Splitting of the His potential (H) in sinus rhythm is generally considered to be pathological but its significance during programmed atrial stimulation is not clear. This phenomenon was observed in 10 out of 53 patients aged between 19 and 45 years (average 31.8 years) not suspected of having paroxysmal intranodal block (asymptomatic, sinus rhythm without bundle branch block). Under basal conditions the H and the HV interval (35 to 50 ms; average 41 ms) were normal. Split H was observed with pacing periods of 680 to 885 ms (average 754 ms) and H1 H2 intervals of 325 to 450 ms (average 395 ms).

Ten years ago a clinical method of recording the electrical activity of the His bundle in man with transvenously inserted electrodes was described. His bundle recording has permitted the breakdown of the P-R interval into three conduction intervals, i.e., intraatrial (P-A), A-V nodal (A-H), and His-Purkinje system (H-V). His bundle studies have demonstrated our inability to accurately predict from the surface electrocardiogram the exact location of most A-V blocks. First- and second-degree A-V block can occur in the atrium, A-V node or His-Purkinje system, and third-degree A-V block in the A-V

BACKGROUND: Premature ventricular complexes with a QRS morphology almost identical to the sinus rhythm (PVC-iSR) are scarce and have been insufficiently investigated.
OBJECTIVES: The purpose of this study was to explore the electrophysiology characteristics, true origin, and ablation strategy for PVC-iSR.
METHODS: Among 3,804 patients referred for PVC ablation, 20 patients with PVC-iSR were identified. Detailed mapping, ablation, and analysis were performed.
RESULTS: The earliest activation site (EAS) of PVC-iSR consistently recorded a sharp Purkinje potential with a Purkinje-ventricular in

An unusual form of tachycardia circuit is described. The circuit incorporates a concealed nodoventricular fiber that conducts in a retrograde path, connects the atrioventricular node and the right ventricle, and also includes the distal portion of the atrioventricular node, the His-Purkinje system, and the ventricle. The study patient was first seen with paroxysmal tachycardias of normal QRS duration, complete right bundle branch block, and complete left bundle branch block. Electrophysiologic studies disclosed poor anterograde atrioventricular nodal conduction with a block proximal to His def

This study aims to review the anatomical variations of the right bundle branch (RBB) in normal hearts and various ventricular septal defect (VSD) subtypes through a systematic literature review. Additionally, it seeks to propose hypotheses for optimizing surgical approaches to minimize conduction disturbances during VSD closure, based on anatomical evidence. We performed a systematic literature review of peer-reviewed articles published up to October 2024, focusing on the anatomy of the cardiac conduction system and its variations in association with VSD subtypes. The review encompassed 30 art

BACKGROUND: Atrial activation during typical atrioventricular nodal reentrant tachycardia (AVNRT) exhibits anatomic variability and spatially heterogeneous propagation inside the Koch's triangle (KT). The mechanism of the reentrant circuit has not been elucidated yet. Aim of this study is to describe the distribution of Jackman and Haïssaguerre potentials within the KT and to explore the activation mode of the KT, in sinus rhythm and during the slow-fast AVNRT.
METHODS: Forty-five consecutive cases of successful slow pathway (SP) ablation of typical slow-fast AVNRT from the CHARISMA registry

We report a 57-year-old male patient with dilated cardiomyopathy, NYHA function class III and complete left bundle branch block (CLBBB) who received cardiac resynchronization therapy using His-bundle pacing (HBP) when a left-ventricular lead could not be placed. In 10 months' follow up, his symptoms resolved rapidly, his QRS, left ventricular and left ventricular ejection fraction normalized. Recent reports support the conception that HBP may be an alternative or even better therapy for heart failure patients with CLBBB, especially if biventricular pacing is not feasible.

This paper reviews the anatomical substrates responsible for the induction and maintenance of supraventricular tachycardia and discusses the ECG findings associated with these tachycardias. The normal anatomy of the supraventricular conducting system, particularly within the atria, is complex with conduction proceeding along preferential pathway, which are in turn determined in part by the anisotropic properties of the atrial myocardium. There appear to be at least dual inputs to the atrioventricular node, a posteriorly situated slow pathway and an anterior fast pathway. It is sometimes possib

A patient who had received balloon atrioseptotomy and B-T shunt operation previously experienced recurrent episodes of supraventricular tachycardia, and was refractory to medical treatment since the age to 3. At 9 years, the patient underwent intraoperative electrophysiological mapping which confirmed the earliest breakthrough at the crux of posterior septal region. Rastelli's operation and division of posterior septal Kent was simultaneously performed successfully. Post operative ECG was normalized with abolition of delta wave, and PSVT was gone. We concluded that the division of Kent bundle

Atrioventricular (AV) block was produced in animals for experimental purposes by use of a newly devised cannula. It has bipolar electrodes; one electrode on the needle tip and the other 5.0 mm proximally from the tip. Otherwise, the cannula was electrically completely insulated. The cannula was inserted from the right external jugular vein and advanced close to the AV ring. While His bundle electrogram was obtained with the electrodes, the AV node was accessed. The injection site was determined as the point where His bundle electrogram can be well recorded and the atrial electrogram is close t

The effects of magnesium ions on the cardiac conduction velocity and on the refractory periods of the single parts of Langendorff perfused guinea pig hearts were studied using a high amplifying ECG recording and a special stimulation technique. Concentrations of 2.3 mM or 3.45 mM Mg++ had only little inhibitory effects on the cardiac conduction velocity and on the sinus node activity. The elevation of the magnesium ion concentration to 4.6 mM caused a reduction of the spontaneous sinus rate and a prolongation of the AV nodal and His bundle conduction time. During stimulation with stepwise incr

Endocavitary recording in a patient with attacks of ventricular tachycardia demonstrated a late potential which activated the bundle of His in a retrograde fashion, and the right bundle branch in a forwards direction. This late potential is evidence of a persistant ventricular microeentry which cannot be seen on the peripheral leads. In certain conditions which have been studied, this localised microreentry leads to ventricular tachycardia by macroreentry using the branches of the bundle of His. Tanks to this unusual case, we were able to study the effects of certain anti-arrhythmic drugs on t…

His bundle recordings obtained during and between attacks of Prinzmetal's variant angina and transient atrioventricular (A-V) block were followed by a comprehensive serial section study of the conduction system in a 33 year old woman. Recordings between attacks showed normal A-H and H-V intervals. During an attack there was block proximal to the His bundle recording site. Pathologic studies revealed severe narrowing of the right coronary artery. Arteriolosclerosis of the heart was diffuse. Insignificant changes were found in the approaches to the A-V node and the A-V node itself. Major changes
