[The value of studying the cervical spine in craniofacial asymmetries].

INTRODUCTION: The relationship between facial asymmetry and cervical anomaly is rarely mentioned in the diagnosis of dento-maxillo-facial orthopaedics. It is regrettable that the study of the cervical spine is often ignored in the etio-pathogenesis of these dysmorphoses, particularly in cases of facial asymmetry. OBJECTIVE: The aim is twofold: to encourage orthodontists and maxillofacial surgeons to make a systematic study of the cervical spine in craniofacial dysmorphoses and in particular craniofacial asymmetries, without claiming that they are becoming specialists in cervical spine patholo

Upper cervical spine injuries: indications and limits of the conservative management in Halo vest. A systematic review of efficacy and safety.

INTRODUCTION: The integrity of the upper cervical spine is essential for survival and function, because of the neurovascular structures contained within its bony elements. Fractures of the upper cervical spine (C1-C2) are frequent. This systematic review assesses the efficacy and safety of the conservative management in Halo vest for patients with upper cervical spine fractures. MATERIALS AND METHODS: Two reviewers independently identified studies in English, by a systematic search of CINAHL, Embase, Medline, HealthSTAR, and the Cochrane Central Registry of Controlled Trials, from inception o

Transoral fusion with internal fixation in a displaced hangman's fracture.

STUDY DESIGN: A case is reported in which late displacement of a "hangman's fracture" was managed by transoral C2-C3 fusion by using bicortical iliac crest graft and a titanium cervical locking plate. OBJECTIVES: To review the management of unstable fractures of the axis and to study other reports of transoral instrumentation of the cervical spine. SUMMARY OF BACKGROUND DATA: Undisplaced fractures of the axis are considered to be stable injuries. Although late displacement is unusual, it can lead to fracture nonunion with persisting instability and spinal cord dysfunction. In this situation,

Radiological evaluation of C1 pedicle screw anatomic feasibility.

C1 pedicle screw instrumentation is a recently documented technique, which may have benefits over other more popularised procedures, however it may not be possible in all patients. This study aims to investigate the applicability of the C1 pedicle screw technique to a cohort of patients through assessment of radiological parameters. A retrospective review of 150 consecutive patients undergoing computer tomography (CT) of the cervical spine was performed. Based on defined parameters images were assessed for feasibility of placement of pedicle screws. C1 pedicle height (PH), pedicle width (PW),

The proatlas: a comprehensive review with clinical implications.

BACKGROUND: The proatlas is derived from the fourth occipital sclerotome in human embryos. It usually fuses with the three upper occipital sclerotomes to form the occipital bone. However, this does not always occur. Manifestations of a partial proatlas structure may persist due to failure of fusion. CLINICAL CONSIDERATIONS: These embryological remnants can induce several symptoms in humans, ranging from mild to severe. On occasion, this structure can go unnoticed until a precipitating traumatic event results in symptoms. Proatlas segmentation abnormalities form bony masses at C1 and the foram

Extended transoral approaches: surgical technique and analysis.

BACKGROUND: The transoral approach provides the most direct exposure to extradural lesions of the ventral craniovertebral junction. Lesions that extend beyond the exposure provided by the standard transoral approach require an extended transoral modification. The exposure can be expanded in the sagittal and axial planes by adding mandibulotomy, mandibuloglossotomy, palatotomy, and transmaxillary approaches to the standard transoral approach. Extended transoral approaches increase the surgical complexity and the risk of cosmetic and functional complications. Until recently, selection of an exte

A vertebral artery tortuous course below the posterior arch of the atlas (without passing through the transverse foramen). Anatomical report and clinical significance.

A vertebral artery (VA) coursing below the posterior arch of the atlas (C1) without passing through the transverse foramen of C1, combined with a tortuous course within the spinal canal has rarely been reported in the literature. This article describes a case encountered during an anatomical study of the far-lateral approach, and reviews its embryonic development and clinical significance. The suboccipital triangle was filled with numerous venous plexures. After exiting from the transverse foramen of C2, instead of passing upwards through the transverse foramen of C1, the VA turned directly me

Traumatic lesions of the occipitoatlantoaxial complex.

