Stakeholder-reported perspectives on decentralized clinical trials: A multi-country descriptive survey.

Decentralized clinical trials (DCTs), which enable trial activities to occur outside traditional clinical trial sites, are reshaping the landscape of clinical research by enabling patient-centric, flexible, and digitally supported trial models. However, widespread adoption faces regulatory, operational, and technological challenges. This study aimed to assess stakeholder perceptions, implementation trends, benefits, and challenges of DCTs across various settings. A cross-sectional, mixed-methods online survey using convenience and snowball recruitment was conducted among 288 stakeholders invol

Integrated Single-Cell and Machine Learning Analysis Identifies Fibroblast-Associated Hub Genes and Potential Therapeutics in Ischemic Cardiomyopathy.

Ischemic cardiomyopathy (ICM), a leading cause of heart failure, is characterised by complex cellular heterogeneity and a dysregulated microenvironment. A systematic computational dissection of its molecular mechanisms and a coherent pipeline from discovery to potential therapeutics is currently lacking. We integrated single-cell RNA sequencing (scRNA-seq) data from ICM patients with four independent bulk transcriptomic cohorts. A cardiac cellular atlas was constructed, and candidate genes were filtered through differential expression analysis. Subsequently, a benchmark of 127 machine learning

Quercetin ameliorates cyclophosphamide-induced premature ovarian insufficiency by modulating SIRT1/HIF-1α pathway.

BACKGROUND: Premature ovarian insufficiency (POI) is characterized by complex etiologies and currently lacks effective treatments, posing significant threats to women’s physical and psychological health. Quercetin (QUE), a bioactive flavonoid inherent in traditional herbs such as Flos Sophorae and Cuscuta chinensis, demonstrates potent redox-modulating and anti-senescence capacities. This investigation validates its therapeutic efficacy against POI, specifically elucidating granulosa cell-centric mechanisms. METHODS: POI mouse models and injury models of the KGN (human ovarian granulosa cell l

Interactions Between E3 Ubiquitin Ligases and Deubiquitinases in AKT Regulation: Implications in Skeletal Muscle Homeostasis.

AKT (protein kinase B, PKB) coordinates the balance between anabolic and catabolic signaling in skeletal muscle through distinct ubiquitin chain types. Some E3 ubiquitin ligases (E3s) and deubiquitinases (DUBs) form stable binary complexes via non-catalytic interfaces, adding a regulatory layer unavailable to either enzyme alone. This mechanistic synthesis review presents a systematic literature analysis (inception to May 2026; 26 eligible studies). It identified four E3-DUB pairs proposed to regulate AKT in skeletal muscle. These are TRAF6-CYLD (plasma-membrane K63-ubiquitination), MUL1-USP9X

Investigating the Risk of Obesity in European Night Shift Workers: A Study Protocol for Cross-Sectional and Mechanistic Studies in the SHIFT2HEALTH Project.

INTRODUCTION: Approximately one in five workers in Europe is engaged in shift work. Studies reveal that night shift work leads to an increased risk of overweight, obesity, and related diseases. Yet, the biological and behavioural mechanisms underlying these associations are not fully understood. The cross-sectional and mechanistic studies within the European SHIFT2HEALTH project aim to investigate biological, behavioural, and psychosocial key risk factors responsible for the association between night work and obesity across five European countries. METHODS: A multi-centric cross-sectional stu

hucMSC-derived exosomes targeting macrophage polarization attenuate systemic inflammation in T1DM via INS/SOD1 delivery.

BACKGROUND: Type 1 diabetes mellitus (T1DM) is a chronic autoimmune disease characterized by absolute insulin (INS) deficiency. As key components of the immune system, macrophages play critical roles in T1DM-associated pancreatic β-cell damage and multiorgan inflammatory injuries. Exosomes derived from human umbilical cord mesenchymal stem cells (hucMSC-EXOs) have emerged as promising therapeutic agents for immune-related disorders due to their immunomodulatory properties and favorable safety profile. However, systematic investigations into the therapeutic potential of hucMSC-EXOs in T1DM are

Retinal alterations resemble brain pathology in a rat model of Parkinson's disease induced by intranigral infusion of α-synuclein oligomers.

Parkinson's disease (PD) is a debilitating neurodegenerative synucleinopathy, characterized by dopaminergic degeneration, pathological deposition of alpha-synuclein (α-Syn), and neuroinflammation in both motor regions of the midbrain and non-motor areas of the cortex. Despite its motor-centric characterization, visual disturbances such as hallucinations, diplopia, altered contrast sensitivity and retinal abnormalities are well-documented non-motor changes of PD. While this evidence points to neuropathological processes in PD that extend beyond the brain, the neuropathological basis of retinal

Gut-Brain Interactions in Neuronal Ceroid Lipofuscinoses: A Systematic Review Beyond the Brain in Paediatric Dementias.

Neuronal ceroid lipofuscinoses (NCLs) are paediatric neurodegenerative disorders that primarily affect the central nervous system (CNS). The high prevalence of gastrointestinal (GI) symptoms has prompted researchers and clinicians to move beyond an exclusively "brain-centric" perspective. At the molecular level, mutations in CLN genes lead to lysosomal dysfunction and impaired autophagy, resulting in intracellular accumulation of storage material that disrupts both central and enteric neuronal homeostasis. To systematically examine current clinical and preclinical knowledge on gut involvement

The double life of glucose metabolism: brain health, glycemic homeostasis, and your patients with type 2 diabetes.

The maintenance of cognitive function is essential for quality of life and health outcomes in later years. Cognitive impairment, however, remains an undervalued long-term complication of type 2 diabetes by patients and providers alike. The burden of sustained hyperglycemia includes not only cognitive deficits but also the onset and progression of dementia-related conditions, including Alzheimer's disease (AD). Recent research has shown that the brain maintains an independent glucose "microsystem"-evolved to ensure the availability of fuel for brain neurons without interruption by transient hyp

Design of a placebo-controlled, randomized study of the efficacy of repetitive transcranial magnetic stimulation for the treatment of chronic tinnitus.

BACKGROUND: Chronic tinnitus is a frequent condition, which can have enormous impact on patient's life and which is very difficult to treat. Accumulating data indicate that chronic tinnitus is related to dysfunctional neuronal activity in the central nervous system. Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive method which allows to focally modulate neuronal activity. An increasing amount of studies demonstrate reduction of tinnitus after repeated sessions of low-frequency rTMS and indicate that rTMS might represent a new promising approach for the treatment of tinnitu
