Assessment and early investigation of cauda equina syndrome- a systematic review of existing international guidelines and summary of the current evidence.

BACKGROUND CONTEXT: Cauda Equina Syndrome (CES) is an emergency clinical syndrome with correlating radiological evidence of spinal compression. Urgent recognition and treatment is key. In many units, it lacks a clear pathway of investigation and referral, despite a constantly expanding evidence base for its management. PURPOSE: The aim of this article is to provide a systematic review of currently available guidelines internationally, and also an up-to-date review of current key evidence on topics central to the accurate assessment and investigation of CES. STUDY DESIGN: A systematic review …

Recurring Paralysis and a Race Against Time: A Case of Cauda Equina Syndrome With Delayed Diagnosis and Incomplete Neurological Recovery.

Cauda equina syndrome (CES) is a rare but critical neurological condition caused by compression of the cauda equina, a collection of lumbosacral nerve roots responsible for lower limb function along with bowel and bladder control. While CES often presents as a sudden and severe onset of symptoms, it can also develop insidiously, mimicking other conditions in its early stages, such as lumbar radiculopathy, spinal stenosis, or urinary tract disorders, often contributing to delayed recognition. Given the variability in clinical presentation, early recognition of suggestive signs and symptoms is c…

A Report of Cauda Equina Syndrome Caused by Spinal Epidural Hematoma, a Complication of Deep Vein Thrombosis (DVT) Management.

This case report delves into the infrequent yet substantial occurrence of cauda equina syndrome (CES) arising from a spinal epidural hematoma (SEH), a potential complication during deep vein thrombosis (DVT) treatment. An 83-year-old female patient previously diagnosed with various medical conditions, including moderate spinal stenosis, chronic kidney disease, and chronic lower extremity stasis, was detected with notable iliofemoral DVT during an office-based venous study. The patient was urgently referred to the Emergency Department. Following hospitalization, her cardiologist diagnosed DVT, …

Dorsal migration of lumbar disc fragments causing cauda equina syndromes: A three case series and literature review.

BACKGROUND: Dorsal migration of an intervertebral lumbar disc fragment is exceedingly rare and may result in spinal cord or cauda equina compression. Radiologically, these lesions may be misdiagnosed as extradural masses or epidural hematomas. CASE DESCRIPTION: We present three cases involving dorsal migration of sequestered lumbar disc fragments resulting in cauda equina syndromes. A 31-year-old male, 79-year-old female, and 47-year-old female presented with cauda equina syndromes attributed to the migration of dorsal sequestered lumbar disc fragments. Prompt surgical decompression resulted …

Cauda equina syndrome following an uneventful spinal anesthesia in a patient undergoing drainage of the Bartholin abscess: A case report.

RATIONALE: Neuraxial anesthesia is a commonly used type of regional anesthesia. Cauda equina syndrome is an unusual and severe complication of neuraxial anesthesia, and is caused by damage to the sacral roots of the neural canal. We present a case of cauda equina syndrome following spinal anesthesia in a patient who underwent Bartholin abscess drainage. PATIENT CONCERNS: A 23-year old female scheduled to undergo surgical drainage of Bartholin abscess. Spinal anesthesia was performed with bupivacaine and fentanyl. There were no perioperative adverse events reported. On postoperative day 1, the…

Misplacement of Stent Into Epidural Venous Plexus With Resultant Cauda Equina Syndrome and Open Surgical Treatment: A Case Report.

BACKGROUND AND IMPORTANCE: Endovascular therapy has proven to be a safe, minimally invasive treatment for multiple etiologies, but proper precautions must be taken to avoid complications. When complications occur, they should be promptly identified and corrected when possible. This case report describes endovascular stents misplaced into the epidural spinous venous plexus rather than the iliofemoral arteries, causing cauda equina syndrome, as well as the spinal procedure performed to treat the resulting spinal canal compression. CLINICAL PRESENTATION: A 67-yr-old man had undergone what he tho…

A rare case of multiple spinal epidural abscesses and cauda equina syndrome presenting to the emergency department following acupuncture.

BACKGROUND: Acupuncture is a form of traditional Chinese medicine being increasingly used as complementary therapy in many countries. It is relatively safe and rarely associated with deep infections. CASE PRESENTATION: In this case report, we describe a middle-aged Chinese patient who presented acutely to our emergency department with cauda equina syndrome secondary to acupuncture-related epidural abscesses, which were treated with surgical decompression and intravenous antibiotics. We also present a review of case reports of this rare condition in available literature. CONCLUSION: Emergency…

Hydroxyurea therapy in paraparesis and cauda equina syndrome due to extramedullary haematopoiesis in thalassaemia: improvement of clinical and haematological parameters.

Patients with beta-globin disorders show amelioration of clinical condition by sustained synthesis of fetal haemoglobin in adult life. We report data on a patient with beta(o)-thalassaemia genotype and thalassaemia intermedia clinical phenotype. He received therapy with hydroxyurea (20 mg/kg/d) because of the presence of extramedullary masses causing paraparesis, neurogenic bladder and impotence. During therapy, the patient showed an improved clinical picture and a significant increase in total Hb (from 71.8 to 103.2 g/L) and a gamma/alpha globin synthetic ratio (from 0.39 to 0.68). The myelos

[A case of Miller Fisher syndrome with gadolinium-enhancing lesions in the cranial nerves and the cauda equina on magnetic resonance imaging].

MRI findings for a patient with Miller Fisher syndrome (MFS) are reported. A 25-year-old woman presented with blephaloptosis, double vision, and gait disturbance after antecedent upper respiratory infection. Neurologic examination on admission revealed bilateral blephaloptosis and restriction of extraocular movement. She was unable to maintain an upright posture, nor able to walk because of severe truncal ataxia. Mild limb ataxia was also seen. The tendon reflexes were generally absent and she had hypesthesia in her shoulder and upper limbs. On the next day, her eyes became fixed in the centra

Stent Graft Intervention in a Lumbar Artery Pseudoaneurysm Improved Cauda Equina Syndrome: A Case Report of Combined Embolization Strategy.

A lumbar artery pseudoaneurysm following a lumbar vertebral compression fraction is rare. Cauda equina syndrome due to spinal canal compression is a rarer complication of this pseudoaneurysm. Endovascular treatment, which involves embolization of the inflow and outflow vessels, is the first-line treatment for lumbar artery pseudoaneurysms. We report a case of a lumbar artery pseudoaneurysm with spinal canal compression that was treated with coil embolization of the outflow vessel and stent graft placement to occlude the inflow vessel. After treatment, the symptoms of cauda equina syndrome impr…

Unintentional Epidural Anesthesia Mimicking Cauda Equina Syndrome from Suprafascial Injection of Liposomal Bupivacaine After Lumbar Foraminotomy: A Case Report.

CASE: This case report describes a 46-year-old woman undergoing right-sided L5 to S1 decompression who received liposomal bupivacaine (LB) for postoperative analgesia and developed unintentional epidural anesthesia with symptoms mimicking cauda equina syndrome. The patient's symptoms resolved 72 hours postoperatively, approximately the length that LB typically lasts. At the 16-month follow-up, the patient demonstrated complete neurological function with no lower extremity strength or sensation deficits. CONCLUSIONS: Tracking of LB into the epidural space after lumbar surgery may cause transie…
