Relation of papillomaviruses to anogenital cancer.

A variety of HPV types infect the anogenital mucosa, giving rise to lesions that differ in clinical appearance, histology, and risk of malignant progression. Human papillomavirus type 16 is distinguished by a strong association with high-grade intraepithelial neoplasia and the greatest prevalence in anogenital malignancy. Most cancers appear to have a multifactorial cause, and HPV infection alone is probably insufficient for malignant transformation. The consistent association between HPV infection and anogenital cancers emphasizes that the papillomaviruses may have a necessary role in carcino

Perianal Bowen's disease associated with anorectal warts: a case report.

The occurrence of anogenital warts has increased both in clinic and in private practice. Both sexes and all races are affected, with the highest prevalence in patients aged 15 to 40 years. The etiologic agent, the human papillomavirus (HPV), has been classified by DNA hybridization techniques into at least 42 types, of which types 16 and 18 are considered to carry a high risk for cancer. A patient who had been seen intermittently over a period of 26 years with perianal and anal warts that responded to treatment finally developed two granulomatous nodules morphologically different from the prev

[Malignant melanoma of the anorectum].

Malignant melanoma of the anal canal in stages I, II and III, respectively, was diagnosed in a 58-year-old woman and two men, aged 69 and 73 years. The patients' age was thus above the average reported age of manifestation (60 years) of this tumour. In two patient the tumours had been incorrectly diagnosed because of their proximity to the linea dentata and their similarity to haemorrhoids: operation had been delayed for four and about six months, respectively. Treatment of choice for stages I and II was abdominoperineal amputation of the rectum, while in the patient with stage III a colostomy

Treating genital condyloma during pregnancy with the carbon dioxide laser.

The therapeutic effectiveness of the carbon dioxide laser was evaluated in 43 pregnant women with extensive urogenital and anal condylomas. All patients received one treatment and were followed up for an average of 9 months after delivery. Laser failures (persistent disease) and recurrences (new disease) were stratified according to the location of the lesions and the gestational age of the patients. The overall failure rate was 5%; 6% of the women with multiple sites involved had persistent disease but none of the lesions confined to the vulva persisted after laser treatment. The recurrence r

Liver Metastasectomy in Anal Squamous Cell Carcinoma: The Mayo Clinic Experience.

BACKGROUND: Metastatic squamous cell carcinoma of the anus (SCCA) carries a poor prognosis, with systemic therapy remaining the standard of care. While metastasis-directed therapy improves outcomes in select gastrointestinal malignancies, the role of liver-directed intervention in metastatic SCCA remains undefined. We evaluated clinicopathologic features and oncologic outcomes of patients with liver-limited metastatic SCCA treated with curative-intent hepatic local therapy at a tertiary academic center. METHODS: We conducted a retrospective cohort study of adults with histologically confirmed

Evidence of 18 F-FDG Utility in Rare Case of Metastatic Anal Melanoma With Prolonged Survival.

A 43-year-old woman presented with an anorectal mass at which time excision was performed, and biopsy revealed anorectal mucosal melanoma (AM), anal subtype. Postexcision, 18 F-FDG PET/CT identified residual melanoma, confirmed on biopsy. Recurrence was monitored every 3 months using 18 F-FDG, which identified a perirectal lymph node 2.5 years later positive for metastatic melanoma. Given the rarity of AM and often delayed diagnosis, patients often present with metastases, resulting in poor prognosis. This unique case of metastatic AM with prolonged survival of 7 years to date supports the imp

Magnetic resonance imaging of anal cancer: tumor characteristics and early prediction of treatment outcome.

PURPOSE: To analyze tumor characteristics derived from pelvic magnetic resonance imaging (MRI) of patients with squamous cell carcinoma of the anus (SCCA) before and during chemoradiotherapy (CRT), and to compare the changes in these characteristics between scans of responders vs. nonresponders to CRT. METHODS: We included 52 patients with a pelvic 3T MRI scan prior to CRT (baseline scan); 39 of these patients received an additional scan during week 2 of CRT (second scan). Volume, diameter, extramural tumor depth (EMTD), and external anal sphincter infiltration (EASI) of the tumor were assess

Comprehensive Review of Anal Cytology.

