The impact of progressive expansion of elexacaftor-tezacaftor-ivacaftor availability on lung transplant for cystic fibrosis in France.

BACKGROUND: Several countries reported a reduction in lung transplants for people with cystic fibrosis (pwCF) after elexacaftor-tezacaftor-ivacaftor (ETI) was introduced. We sought to evaluate how lung transplant indications evolved in relation to CFTR genotypes and the use of ETI. METHODS: All lung transplants performed on pwCF in France between 2015 and 2024 were identified using data from the French Agence de la Biomédecine. Individual-level patient data, including CFTR genotypes and use of ETI at the time of transplant, were obtained from the French CF reference network centers. RESULTS:

Predictive capacity of paediatric nasal epithelial cells in sequential CFTR modulator therapy.

BACKGROUND: CFTR modulators have transformed cystic fibrosis (CF) treatment, but individual responses vary even among patients with identical CFTR genotypes. This underscores the need for predictive biomarkers to optimise therapeutic selection. METHODS: We evaluated 24 paediatric patients homozygous for F508del-CFTR, assessing lung function (FEV1pp) and sweat chloride (SC) before and after CFTR modulator therapy. Whole-gene sequencing was utilised to identify CFTR and pharmacogene variants. Patient-derived human nasal epithelial cells (HNECs) were expanded and differentiated at the air-liquid

Cepharanthine Triggers Ferroptosis in Gastric Cancer by PINK1/FUNDC1-Mediated Mitophagy-Dependent GPX4 Degradation.

Cepharanthine (CEP), a natural compound derived from the plant Stephania cephalantha Hayata, demonstrates pharmacological properties including anti-inflammatory, immuno-regulatory, antiviral, antitumor and antiparasitic effects. Due to its insidious progression and drug resistance, gastric cancer (GC) continues to have a high incidence and mortality rate worldwide. While the great potential of ferroptosis in cancer therapy is well-established, and CEP has shown potential antitumor activity, the role of ferroptosis in CEP's impact on GC remains unknown. Our aim in this study is to uncover what

ORIGIN-1 trial: study protocol for an organoid-guided N-of-1 trial evaluating CFTR modulator response in cystic fibrosis with rare non-F508del mutations.

BACKGROUND: Cystic fibrosis (CF) is a genetic condition caused by mutation of the cystic fibrosis transmembrane regulator (CFTR) gene. Recently licensed modulator therapies target the defective CFTR protein and have transformed the formerly life-limiting trajectory of people with CF (pwCF). elexacaftor/tezacaftor/ivacaftor (ETI) has shown outstanding clinical efficacy in pwCF homozygous for F508del (approximately 50%), and those with a F508del mutation paired with any of the > 2000 other mutations identified in the CFTR gene (heterozygous pairing). However, clinical response to modulators is v

Determinants of early Elexacaftor-Tezacaftor-Ivacaftor use in adults with cystic fibrosis and preserved lung function: insights from a European multicenter survey.

BACKGROUND: Limited evidence exists to guide Elexacaftor-Tezacaftor-Ivacaftor (ETI) use in adults with cystic fibrosis (awCF) and preserved lung function (ppFEV1 >90%). To address the resulting variability in prescribing practices, we conducted a European survey among a group of adult CF centres aiming at identifying factors influencing ETI initiation decisions in adults with preserved lung function. METHODS: Between April and June 2024, we invited 25 ECFS prescribers from 25 adult CF centres to participate in a web-based survey to explore factors influencing ETI initiation in adults with CF

Clinical and biochemical response to elexacaftor/tezacaftor/ivacaftor in a patient with the rare L927P/N1303K CFTR genotype.

Elexacaftor/tezacaftor/ivacaftor (ETI) has transformed the treatment of cystic fibrosis (CF), although evidence in individuals carrying rare CFTR variants remains limited. We report the clinical and biochemical response to ETI in a 25-year-old woman with advanced CF carrying the rare L927P/N1303K genotype. Following initiation of ETI under specialist supervision, the patient experienced marked improvement in respiratory symptoms, exercise tolerance, nutritional status, and oxygen requirements. Six pulmonary exacerbations requiring hospitalisation and intravenous antibiotics occurred during the

Targeting ALDH2 with Alda-1 to reverse cisplatin resistance in lung adenocarcinoma.

