The impact of military occupational combustion smoke inhalation on neuroinflammation and brain health.

Airborne combustion emissions from military burn pits, wildfires, and urban/industrial sources are increasingly recognized as a component of the neurotoxic exposome, with potential consequences extending beyond cardiopulmonary disease to brain health. These aerosols comprise heterogeneous mixtures of fine and ultrafine particulate matter (PM₂.₅/PM₀.₁), polycyclic aromatic hydrocarbons, volatile organic compounds, metals, and reactive gases whose composition varies with fuel type, combustion efficiency, and atmospheric aging. Evidence from experimental models, epidemiology, and exposed human co

Ventilation and perfusion alterations after smoke inhalation injury.

Previous studies of human victims of smoke inhalation injury have demonstrated retention of intravenously infused 133xenon2, 6 suggesting either: (1) true intrapulmonary shunting (Qs) secondary to alveolar collapse, flooding, or obliteration, or (2) perfusion of low ventilation/perfusion compartments (low VA/Q) secondary to bronchospasm, bronchial constriction, or partial bronchial occlusion by cellular debris. To differentiate between and quantitate the relative contribution of intrapulmonary shunt versus low VA/Q compartments, multiple inert gas analysis, as described by Wagner et al.,12 was

Smoke toxicity of rainscreen façades.

The toxic smoke production of four rainscreen façade systems were compared during large-scale fire performance testing on a reduced height BS 8414 test wall. Systems comprising 'non-combustible' aluminium composite material (ACM) with polyisocyanurate (PIR), phenolic foam (PF) and stone wool (SW) insulation, and polyethylene-filled ACM with PIR insulation were tested. Smoke toxicity was measured by sampling gases at two points - the exhaust duct of the main test room and an additional 'kitchen vent', which connects the rainscreen cavity to an occupied area. Although the toxicity of the smoke w

The toxicological examination of the victims of the British Air Tours Boeing 737 accident at Manchester in 1985.

The results of toxicological analyses of the body fluids of the victims from the accident involving the British Air Tours Boeing 737 in August 1985 are presented for carboxyhemoglobin, cyanide, and volatiles. All the victims except one had raised concentrations of carbon monoxide. All the victims had raised concentrations of cyanide. All the victims showed the presence of volatile substances in the blood. Autopsies revealed that all the victims had carbon particles in the trachea and bronchi. Thus, all the victims must have inhaled fire products in the burning aircraft cabin. Six victims had c

Effectiveness of High-Flow Nasal Oxygen vs. Standard Nasal Oxygen in Preventing Hypoxia During Procedural Sedation in Minor and Moderate Burn Patients: A Prospective Observational Study.

Background and Objectives: Hypoxia is the most common adverse event in patients undergoing procedural sedation. Burn-related pain is severe and often requires high doses of opioids and anxiolytics, increasing the risk of respiratory depression. This study aimed to compare the effectiveness of high-flow nasal oxygen (HFNO) and standard nasal oxygen (SNO) in preventing hypoxia during deep sedation in patients with minor and moderate burns. Materials and Methods: This single-center prospective observational study enrolled 76 adult patients (ASA I-III) with minor to moderate burns undergoing supin

Evaluation of hydroxocobalamin use for the treatment of suspected cyanide toxicity secondary to smoke inhalation.

Hydroxocobalamin is used for cyanide toxicity after smoke inhalation, but diagnosis is challenging. Retrospective studies have associated hydroxocobalamin with acute kidney injury (AKI). This is a retrospective analysis of patients receiving hydroxocobalamin for suspected cyanide toxicity. The primary outcome was the proportion of patients meeting predefined appropriate use criteria defined as ≥1 of the following: serum lactate ≥8 mmol/L, systolic blood pressure (SBP) <90 mmHg, new-onset seizure, cardiac arrest, or respiratory arrest. Secondary outcomes included incidence of AKI, pneumonia, re

Exploring the molecular mechanisms of nickel-induced genotoxicity and carcinogenicity: a literature review.

Nickel, a naturally occurring element that exists in various mineral forms, is mainly found in soil and sediment, and its mobilization is influenced by the physicochemical properties of the soil. Industrial sources of nickel include metallurgical processes such as electroplating, alloy production, stainless steel, and nickel-cadmium batteries. Nickel industries, oil- and coal-burning power plants, and trash incinerators have been implicated in its release into the environment. In humans, nickel toxicity is influenced by the route of exposure, dose, and solubility of the nickel compound. Lung i

Mercury in the Amazon.