Injuries of the occipitoatlantoaxial complex are relatively rare but form a group of ligamentous and bony lesions that may unnecessarily confuse the clinician. Since fractures of the dens and traumatic spondylolisthesis of the axis have been well described in a number of large series, this article concentrates on the lesions less frequently seen and described. The ligamentous injuries are occipitoatlantal dislocation, rupture of the transverse ligament, and atlantoaxial rotatory fixation. The bony injuries are fractures of the occipital condyles, atlas, and the lateral masses of C2. The incide

Atlantoaxial instability in Marfan's syndrome. Diagnosis and treatment. A case report.

A case of Marfan's syndrome is described in which progressive neurological signs and symptoms were produced by flexion of the head. Radiological examinations revealed hypermotility between the atlas and axis, as well as compression of the medulla oblongata by the odontoid process on flexion, since low positioned cerebellar tonsils prevented dorsal shift of the oblongata. The signs and symptoms disappeared after occipitocervical internal transfixation.

Biomechanical Rationale for the Development of Atlantoaxial Instability and Basilar Invagination in Patients with Occipitalization of the Atlas: A Finite Element Analysis.

OBJECTIVE: Occipitalization of the atlas (OA) often is associated with atlantoaxial dislocation and basilar invagination. The purpose of this study is to determine the biomechanical difference between normal and OA conditions in the craniovertebral junction and to further explore the rationale for development of atlantoaxial dislocation and basilar invagination using the finite element model (FEM). METHODS: A ligamentous, nonlinear, sliding-contact, 3-dimensional FEM of the occipitoatlantoaxial complex was generated. Validation of the model was accomplished by comparing kinematic predictions

Distraction Arthrodesis of the C1-C2 Facet Joint with Preservation of the C2 Root for the Management of Intractable Occipital Neuralgia Caused by C2 Root Compression.

STUDY DESIGN: Prospective observational cohort study. OBJECTIVE: To compare the outcomes of our new technique, distraction arthrodesis of C1-C2 facet joint with C2 root preservation (Study group), to those of conventional C1-C2 fusion with C2 root transection (Control group) for the management of intractable occipital neuralgia caused by C2 root compression. SUMMARY OF BACKGROUND DATA: We are not aware of any report concerning C2 root decompression during C1-C2 fusion. MATERIALS AND METHODS: Inclusion criteria were visual analogue scale (VAS) score for occipital neuralgia 7 or more; C2 root

Craniocervical hyperpneumatization with concurrent pneumorrhachis, pneumomediastinum, and subcutaneous emphysema in a weightlifter.

BACKGROUND CONTEXT: Acquired hyperpneumatization of the skull base and upper cervical vertebrae is extremely rare and is thought to occur in patients who habitually perform the Valsalva maneuver or engage in repetitive positive pressure activities such as scuba diving or free diving. Craniocervical hyperpneumatization has been reported to cause intracranial and extracranial pneumatoceles but is not generally considered as a cause of pneumorrhachis (air in the spinal canal). Pneumorrhachis is relatively rare, and usually occurs in a localized form, either in the cervical spine secondary to skul

Stabilization of the atlantoaxial joint with C1-C3 lateral mass screw constructs: biomechanical comparison with standard technique.

BACKGROUND: Anatomically and biomechanically, the atlantoaxial joint is unique compared with the remainder of the cervical spine. OBJECTIVE: To assess the in vitro stability provided by 2 C2 screw sparing techniques in a destabilized model of the atlantoaxial joint and compare with the gold standard system. METHODS: The 3-dimensional intervertebral motion of 7 human cadaveric cervical spine specimens was recorded stereophotogrammetrically while applying nonconstraining, nondestructive pure moments during flexion-extension, left and right axial rotation, and left and right lateral bending. Atlantoaxial instability in a white-tailed deer fawn (Odocoileus virginianus).

On 14 March 2001, an 8 mo old, male white-tailed deer (Odocoileus virginianus) was found in lateral recumbency exhibiting neurologic signs including inability to rise, opisthotonus, paddling, and respiratory distress. There was evidence of minor cranial trauma. Postmortem examination revealed atlantoaxial instability with ventral deviation of the axis due to malformation of the caudal atlas and cranial axis. Given the age of the fawn, the instability was assumed to be congenital with minor trauma inducing severe, acute neurologic signs.
