Bystanders' attitudes towards drone delivered Automated External Defibrillators for out-of-hospital cardiac arrest: A qualitative interview study.

BACKGROUND: Early cardiopulmonary resuscitation (CPR) and defibrillation with Automated External Defibrillators (AEDs) by the public at an out-of-hospital cardiac arrest (OHCA) increases patient survival, but AEDs are infrequently used. Using drones to deliver AEDs may be one way to increase uptake, but there is limited understanding about what members of the public think about this. The aim of the study was to explore public attitudes towards drone AED delivery for OHCA. METHODS: We conducted 14 remote, semi-structured interviews with real-life OHCA bystanders. Participants were recruited vi

Self-pay insurance status is associated with failure of medical therapy in patients with acute uncomplicated type B aortic dissection.

BACKGROUND: Traditionally, acute uncomplicated type B aortic dissections are managed medically, and acute complicated dissections are managed surgically. Self-pay patients with medically managed acute uncomplicated type B aortic dissections may fare worse than their insured counterparts. METHODS: In this single-center, retrospective cohort study, demographics, follow-up, and outcomes of patients with acute type B aortic dissections from 2011 to 2020 were analyzed. RESULTS: In total, 159 patients presented with acute type B aortic dissections; 102 were complicated and managed with thoracic en

Understanding the associations between owner and pet demographics on pet body condition among those experiencing homelessness and housing vulnerability in Canada.

Approximately 35 000 people experience homelessness in some form each night in Canada, with similar rates (approximately 1/200 individuals) among developed countries. Ten to twenty percent of those individuals are pet owners. Animal companionship provides a variety of mental and physical benefits to people who are living homeless or vulnerably housed, but many in the non-vulnerable sector, including veterinary professionals and animal welfare advocates, express concern for the health and welfare of these animals. We describe the demographics of a population of animals owned by individuals expe

Smallpox Vaccine and Resident Responses in Modern Shanghai: Focusing on Regional and Cultural Comparison.

In modern Shanghai, smallpox was one of the most threatening diseases with its mortality rate going up to 30 percent. In response to the disease, Dr. William Lockhart, a medical missionary of the London Missionary Society, introduced vaccination to the Chinese people in Shanghai. He built the first western style hospital in Shanghai, Renji Hospital, in 1843. At this time, native doctors also played a very important role. For example, Huang Chun-pu was in charge of the dispensary in the Chinese City in Shanghai, and he was the one who introduced vaccination under Dr. Lockhart's direction. In 18

Infantile and adult mortality in precarious conditions.

INTRODUCTION: Maxillofacial surgery plays an important role in humanitarian surgery because this specialty includes common procedures such as facial reconstruction after burns, facial clefts or Noma's sequelae, which do not require complex infrastructures and thus can be carried out in sub-optimal conditions. Nevertheless, there is a mortality rate during those missions and few studies evaluating their rate and circumstances have been published. The aim of our study was threefold: to study the deaths reported during humanitarian missions, to analyze the circumstances of those deaths, and to de

Laser peripheral iridotomy for the prevention of angle closure: a single-centre, randomised controlled trial.

BACKGROUND: Primary angle-closure glaucoma affects 20 million people worldwide. People classified as primary angle closure suspects have a higher but poorly quantified risk of developing glaucoma. We aimed to assess efficacy and safety of laser peripheral iridotomy prophylaxis against primary angle-closure glaucoma in Chinese people classified as primary angle closure suspects. METHODS: In this randomised controlled trial, bilateral primary angle closure suspects aged 50-70 years were enrolled at the Zhongshan Ophthalmic Center, a tertiary specialised hospital in Guangzhou, China. Percutaneous coronary intervention in stable angina (ORBITA): a double-blind, randomised controlled trial.

