Beyond the bench: Sociotechnical barriers to ergonomic change among interventional cardiology clinicians.

BACKGROUND: In high-volume image-guided procedural suites, clinicians work in constrained spaces requiring frequent twisting, reaching, and bending, contributing to fatigue and long-term musculoskeletal strain. OBJECTIVE: Within a broader evaluation of an over-the-patient worktable designed to improve procedural posture and reduce physical strain, this qualitative study examined how interventional cardiologists understand physical discomfort as part of their work and professional identity. METHODS: Semi-structured interviews were conducted with nine clinicians (1 female) at a quaternary care

Joint British Societies' position statement on cardiology training in the United Kingdom.

Cardiology training in the UK is facing significant challenges due to a range of factors. Recent curriculum changes have further compounded this issue and significantly risk the ability to produce adequately trained consultants capable of managing patients with increasingly complex cardiovascular disease. The introduction of mandatory dual accreditation in general internal medicine (GIM) alongside cardiology, by design, results in significantly reduced training opportunities, including procedural and subspecialty exposure. Despite prolongation in training duration to mitigate these effects, mo…

Cardiology Overview of Pulmonary Hypertension Management in the Neonatal Intensive Care Unit.

Pulmonary hypertension (PH) in the neonatal intensive care unit represents a complex and diverse spectrum of conditions, from transient persistent pulmonary hypertension of the newborn to chronic PH associated with bronchopulmonary dysplasia, congenital heart disease, and other conditions, and can cause significant morbidity and mortality. Proper classification, timely diagnosis using echocardiography and imaging, and tailored therapy are essential. A multidisciplinary approach is critical, particularly for infants with unclear or genetic causes. This article provides a thorough overview of th

Left Atrial Strain in Pediatric Cardiology: Evidence to Date and Future Directions.

Left atrial strain (LAS) is a sensitive marker of early atrial dysfunction and left ventricular diastolic abnormalities in pediatric heart disease, yet clinical integration remains limited. Evidence from 57 studies (2015-2024) including >5,300 patients <21 years of age demonstrates that healthy pediatric patients have higher LAS values, particularly left atrial conduit strain, than neonates and adults. Reduced left atrial reservoir and conduit strain is consistently reported in congenital heart disease and is associated with adverse outcomes across single-ventricle palliation stages. In pediat

Adenosine in Interventional Cardiology: Physiopathologic and Pharmacologic Effects in Coronary Artery Disease.

This review article focuses on the role of adenosine in coronary artery disease (CAD) diagnosis and treatment. Adenosine, an endogenous purine nucleoside, plays crucial roles in cardiovascular physiology and pathology. Its release and effects, mediated by specific receptors, influence vasomotor function, blood pressure regulation, heart rate, and platelet activity. Adenosine therapeutic effects include treatment of the no-reflow phenomenon and paroxysmal supraventricular tachycardia. The production of adenosine involves complex cellular pathways, with extracellular and intracellular synthesis

Pediatric Cardiology Fellowship Standards for Training in Exercise Medicine and Curriculum Outline.

Over the past 2 decades, fundamentals of exercise medicine, including clinical exercise testing, assessment and promotion of physical activity, exercise prescription, and supervised exercise training/rehabilitation programming have demonstrated considerable clinical value in the management of children and adolescents with congenital and acquired heart disease. Although the principles of exercise medicine have become an integral component in pediatric cardiology, there are no standardized training recommendations for exercise physiology during pediatric cardiology fellowship at this time. Thus,…

Making every contact count: recognising obesity in paediatric and young adult cardiology.

INTRODUCTION: With increased survival, children with CHD are reaching adulthood, however, obesity amongst this cohort is an emerging problem. Making every contact count encourages clinicians to utilise contact to elicit behaviour change. The aim of this work was to identify whether the body habitus of children classified as obese was addressed during a clinical review. METHODS: A retrospective observational cohort study was completed using a cardiology outpatient dataset from 2010 to 2019. Inclusion criteria are all children with a body mass index z score classified as obese (≥ 2 z scores). I…

Fabry disease in cardiology practice: Literature review and expert point of view.

Fabry disease is an X-linked progressive multisystemic genetic sphingolipidosis caused by deficient activity of lysosomal α-galactosidase A. Men aged>30 years and women aged>40 years most often present with unexplained left ventricular hypertrophy, usually concentric and non-obstructive, but sometimes mimicking sarcomeric hypertrophic cardiomyopathy, particularly when isolated, as in the cardiac or late-onset variant of the disease. In hypertrophic cardiomyopathy cohorts, up to 1% of patients have been diagnosed with Fabry disease. Frequent cardiac symptoms include chronotropic incompetence, s
