[The role of a sympathetic pharmacologic blockade in restoring blood flow to the organ in acute cholecystitis].

By means of a method of rheography, in 74 patients with acute cholecystitis before and after the operation, and in 26 healthy subjects as well, the blood flow of the brain, lungs, liver and lower extremities under conditions of the use of benzohexamethonium, obsidan, aminazine and neostigmine methylsulfate was studied. Resulting from the use of sympathetic pharmacologic blockade, already by day 2, the indices of cerebral, pulmonary and hepatic blood flow normalized or exceeded the normal level.

The effect of extradural blockage upon glucose and urea kinetics in surgical patients.

We have determined the metabolic effects induced by the use of extradural blockage with 0.5 per cent bupivacaine hydrochloride in a group of surgical patients. Turnover rates of glucose and urea were determined isotopically using radioisotopes and studies were performed both in the basal state and during total parenteral nutrition. In the basal state, extradural blockade resulted in a decrease in the turnover rates of both glucose and urea. In addition, when extradural blockade was instituted while the patients were receiving total parenteral nutrition, there was also a significant fall in glu

Electrophysiologic effects of unilateral right and left stellate ganglion block on the human heart.

To determine the electrophysiologic effects of stellate ganglion (SG) block on the human heart, the two SGs were anesthetized separately, with a 24-hour interval between the two procedures, in 13 patients with episodes of supraventricular tachycardia (six had Kent bundles). Left SG block caused: (1) a lengthening of the AH interval, measured at fixed atrial rates of 10 +/- 12 msec (p less than 0.01); (2) a marked depression of the VA conduction in six of the seven patients with measurable VA interval (in two patients it produced complete VA block); (3) a slowing of 20 to 40 msec of the cycle o

[Bell's palsy (prevention and treatment)].

The effectiveness of a variety of the most popular methods used for treatment Bell's paralysis is analyzed on a great number (1168 patients) of cases. It has been found that the results of the treatment are predetermined primarily by the gravity of the facial nerve affection. No convincing dependence of the treatment effect on the disease etiology was noted, except cases of Bell's paralysis of vascular etiology, the outcome of which was worse. A less favourable outcome was also observed in patients over 60 years of age and in cases of paralysis relapses. Of the methods of treating Bell's paral

Pro-arrhythmic role of adrenergic spatial densities in the human atria: An in-silico study.

Chronic stress among young patients (≤ 45 years old) could result in autonomic dysfunction. Autonomic dysfunction could be exhibited via sympathetic hyperactivity, sympathetic nerve sprouting, and diffuse adrenergic stimulation in the atria. Adrenergic spatial densities could alter atrial electrophysiology and increase arrhythmic susceptibility. Therefore, we examined the role of adrenergic spatial densities in creating arrhythmogenic substrates in silico. We simulated three 25 cm2 atrial sheets with varying adrenergic spatial densities (ASD), activation rates, and external transmembrane curre

The enteric nervous system and the musculature of the colon are altered in patients with spina bifida and spinal cord injury.

Neurogenic bowel dysfunction occurs in a large percentage of adult patients with spina bifida (SB) and spinal cord injury (SCI), significantly affecting their quality of life. Although bowel motility is autonomously regulated by the enteric nervous system (ENS), disruption of the modulation of the ENS by extrinsic innervation as present in many patients with SB and SCI might lead to motility disorders. In order to gain insight in the pathophysiology, we studied histological changes of the neuromuscular structures in the colon of SB and SCI patients. Archival colon tissue blocks from SB (n = 13

Modulation of arginine-induced insulin and glucagon secretion by the hepatic vagus nerve in the rat: effects of celiac vagotomy and administration of atropine.

We hypothesized the existence of vagal arginine sensors in the liver which modulate arginine-induced pancreatic hormone secretion. The present study was carried out to examine the efferent pathways and receptor mechanisms from arginine sensors using selective vagotomies and autonomic drugs on the secretion of insulin and glucagon after ip injection of L-arginine (1 g/kg BW) in rats in an unanesthetized and unrestrained state. Hepatic vagotomy (sectioning of the hepatic branch of the vagus nerve) enhanced both plasma insulin and glucagon concentrations after ip arginine more than those in sham-

Participation of GABA/benzodiazepine receptor system in the adrenal chromaffin cell function.

Histochemical studies have shown that GABA-containing nerve terminals impinge upon the chromaffin cells and that approximately 40% of the chromaffin cells store and release GABA. These observations are compatible with the possibility that GABA receptors located on specific populations of chromaffin cells can be activated either by GABA released from nerve terminals or by GABA released from adjacent chromaffin cells. Indeed, experiments with bicuculline indicate that in bovine chromaffin cells in culture and in the adrenal medulla of dog in vivo, the secretion of CA and opioid peptides mediated

Intraoperative Pain Management for Treatment-Resistant Complex Regional Pain Syndrome: A Case Report.