BACKGROUND: Symptomatic relief is the primary goal of percutaneous coronary intervention (PCI) in stable angina and is commonly observed clinically. However, there is no evidence from blinded, placebo-controlled randomised trials to show its efficacy. METHODS: ORBITA is a blinded, multicentre randomised trial of PCI versus a placebo procedure for angina relief that was done at five study sites in the UK. We enrolled patients with severe (≥70%) single-vessel stenoses. After enrolment, patients received 6 weeks of medication optimisation. Patients then had pre-randomisation assessments with car

A literature-based intervention for older people living with dementia.

BACKGROUND: While several studies have explored the impact of literature and reading on mental health, there has been relatively little work done on how a literature-based intervention might impact on the behaviours of those living with dementia. The present report addresses the effect that a specific literature-based intervention - Get into Reading, designed and practised by national charity The Reader Organisation - might have on the health and well-being of people living with dementia. AIMS: This present study arises out of a service evaluation that specifically assessed to what extent the

Use of an online survey to explore positive and negative outcomes of rehabilitation for people with CFS/ME.

PURPOSE: First, to explore the experiences of people with Chronic Fatigue Syndrome/Myalgic Encephalomyelitis (CFS/ME) of rehabilitation therapies so as to build an understanding of reasons for the discrepancy between the notably mixed experiences regarding effectiveness reported in patient surveys and the RCT evidence about the efficacy of Graded Exercise Therapy (GET). GET is a form of structured and supervised activity management that aims for gradual but progressive increases in physical activity. Second, to review patient experiences of two related rehabilitation approaches, Exercise on Pr

Improving the detection, assessment, management and prevention of delirium in palliative care units (the DAMPen-Delirium II study): protocol for an implementation-to-target Type III hybrid effectiveness-implementation cluster randomised-controlled trial, economic evaluation and process evaluation of the Creating Learning Environments for Compassionate Care-Palliative (CLECC-Pal) implementation strategy.

BACKGROUND: Delirium affects up to two-thirds of palliative care unit (PCU) inpatients, causing distress for patients, families, and staff, and contributing to complex care needs and increased healthcare resource use. Although evidence-based guidelines exist, their consistent implementation in PCUs is limited, and implementation strategies to support guideline-adherent care have not been rigorously evaluated in this setting. Creating Learning Environments for Compassionate Care-Palliative Delirium (CLECC-Pal Delirium) is a co-designed multi-component implementation strategy to help embed guide

Comparing user satisfaction with grief-informed texts to other types of bereavement support in the United Kingdom.

BACKGROUND: Grief following the death of a loved one is a painful human experience, and yet bereaved people often have trouble getting the right grief support. In 2022, the UK Commission on Bereavement found significant deficits in the availability, accessibility, length, and helpfulness of bereavement services. Around that time, a national charity in the UK piloted Grief Coach, a low-cost, scalable, text message-based grief support program with the potential to fill gaps in bereavement services. This research assessed user satisfaction with Grief Coach, how user satisfaction ratings compared

Invasive Arterial Blood Pressure Monitoring to Detect Postintubation Hypotension in Patients Who Receive a Prehospital Emergency Anesthetic for Suspected Traumatic Brain Injury.

OBJECTIVE: The prehospital management of moderate/severe traumatic brain injury (TBI) centers on preventing secondary brain injury. Prehospital emergency anesthesia (PHEA) may be required for optimal neuroprotective care. Continuous invasive arterial blood pressure (IBP) monitoring is increasingly used in this cohort. PHEA can result in significant blood pressure (BP) changes, particularly around induction. IBP allows targeted BP management. This study analyzed hypotension frequency, depth, and duration in patients with suspected TBI monitored with IBP before PHEA. METHODS: This was a retrosp

A multiple-methods study of the SKCIN Sun Safe Schools international programme 2012-23: promoting sun safety and empowering behavioural change in primary education.