Release and spreading of mercury from gold mining is a widespread problem in the Amazon area. Today we have rather good knowledge of the mercury situation. Tens of investigations have considered mainly concentrations in fish and human hair. Metallic mercury is used for amalgamation of gold, and the mercury is released by evaporation at reburning sites. The first extraction (burning) is performed in the field at the garimpos and the second (reburning) in gold shops in towns. This practice may cause severe exposure to elemental mercury by inhalation for people working with gold purification. Mer

Effects of crystalloid on lung fluid balance after smoke inhalation.

Inhalation injury occurs in 21% of flame burn victims who require large fluid volumes for resuscitation and have a mortality rate greater than 30%. This study was done to determine how vulnerable the smoke-injured lung is to fluid accumulation when crystalloids are infused rapidly. Mongrel dogs were exposed to smoke and 10% body-weight Ringer's lactate in three groups: (I) fluid only, (II) smoke only, and (III) smoke and fluid. The increase in wet-dry lung weight ratio was 2% in Group I, 28% in Group II, and 42% in Group III, consistent with pulmonary edema present only in Group III. The decre

Concentrations of trichloroethylene and its metabolites in blood and urine after acute poisoning by ingestion.

A 58-year-old man fell into a trichloroethylene reservoir bath head first, during a maintenance degreasing bath and accidentally ingested the solvent. Although he showed deep coma, chemical burns and pneumonia on admission, these symptoms gradually subsided. The concentrations of trichloroethylene (TRI) and its metabolites, trichloroethanol (TCE) and trichloroacetic acid (TCA) in blood and urine were measured during hospitalization. Eight hours after the accident, the concentrations of TRI and its metabolites in serum were 31.4 micrograms/ml TRI, 16.5 micrograms/ml TCE and 79.5 micrograms/ml T

Morphometric analysis of alveolar responses of F344 rats to subchronic inhalation of nitric oxide.

Nitric oxide (NO)*, the principal airborne pollutant generated from combustion processes such as gas stoves, tobacco smoke, and burning of fossil fuels, is being tested as a therapeutic agent in clinical trials. A prior morphometric study of rats exposed for 9 weeks to 0.5 parts per million (ppm) NO demonstrated focal degeneration of the alveolar interstitium and increased numbers of fenestrated alveolar septa (Mercer et al. 1995). The limited size and distribution of defects in this NO exposure did not alter alveolar surface area or other morphometric indicators of lung function, but were of

Diagnostic problems of the mucormycosis of the maxillary sinus Case presentation.

Mucormycosis is a fulminant opportunistic infection with significant mortality in susceptible individuals. Although mortality rates vary widely (30 to 100%) according to professional literature, recently in instances with no central nervous system (CNS) involvement the survival rate averages varies between 50 and 80% owing to complex therapy. With CNS involvement, however, the fatality rate is over 80%. Predisposing diseases include diabetes mellitus, malnutrition, hematologi- cal diseases, neutropenia, burns, surgical procedures, antibiotic treatments, long-term steroid therapy and immunosupp

Characterization of inhalation exposures at a wildfire incident during the Wildland Firefighter Exposure and Health Effects (WFFEHE) Study.

Wildland firefighters (WFFs) are exposed to many inhalation hazards working in the wildland fire environment. To assess occupational exposures and acute and subacute health effects among WFFs, the wildland firefighter exposure and health effects study collected data for a 2-year repeated measures study. This manuscript describes the exposure assessment from one Interagency Hotshot Crew (N = 19) conducted at a wildfire incident. Exposures to benzene, toluene, ethylbenzene, xylene isomers, formaldehyde, acetaldehyde, and naphthalene were measured through personal air sampling each work shift. Bi

Acute airway obstruction following facial scalding: differential diagnosis between a thermal and infective cause.

A 16-month-old boy received a 6 per cent hot water scald to his face and chest. The child had been suffering from an upper respiratory tract infection for 10 days prior to the injury. However, as the child rapidly developed respiratory tract obstruction, a differential diagnosis between acute infective epiglottitis and thermal injury to the hypopharynx was required. In fact, the hot water had reached the epiglottis and the aryepiglottic folds causing significant damage, whilst sparing the tongue and mouth. The child was admitted to the intensive care unit, intubated and ventilated for 4 1/2 da

Characterization of emissions from burning incense.

The primary objective of this study was to improve the characterization of particulate matter emissions from burning incense. Emissions of particulate matter were measured for 23 different types of incense using a cyclone/filter method. Emission rates for PM2.5 (particulate matter less than 2.5 microm in aerodynamic diameter) ranged from 7 to 202 mg/h, and PM2.5 emission factors ranged from 5 to 56 mg/g of incense burned. Emission rates were also determined using an electrical low pressure impactor (ELPI) and a small electrostatic precipitator (ESP), and emission rates were compared to those d