Complex regional pain syndrome (CRPS) is a chronic neuropathic pain disorder often following trauma, associated with severe pain and autonomic disturbances in the affected limbs. Managing CRPS is challenging due to the lack of FDA-approved medications, often requiring off-label treatments. Traditional options like nonsteroidal anti-inflammatory drugs (NSAIDs) and corticosteroids show limited efficacy, while adjunctive treatments such as gabapentin, antidepressants, and bisphosphonates are increasingly favored. Surgical interventions, including nerve blocks and spinal cord stimulation, may help

[Pharmacodynamics of 2-(4-benzyl-piperidino)-1-(4-hydroxyphenyl)-1-propanol (Ifenprodil). (3) Effects on the autonomic, peripheral and central nervous systems].

Effects of ifenprodil tartrate, a potent vasodilator, on the autonomic, peripheral and central nerve system were studied in experimental animals. In isolated vas deferens of guinea pigs, the contraction in response to noradrenaline and sympathetic nerve stimulation was competetively antagonized by ifenprodil 10(-7)--10(-5) M (pA2: 7.69 against noradrenaline). Ifenprodil (50 approximately 1,000 mug/kg i.v.) inhibited the contraction of cat nictitating membrane and dog urinary bladder induced by sympathetic nerve stimulation. Ifenprodil (250 approximately 1,000 mug/kg i.v.) lowered adrenaline-in

Botulinum toxin A in prostate disease: a venom from bench to bed-side.

Botulinum toxin is one of the most toxic natural substances; it acts by blocking the neuromuscular transmission by inhibiting Acetylcholine (Ach) releasing from the motor nerve into the neuromuscular junction. Although the toxin inhibits ACh release, other transmitters can also be inhibited. Botulinum toxin, specifically toxin type A (BONT-A) has been used since the 1970s to treat many different disorders, such as general spasticity resulting from stroke, multiple sclerosis or cerebral palsy, strabismus, hyperhidrosis or excessive sweating, pain, and it is effective in combating migraine and t

Botulinum toxin: basic science and clinical uses in otolaryngology.

The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered. These effects are also graded according to dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dysto

A study of profound hypothermia by surface cooling.

Profound hypothermia below 20 degrees C achieved by surface cooling using simple ice water bath equipment and deep ether anaesthesia is used with the aid of autonomic nerve blocking agents to obtain cardiac arrest for periods of over one hour for open-heart surgery. Blood levels of ether were between 40.6 mg/dl and 285.7 mg/dl during anaesthesia. No arrhythmia occurred and vital signs were quite stable. Hypocarbia throughout the procedure, severe base deficit after circulatory arrest, spontaneous recovery of metabolic acidosis, and a nearly normal cH+ (pH) were observed. Catecholamine increase…

Secretory interactions between the sympathetic and parasympathetic innervations of the submandibular gland in the anaesthetized cat.

Interactions between the sympathetic and parasympathetic innervations of the submandibular gland have been investigated in the anaesthetized cat. At low frequencies of chorda lingual (parasympathetic) stimulation, simultaneous stimulation of the ascending cervical sympathetic nerve in bursts (20 Hz for 1 s at 10 s intervals) increased the flow of submandibular saliva, but the effect was never more than additive. The output of protein was consistently reduced by simultaneous stimulation of both the sympathetic and parasympathetic innervations, below that evoked by stimulation of either alone. S

The neural control of the serotonin content in mammalian enterochromaffin cells.

Serotonin (5-HT) in the mammalian gut is mainly stored in enterochromaffin cells (EC). Vagal or splanchnic nerve stimulation respectively increase portal blood levels of 5-HT and decrease 5-HT content in EC, suggesting a neural control of these cells. Previous studies have also suggested that the vagal effect is mediated via vagal adrenergic nerve fibers, since this effect could be blocked by sympathectomy, removal of the superior cervical sympathetic ganglia or by the administration of a beta-adrenoceptor blocking agent, d, 1-propranolol. In the present study the control of the 5-HT content i

Clinical, neurophysiological and morphological findings in Eaton Lambert syndrome.

Two patients with maysthenic syndrome apparently not associated with carcinoma were studied in detail with repetitive stimulation of motor nerves, single fibre electromyography and histology. One patient showed signs of widespread autonomic dysfunction and the other patient had multifocal CNS symptoms of unclear etiology. Both cases showed a marked improvement with guanidine treatment which could be documented by the electrophysiological investigations. Due to severe adverse effects of the guanidine treatment on kidney, pancreatic and bone marrow functions the drug had to be withdrawn. Edropho

Chronic heart failure: a disease of the brain.

The underlying mechanism for clinical and biochemical manifestations of chronic heart failure (HF) may be due in part to neurohumoral adaptations, such as activation of the renin-angiotensin-aldosterone and sympathetic nervous systems in the periphery and the brain. Internet search and discussion with colleagues are the methods for this study. Since chronic HF is associated with autonomic imbalance with increased sympathetic nerve activity and a withdrawal of parasympathetic activity, it may be considered a brain disease. This phenomenon may be the result of an increased systemic and cerebral