BACKGROUND: Cisplatin resistance remains a major obstacle in lung adenocarcinoma (LUAD) treatment. The role of Aldehyde dehydrogenase 2 (ALDH2), a detoxifying enzyme, in LUAD prognosis and chemoresistance is poorly understood. METHODS: We analyzed ALDH2's prognostic value using clinical cohorts, TCGA, and proteomic data. Cisplatin-resistant cell lines and xenograft models were used to assess the effect of the ALDH2 agonist Alda-1. Molecular mechanisms were investigated via gain/loss-of-function studies. RESULTS: High ALDH2 expression was significantly associated with improved survival in uni

Cystic fibrosis: new challenges and perspectives beyond elexacaftor/tezacaftor/ivacaftor.

Over the past decade, major clinical advances have been made in the healthcare and therapeutic development for cystic fibrosis (CF), a lethal genetic disease caused by mutations in the gene encoding the CF transmembrane conductance regulator (CFTR) protein. CFTR modulators represent innovative treatments that directly target the primary defects in the mutated CFTR protein and have demonstrated significant clinical benefits for many people with CF (pwCF) who are eligible for these treatments. In particular, the triple combination therapy composed of elexacaftor, tezacaftor, and ivacaftor (ETI)

Long-term outcomes in people with CF lacking FEV1 response to elexacaftor/tezacaftor/ivacaftor therapy.

BACKGROUND: Real-world data demonstrate variability in the response to elexacaftor/tezacaftor/ivacaftor (ETI) treatment among people with CF (pwCF). The aim of this study was to evaluate long-term outcomes in pwCF that had not shown early improvement in the percentage of predicted FEV1 (ppFEV1) following ETI treatment. METHODS: A single-center prospective study in pwCF who initiated ETI. Patients were categorized as 'early responders' if showing an improvement of at least 10 % in ppFEV1 within three months of treatment or 'non-early responders' if not. Patients with pretreatment ppFEV1 of abo

Effect of CFTR modulators on glucose homeostasis in children and young adults with cystic fibrosis-related diabetes: a systematic review.

INTRODUCTION: Cystic fibrosis (CF) is an autosomal recessive disorder caused by mutations in the CFTR gene, leading to impaired chloride transport, thickened mucus, and multiorgan dysfunction. Among its complications, cystic fibrosis-related diabetes (CFRD) is a major concern, characterized by progressive b-cell dysfunction and insulin deficiency. The advent of CFTR modulators, including ivacaftor, lumacaftor/ivacaftor, and elexacaftor/tezacaftor/ivacaftor (ETI), has revolutionized CF management by improving pulmonary function, nutritional status, and overall survival. However, their effects o

Inflammation in preschool cystic fibrosis is of mixed phenotype, extends beyond the lung and is differentially modified by CFTR modulators.

BACKGROUND: Early-life inflammation has long been recognised as a key pathophysiological process in the evolution of cystic fibrosis (CF) lung disease. Despite this, no CF-specific anti-inflammatory treatments have been developed. This is crucial even in the era of highly effective modulator therapy as recent evidence suggests that modulators alter, but may not fully resolve, pulmonary inflammation. METHODS: In this study, we used clinical microbiology data, high-dimensional flow cytometry and multiplex immunoassays to compare pulmonary (bronchoalveolar lavage (BAL)) and systemic immunity in

Design and Characterization of Novel Gastroretentive Drug Delivery System of Antibiotics and Piperine for the Eradication of H. pylori Infection.

Helicobacter pylori (H. pylori) infection affects about half the world population, and if left untreated, can cause painful sores in the stomach lining and intestinal bleeding, leading to peptic ulcer disease (PUD) and stomach cancer. Treatment of H. pylori infection is always a challenge to the treating doctor because of the treatment inefficiency resulting from the poor bioavailability of the drug at the inner layers of the gastric mucosa, where the bacteria reside. This also results in the development of antibiotic resistance. In this work, we developed a mucoadhesive gastroretentive drug d