BACKGROUND: The incidence of anal cancer has increased over at least the past decade, with estimates of a continued increase in the coming years. Women are more commonly affected than men in the general population; separate high-risk populations have also been identified. While the pathophysiology of anal cancer is thought to be similar to its cervical counterpart, well-defined and standardized screening guidelines have not been established as is seen in cervical cancer prevention. Nonetheless, multiple screening modalities have been examined and are components of proposed institutional and so

The Role of Immunotherapy in the Treatment of Anal Cancer and Future Strategies.

Despite being markedly sensitive to chemoradiotherapy, patients with locally advanced (T3-4 and/or node-positive) squamous cell carcinoma of the anal canal (SCCA) still present high rates of disease recurrence, which is characterized by meaningful morbidity and mortality. Abdominoperineal resection as salvage surgery may be considered for patients with local recurrence, but with an important negative impact in the quality of life. Systemic therapy of advanced SCCA is an unmet clinical need. Palliative chemotherapy for the management of unresectable or metastatic disease yields approximately 60

HPV vaccination to prevent recurrence of anal intraepithelial neoplasia in HIV+ MSM.

OBJECTIVE: Anal cancer precursor lesions high-grade anal intraepithelial neoplasia (HGAIN) are highly prevalent among HIV+ MSM. Treatment of HGAIN is frustrated by high recurrence rates. We investigated the efficacy of the quadrivalent human papillomavirus (qHPV) vaccine as posttreatment adjuvant in preventing HGAIN recurrence in HIV+ MSM. DESIGN: Randomized, double-blind, placebo-controlled, multicentre trial. SETTING: Three HIV outpatient clinics in Amsterdam, the Netherlands. SUBJECTS: HIV+ MSM with CD4+ cell count more than 350 cells/μl, biopsy-proven intra-anal HGAIN successfully treat

[A Case of Anal Canal Squamous Cell Carcinoma Detected with Anal Stenosis Eight Years Following Resection for Rectal Adenocarcinoma].

An 89-year-old woman underwent low anterior resection for rectal adenocarcinoma(Ra, pT3N0M0, pStage II , Cur A)in 2008. In February 2016, she underwent an outpatient examination because of a defecation disturbance. Lower gastrointestinal endoscopy was performed and the stenotic region was biopsied. However, no malignancy was detected and the stenotic site expanded. However, the patient experienced recurrence of the same symptoms, developed severe anal stenosis, and underwent another examination in December 2016. Magnetic resonance imaging indicated a neoplastic lesion around the entire circumf

[Our experiences of anal squamous cell carcinoma treated by chemoradiotherapy].

We reviewed the clinical records of 6 cases with anal squamous cell carcinoma to evaluate the clinical effectiveness of chemoradiotherapy (CRT). The radiotherapy consisted of 40 Gy delivered to the pelvis and bilateral inguinal lesion, and a perianal booster dose of 20 Gy, in fractions of 2.0 Gy per day, 5 days per week. 5-FU and mitomycin C were administrated 3 times every 4 weeks as standard chemotherapy. On the first day of radiation therapy, 750 mg/m2 of 5-FU in the form of a continuous 24-hour infusion for 5 days was given. On the first day of chemotherapy, 10 mg/m2 of mitomycin C was als

Treatment of grade III anal intraepithelial neoplasia with photodynamic therapy: report of a case.

PURPOSE: We report the use of photodynamic therapy to treat a 53-year-old female with Grade III anal intraepithelial neoplasia. METHODS: Topical 5-aminolevulinic acid cream was applied to the affected area five hours before light treatment. The distribution of 5-aminolevulinic acid-induced protoporphyrin IX was identified by its characteristic red fluorescence. The lesion was treated by illumination with a 630-nm red laser light that used a total energy of 125 J/cm2 for approximately 17 minutes. RESULTS: Complete symptomatic relief was achieved after the first photodynamic therapy session, a
