Adenocarcinoma of oesophagus involving gastro-oesophageal junction following mini-gastric bypass/one anastomosis gastric bypass.

Mini-gastric bypass/one anastomosis gastric bypass (MGB/OAGB) is an emerging weight loss surgical procedure. There are serious concerns not … oesophageal junction and biopsy reported adenocarcinoma of oesophagus. MGB/OAGB has a potential for bile reflux with increased chances of malignancy. Surveillance by endoscopy at regular intervals for all patients who have undergone MGB/OAGB might help in early detection of Barrett's oesophagus or carcinoma of oesophagus or stomach.

Barrett's esophagus: "All diseases are divine and all are human".

Barrett's esophagus (BE) is a controversial condition. The significance of this condition lies in its premalignant potential, so it is important that clinically applicable biomarkers be identified for early detection and targeted prevention. Dysplasia is currently used as main biomarker, but others most recently surveyed in cancer also include microRNAs. Classically, BE was considered to be an acquired disease related to pathological gastroesophageal acid and bile reflux. However, some cases are associated with genetic predisposition, representing an inherited, familial form of BE. The actual …

[Temporary closure of interposed intestine in treatment of anastomotic dehiscence in esophageal surgery].

INTRODUCTION: Though resections of the esophagus are performed today with a mortality of below 10% anastomotic leakages still represent a serious threat especially when located intrathoracically. METHODS: In cases with intrathoracic leakage and severe septic disease the interposed intestinum was interrupted by staples within the abdomen in order to inhibit bile and intestinal reflux to the anastomosis. Care was taken not to hurt the mesentery. RESULTS: Of 102 patients with adenocarcinoma of the gastroesophageal junction and the distal esophagus 48 were treated with cervical anastomosis and s

Deoxycholic acid induces gastric intestinal metaplasia by activating STAT3 signaling and disturbing gastric bile acids metabolism and microbiota.

Intestinal metaplasia (IM) is the inevitable precancerous stage to develop intestinal-type gastric cancer (GC). Deoxycholic acid (DCA) is the main bile acid (BA) component of duodenogastric reflux and has shown an increased concentration during the transition from chronic gastritis to IM associated with continued STAT3 activation. However, the mechanisms underlying how DCA facilitates IM in the gastric epithelium need exploration. We evaluated IM and bile reflux in corpus tissues from 161 subjects undergoing GC screening. Cell survival and proliferation, proinflammatory cytokine expression and…

The spatiotemporal ecosystem of gallbladder cancer: From inflamed areas to invasive margins - Integrating biomechanical injury, the bile microenvironment, precursor lineages, and spatial niches.

Gallbladder cancer (GBC) is usually described as the end point of chronic cholelithiasis-associated inflammation, and yet this linear formulation does not explain why only a small fraction of people with gallstones develop cancer, why histologically similar precursor lesions follow different evolutionary routes, or why invasion preferentially advances through anatomically and immunologically distinct interfaces. We propose a spatiotemporal ecosystem model that links five domains: gallstones and reflux as precipitating conditions; bile as a chemically active habitat; resident or transient micro

Single-anastomosis duodenoileal bypass versus one-anastomosis gastric bypass: 1-year results of the OASIS randomized trial.

BACKGROUND: Single-anastomosis duodenoileal bypass with sleeve gastrectomy (SADI-S) and one-anastomosis gastric bypass (OAGB) are increasing … lications were significantly more frequent after OAGB (35.4% vs 7.1%; P = .0003), mainly related to bile reflux and managed conservatively or endoscopically. SADI-S was associated with fewer complications but included two cases requiring surgical reintervention. One death occurred in the SADI-S group and was not directly attributable to the procedure. Quality …

A human Barrett's esophagus organoid system reveals epithelial-mesenchymal plasticity induced by acid and bile salts.

The pathogenesis of subsquamous intestinal metaplasia (SSIM), in which glands of Barrett's esophagus (BE) are buried under esophageal squamous epithelium, is unknown. In a rat model of reflux esophagitis, we found that columnar-lined esophagus developed via a wound-healing process involving epithelial-mesenchymal plasticity (EMP) that buried glands under ulcerated squamous epithelium. To explore a role for reflux-induced EMP in BE, we established and characterized human Barrett's organoids and sought evidence of EMP after treatment with acidic bile salts (AB). We optimized media to grow human

Contemporary experience with cholecystectomy: establishing 'benchmarks' two decades after the introduction of laparoscopic cholecystectomy.

With quality and public reporting of increasing importance, benchmarks are anticipated to grow in relevance. We studied cholecystectomy in a practice in an urban tertiary care hospital. A total of 1083 cholecystectomies were performed in 2008 and 2009. Laparoscopic cholecystectomy was performed in 97.8 per cent of patients with a 2.2 per cent conversion rate. A planned open procedure was performed in only 2.2 per cent of patients. Approximately half of procedures were urgent and performed during an acute hospitalization. Most patients (74%) were female and most patients were overweight or obes

Pathophysiology of Gastroesophageal Reflux Disease.

<p>Background: Gastroesophageal reflux disease (GERD) is a prevalent gastrointestinal disorder caused by the retrograde flow of gastric contents into the esophagus, leading to bothersome symptoms and complications. Its pathophysiology is complex and multifactorial, and recent research has aimed to explain the heterogeneity of GERD phenotypes, each influenced by different underlying mechanisms that contribute to symptom presentation and disease progression. Summary: GERD arises from an imbalance between defensive mechanisms and disruptive factors. Key pathophysiological contributors include eso

Bile acids in the treatment of hepatobiliary diseases: rational use of UDCA.

Ursodeoxycholic acid (UDCA) is a hydrophilic, non-toxic bile acid that represents the gold standard in the treatment of various cholestatic and hepatobiliary diseases. Its therapeutic benefit lies in its complex pleiotropic mechanism of action, including cytoprotection of hepatocytes and cholangiocytes, stimulation of hepatobiliary secretion, antiapoptotic effects, and immunomodulatory properties. This article comprehensively summarizes the pharmacological characteristics of UDCA and its rational clinical use in key indications such as primary biliary cholangitis (PBC), primary sclerosing chol

HISTORY: A 46-year-old woman presented for an emergency admission because of colic-like upper abdominal pain and markedly impaired general condition. Eight days before admission she had started and continued to take pantoprazole because of symptoms of gastroesophageal reflux. INVESTIGATIONS: Clinical examination and laboratory tests indicated abnormal liver functions suggesting hepatitis. Serology largely excluded an infectious, autoimmunological or metabolic cause. Duplex sonography gave no evidence of bile obstruction or Budd-Chiari syndrome. DIAGNOSIS AND COURSE: The patient's condition a

[Anatomy of the choledocho-pancreatico-duodenal junction in infancy, childhood and adolescence].

Series of histological sections of the major duodenal papilla were examined. The samples had been taken from human aged between 28 gestational weeks and 19 years. Reflux-preventing structures were found regularly developed in all specimens. Short common terminal canals (duodenal diverticle) were seen in 44 of 46 cases (96 per cent). Pancreatico-biliary union at the level of the duodenal window or below was established even from immature newborns. In two cases, the common bile duct and the main pancreatic duct had orifices in close neighbourhood to each other. They ended on the papilla without
