PURPOSE OF REVIEW: The technological advancement of intraocular lenses (IOLs) in recent years has increased their use in the pediatric population. This has led to the use of IOLs in children in whom they would not have been considered in the past. Yet, the majority of the literature looking at the use of specialty IOLs is in the adult literature. This review will look at the use of IOLs in special circumstances as reported in the pediatric population.

BACKGROUND: Though plano bandage contact lenses used for therapeutic purposes are not uncommon for dogs, no literature regarding contact lenses to correct aphakic canines currently exists.
CASE REPORT: Oliver, a 7-year-old terrier mix, was aphakic in his left eye and essentially blind in his right eye as the result of a large retinal detachment. Surgical complications and endothelium damage contraindicated an intraocular lens implant in his left eye. While co-managed with his veterinary ophthalmologist, Oliver was prescribed an aphakic hydrogel contact lens to improve his monocular vision.

Formula for estimating the optic force of intraocular lenses made from elastic materials has been tried in 106 patients. The formula takes account of forward deflection of the elastic intraocular lens in the posterior chamber of the eye. Intrastromal injection of viscoelastic (2% methylcellulose or healon) will facilitate the separation of deep stroma during dissection of recipient stroma. After 8.5 years a rhesus monkey with bilateral hydrogel (Permalens) intracorneal implants was evaluated by light microscopy and transmission electron microscopy to determine the long-term effects of a synthetic plastic upon the primate cornea. Cataract extraction with implantation of an anterior chamber lens was performed in 83 patients (118 eyes). In 3 patients occurred an intraoperative breaking up of the anterior limiting vitreous membrane. It had no influence on the early or late localization of the implanted lens. In the postoperative course one could observe the breaking up of the anterior limiting vitreous membrane in 1 case only; the vitreous pucker caused a delicate deflection of the implanted lens without causing any deterioration of the visual acuity.

The authors discuss the results of cryopexy applied prophylactically in 32 eyes. The freezing was executed at least 6 weeks before the cataract extraction in eyes endangered by retinal detachment. The success rate and complications for contact lens wear in 366 aphakic patients were evaluated retrospectively over a mean follow-up period of 36 months. Successful use of a daily wear contact lens (DWCL) was related to lens handling ability.
