with severe PE or underlying cardiac dysfunction. Maternal hemodynamics undergo substantial alterations during cesarean section (CS) as a result of sympathetic blockade induced by spinal anesthesia, the vasodilatory effects of general anesthetics, and changes in blood flow related to aortocaval compression in the supine position and during delivery. Massive hemorrh

Perioperative Outcomes in Endoscopic Lumbar Decompression Surgery Under Spinal and General Anesthesia: A Matched Control Cohort Study.

SUMMARY OF BACKGROUND DATA: The prevalence of endoscopic techniques in lumbar spine surgery is increasing. While general anesthesia (GA) remains the standard approach, spinal anesthesia (SA) is a promising alternative. OBJECTIVE: Whereas much of the existing literature examines the use of SA in traditional surgical techniques, this study aims to compare perioperative outcomes in patients undergoing endoscopic lumbar spine surgeries under GA and SA. STUDY DESIGN: We retrospectively analyzed data from 22 patients who underwent elective endoscopic lumbar spine surgery under SA and were matched

Comparing the Quality of Patient Recovery After Open Appendectomy Under General Versus Spinal Anesthesia: Prospective Cohort Study.

BACKGROUND: Open appendectomy is commonly performed under either general or spinal anesthesia. Postoperative quality of recovery, a multidimensional patient-centered measure of outcomes after surgery, is affected by the choice of anesthesia technique. OBJECTIVE: The objective of our study was to compare the effects of general and spinal anesthesia on the immediate postoperative quality of recovery in adults undergoing open appendectomy. METHODS: In this prospective cohort study, 74 patients were assigned to either the general (n=37) or the spinal anesthesia (n=37) groups. The primary outcome

Thirty-day outcomes of spinal versus general anesthesia for high-risk patients undergoing open inguinal hernia repair.

BACKGROUND: General anesthesia (GA) is the predominant choice for open inguinal hernia repair but may be associated with a higher risk of postoperative complications, particularly in patients with multiple comorbidities. Spinal anesthesia (SA) is therefore considered as an alternative for high-risk patients. However, its potential benefits in reducing postoperative complications remain unclear. This study aimed to compare 30-day outcomes between SA and GA in high-risk patients undergoing initial open inguinal hernia repair. METHODS: Patients who underwent initial open inguinal hernia repair w

Comparing local versus spinal anesthesia for transurethral cystolithotripsy: A retrospective review of efficacy, safety, and pain outcomes.

INTRODUCTION: Urinary bladder stones cause significant morbidity and are increasingly treated by transurethral holmium laser cystolithotripsy (TULC), a minimally invasive procedure. While spinal anesthesia is commonly used, local anesthesia is gaining interest, especially for high-risk or elderly patients. This study compares the safety, efficacy, and outcomes of TULC under local versus spinal anesthesia. METHODS: A retrospective observational study at AIIMS, Jodhpur, India (March 2023-March 2025) included 81 bladder stone patients undergoing TULC. 43 received local anesthesia and 38 spinal a

Comparative evaluation of spinal versus epidural anesthesia in lower limb orthopedic surgery: Hemodynamic stability and block duration.

Regional anesthesia remains essential in lower limb orthopedic surgeries for providing effective intraoperative anesthetic control and post-operative analgesia with fewer systemic side effects. Therefore, it is of interest to evaluate the fifty patients undergoing elective lower limb orthopedic procedures to assess spinal versus epidural anesthesia in relation to hemodynamic stability and block duration. Twenty-five patients received hyperbaric bupivacaine via spinal route and twenty-five received bupivacaine through a lumbar epidural catheter. Spinal anesthesia demonstrated faster onset and d

Spinal Anesthesia in a Respiratory-Compromised Patient With Fabry Disease Undergoing Umbilical Hernia Repair: A Case Report.

Fabry disease (FD) is a rare X-linked lysosomal storage disorder associated with multisystemic involvement, making anesthetic management challenging due to potential cardiomyopathy, renal insufficiency, and airway difficulties. We present the case of a 42-year-old female patient with FD and significant respiratory comorbidities, including active smoking and moderate-to-severe airflow obstruction (FEV1: 50% predicted), alongside a body mass index (BMI) of 32.3 kg/m², presenting for umbilical hernia repair. Given the high risk of perioperative respiratory complications, spinal anesthesia was sel