BACKGROUND: Sun protection education in schools is crucial to prevent skin cancer; however, there is a lack of published international literature regarding this topic. The Karen Clifford Skin Cancer (SKCIN) charity developed and launched the online and openly available education programme called 'Sun Safe Schools' (SSS) in 2012. SSS has four steps and is targeted at the English primary school curriculum. OBJECTIVES: To evaluate the outcomes of SSS by reporting regional distribution and exploring themes from online feedback surveys. METHODS: Descriptive statistics of school registration data

Adolescents' Preferences, Experiences, and Views of 2 Online Programs for Anxiety and Depressive Symptoms: Qualitative Interview Study.

BACKGROUND: Anxiety and depression are among the most prevalent mental health problems affecting adolescents. Digital mental health interventions have emerged as promising means of addressing these challenges. Understanding adolescents' experiences and perspectives, and incorporating them into intervention design, can enhance their relevance, engagement, and effectiveness, ensuring interventions meet young people's needs. OBJECTIVE: This qualitative study aimed to identify how adolescents aged 16-19 years engaged with 2 distinct interventions (Bite Back [Black Dog Institute] and Project YES [

Clinical and cost-effectiveness of the iStep-MS physical activity and sedentary behaviour intervention for managing fatigue in people with multiple sclerosis: protocol for a multicentre randomised controlled trial.

INTRODUCTION: Regular physical activity and limiting sedentary behaviour are important aspects in managing multiple sclerosis (MS). Fatigue is a common and disabling symptom in MS, contributing to impairments in activities of daily living and poorer quality of life. This study aims to determine the effectiveness of a physical activity and sedentary behaviour intervention, called iStep-MS, for reducing fatigue in people with MS when delivered across the MS care pathway. METHODS AND ANALYSIS: This is a multicentre, two-arm randomised controlled superiority trial with embedded economic and proce

Co-producing data-intensive research with an underserved group: a case study and evaluation identifying pathways to impact.

INTRODUCTION: Co-production of research, where researchers and experts by experience work as equal partners throughout a research project, can improve the quality, relevance, implementation and impact of research. However, there is limited evidence on methods for successful co-production in data-intensive research with underserved groups. In partnership with the charity Voice of Young People in Care (VOYPIC) and a group of care experienced young people, the Administrative Data Research Centre Northern Ireland (ADRC NI) piloted and evaluated a co-production approach in a research project that u

Shortening duration of untreated illness in young people with first episode eating disorders: protocol of a randomised controlled feasibility trial of a smartphone friendly multi-modal decision-making tool (FREED-M) to improve help-seeking.

BACKGROUND: Early intervention gives young people the best chance to recover from eating disorders (EDs). An important focus of early intervention is shortening the time between a person first developing symptoms and starting treatment (duration of untreated eating disorder; DUED). Patient-related factors (e.g. poor mental health literacy and help-seeking difficulties) are strongly associated with DUED. The aims of our study are to co-design and test the feasibility of FREED-Mobile (FREED-M), an online intervention tool for young people with early-stage EDs. This tool aims to improve knowledge

Breaking the cycle: a pilot study on autonomous Digital CBTe for recurrent binge eating.

BACKGROUND: Only a minority of people with eating disorders receive evidence-based psychological treatment. This is especially true for those with recurrent binge eating because the shame that accompanies binge eating affects help seeking and there is a shortage of therapists to provide psychological treatments. Digital programme-led interventions have the potential to overcome both barriers. OBJECTIVE: This study examined the acceptability and effectiveness of a new digital programme-led intervention directly based on enhanced cognitive behaviour therapy (CBT-E), which is an empirically supp

Composition of the neutralising antibody response predicts risk of BK virus DNAaemia in recipients of kidney transplants.

BACKGROUND: BK polyomavirus (BKV) DNAaemia occurs in 10% of recipients of kidney transplants, contributing to premature allograft failure. Evidence suggests disease is donor derived. Hypothetically, recipient infection with a different BKV serotype increases risk due to poorer immunological control. Thus, understanding the composition and activity of the humoral anti-BKV responses in donor/recipient (D/R) pairs is critical. METHODS: Using 224 paired pre-transplant D/R samples, BKV VP1 genotype-specific pseudoviruses were employed to define the breadth of the antibody response against differen
