Clinical Implications of Leaks Following Left Atrial Appendage Ligation With the LARIAT Device.

OBJECTIVES: The aim of this study was to evaluate the incidence and clinical implications of leaks (acute incomplete occlusion, early and late reopenings) following LAA ligation with the LARIAT device. BACKGROUND: Percutaneous LAA ligation with the LARIAT device may represent an alternative for stroke prevention in high-risk patients with atrial fibrillation with contraindications to oral anticoagulation. METHODS: This was a retrospective, multicenter study of 98 consecutive patients undergoing successful LAA ligation with the LARIAT device. Leaks were defined as the presence of flow as eval

Left atrial appendage occlusion and pulmonary vein isolation: interest of non-invasive imaging.

A 67-year-old woman with paroxysmal atrial fibrillation (AF), not a candidate for anticoagulant therapy, underwent a combined procedure of pulmonary vein isolation (PVI) and occlusion of the left atrial appendage (LAA) with the Amplatzer cardiac plug prosthesis (AGA Medical Corporation, Plymouth, U.S.A.). After PVI, implantation of the Amplatzer cardiac plug was performed under transoesophageal echocardiography guidance after a complete evaluation of the LAA obtained by different imaging techniques. One month later, multidetector computed tomography and transoesophageal echocardiography confir…

Congenital left atrial appendage aneurysm: a case report and brief review of literature.

A 28-year-old woman with congenital aneurysm of the left atrial appendage was successfully treated by aneurysmectomy and is reported for its rarity. The exact diagnosis is relatively easy to make by contrast echocardiography, magnetic resonance imaging, and/or computerised tomographic angiography, provided the possibility is entertained. Because of supraventricular arrhythmias and systemic thromboembolism, surgical resection was deemed the best curative option to avoid further episodes and recurrence. Published reports detailing the diagnostic evaluation and surgical management for congenital …

Mitral annular systolic velocity reflects the left atrial appendage function in mitral stenosis.

BACKGROUND: Left atrial appendage (LAA) dysfunction is an independent predictor of thromboembolism in mitral stenosis (MS). OBJECTIVES: To investigate whether there is a relation between annular velocities obtained by tissue Doppler imaging and LAA function and to determine if the annular velocities can predict the presence of the inactive LAA in MS. METHODS: Eighty-five MS patients and 80 healthy controls were evaluated by transthoracic echocardiography and all patients underwent transesophageal echocardiography. The annular systolic (S-wave) and diastolic (E- and A-waves) velocities were r…

Color Doppler tissue imaging to evaluate left atrial appendage function in mitral stenosis.

Two-dimensional color Doppler tissue imaging (CDTI) has so far been used, in general, to evaluate ventricular function. This study examined if the left atrial appendage tissue velocity could reproducibly be measured with CDTI and if they have any predictive value for left atrial appendage (LAA) function and former thromboembolism. Thirty-six patients (24 women, 12 men; mean age 45 +/- 12 years; 18 AF; 11 former thromboembolic stroke) with mitral stenosis undergoing transesophageal echocardiography were examined with CDTI. Peak systolic tissue velocity (m/sec, peak systolic velocity [PSV]) was …

[A left atrial appendage thrombus without mitral stenosis--a successful case of removal].

Successful thrombectomy for left atrial ball thrombus without mitral stenosis is presented. A 62-year-old man was admitted to our department … cclusion, when the intraoperative transesophageal echocardiography showed the ball thrombus in left atrial appendage. The urgent operation was successfully carried out on 14th postoperative day after bypass operation. The thrombus occupied almost all the left atrial appendage, and its size was 24 x 19 x 13 mm. The transesophageal echocardiography was useful in dia …

"One-stop" combined percutaneous left atrial appendage and atrial septal defect closure in atrial fibrillation: safety and feasibility from a single-center cohort.

BACKGROUND: Patients with atrial fibrillation (AF) and atrial septal defect (ASD) face elevated thromboembolic risks, yet evidence on combined left atrial appendage closure (LAAC) and ASD closure remains limited. We aimed to assess the feasibility and safety of a "one-stop" strategy for simultaneous LAAC and ASD closure. METHODS: A retrospective analysis included 40 patients with non-valvular AF and ASD undergoing combined procedures (2016-2024). Procedural success, complications, and long-term outcomes (mean follow-up: 1,194.3 days) were analyzed. RESULTS: All procedures were technically su…

Left and right atrial appendage functional features as predictors for voltage-defined left atrial remodelling in patients with long-standing persistent atrial fibrillation.

It was hypothesised that left atrial (LA) fibrosis identified by the presence of low-voltage areas (LVA) may influence the mechanical and electrical function of the left (LAA) and right (RAA) atrial appendage among the long-standing persistent atrial fibrillation (LSPAF) population. 140 consecutive patients underwent voltage mapping of LA with a multielectrode catheter following pulmonary vein isolation and restoration of sinus rhythm with cardioversion. Echocardiography determined LAA peak outflow and inflow velocities and intracardiac catheter-based mean LAA and RAA AF cycle length (AFCL) we…

Thoracoscopic Pulmonary Vein and Left Atrial Posterior Wall Isolation Combined with Left Atrial Appendage Resection in Patients with Long-Standing Persistent Atrial Fibrillation.

OBJECTIVE: To evaluate the efficacy and safety of a modified technique for totally thoracoscopic left atrial posterior wall and pulmonary vein isolation in patients with long-standing persistent atrial fibrillation. METHODS: From April 2017 to December 2018, we included in this study 28 consecutive patients who underwent thoracoscopic left atrial posterior wall and pulmonary vein radiofrequency isolation combined with left atrial appendage resection. We used a device with irrigated electrodes (Medtronic Cardioblate Gemini-s). The original surgical technique "GALAXY" proposed by Doty in 2012 w…

Does Spontaneous Echo Contrast in the Left Atrial Appendage Increase Thromboembolism Risk After Left Atrial Appendage Closure? A Retrospective Study on Its Impact on Device-Related Thrombosis and Arterial Thromboembolic Events.

Background: Left atrial appendage closure (LAAC) prevents arterial thromboembolic events (ATEs) in nonvalvular atrial fibrillation (AF). Spontaneous echo contrast (SEC) is an independent risk factor for left atrial appendage (LAA) thrombosis; however, there is little knowledge about the relationship between preoperative SEC and the increased risk of device-related thrombosis (DRT) or ATEs in patients with AF who have undergone LAAC. Methods: This retrospective study focused on patients with nonvalvular AF who successfully underwent LAAC surgery. Transesophageal echocardiography (TEE) was used …

[Residual thrombosis in the left atrial appendage after surgical closure (AtriClip®) in a patient with permanent atrial fibrillation].

Atrial fibrillation is one of the main causes of cardioembolic stroke, frequently associated with thrombus formation in the left atrial appendage (LAA). Surgical LAA closure (LAAC) using the AtriClip® device is an effective and guideline-recommended strategy in patients undergoing cardiac surgery for other indications, with continuation of oral anticoagulation therapy where indicated. However, thrombus formation on the residual LAA stump, although rare, is a clinically significant complication. We report the case of an 80-year-old woman with permanent atrial fibrillation, heart failure, chroni…

Left atrial appendage occlusion versus NOACs in atrial fibrillation: Which way is the weight of evidence tilting in 2026?

Preventing stroke is a key aspect of managing atrial fibrillation (AF). Non-vitamin K antagonist oral anticoagulants (NOACs) have become the preferred choice due to their improved safety profiles and ease of use compared to warfarin. At the same time, left atrial appendage occlusion (LAAO) has evolved from being an option only for patients unable to tolerate anticoagulation to a legitimate treatment supported by clinical trials, longer-term follow-up, and real-world data. As ongoing research progresses, clinicians are increasingly considering whether LAAO could potentially replace long-term an…

Hybrid ablation in a patient with atypical atrial flutter originating from a left atrial appendage aneurysm: a case report.

Left atrial appendage (LAA) aneurysm is a rare structural abnormality that can predispose to significant complications, including systemic embolism and complex supraventricular arrhythmias. Here, we report a 44-year-old female with a coexistent LAA aneurysm and persistent atypical atrial flutter (AFL). Despite cardioversion and medical therapy, she had recurrences of persistent AFL and continued to experience reduced exercise tolerance and chest pain. Computed tomography of the heart revealed a large LAA aneurysm measuring 51 × 53 × 25 millimeters. Due to the risk of perforation associated wit…

Left atrial myxoma arising from the left atrial appendage presenting with cardiorespiratory arrest in a neonate: a case report.

INTRODUCTION: Primary cardiac tumors are exceedingly rare in neonates, with rhabdomyomas predominating in this age group. While myxomas are the most common primary cardiac tumor in adults, they remain a clinical rarity in newborns and typically arise in the right atrium. We report an exceptional case of a left atrial myxoma originating from the left atrial appendage, associated with an unexplained cardiorespiratory arrest and imaging features consistent with a "ball-valve" mechanism, a presentation that, to our knowledge, has not been previously reported in the neonatal population. CASE PRESE…

Prediction of left atrial appendage dysfunction by atrial strain imaging in patients with rheumatic mitral stenosis in sinus rhythm.

AIMS: In severe rheumatic mitral stenosis (MS) with sinus rhythm, left atrial appendage (LAA) dysfunction increases thromboembolic risk, yet clinical guidelines provide no recommendations, and its assessment via transoesophageal echocardiography is not feasible for routine or repeated use. This study assessed whether left atrial (LA) strain imaging could non-invasively predict LAA contractile dysfunction. METHODS AND RESULTS: We prospectively enrolled 138 patients with severe rheumatic MS in sinus rhythm who underwent transthoracic and transoesophageal echocardiography. Left atrial appendage …

Design and Validation of a High-Fidelity Left Atrial Cardiac Simulator for the Study and Advancement of Left Atrial Appendage Occlusion.

PURPOSE: Atrial fibrillation (AF) is the most common chronic cardiac arrhythmia that increases the risk of stroke, primarily due to thrombus formation in the left atrial appendage (LAA). Left atrial appendage occlusion (LAAO) devices offer an alternative to oral anticoagulation for stroke prevention. However, the complex and variable anatomy of the LAA presents significant challenges to device design and deployment. Current benchtop models fail to replicate both anatomical variability and physiological hemodynamics, limiting their utility. This study introduces a novel left atrial cardiac simu…
