Computerized history-taking improves data quality for clinical decision-making-Comparison of EHR and computer-acquired history data in patients with chest pain.

Patients' medical histories are the salient dataset for diagnosis. Prior work shows consistently, however, that medical history-taking by physicians generally is incomplete and not accurate. Such findings suggest that methods to improve the completeness and accuracy of medical history data could have clinical value. We address this issue with expert system software to enable automated history-taking by computers interacting directly with patients, i.e. computerized history-taking (CHT). Here we compare the completeness and accuracy of medical history data collected and recorded by physicians i

Selective use of RAI for ablation and adjuvant therapy after total thyroidectomy for differentiated thyroid cancer: a practical approach to clinical decision making.

OBJECTIVES: For the past 40 years, many clinicians have recommended RAI remnant ablation for essentially all differentiated thyroid cancer patients with a primary tumor size greater than 1.5 cm or with any evidence of even microscopic disease outside the thyroid capsule. This "one size fits all" approach exposes many low risk thyroid cancer patients to the risks of ionizing radiation with little potential benefit. Current thyroid cancer management guidelines call for a far more risk adapted approach to the selection of patients for post-operative RAI treatment. MATERIALS AND METHODS: We will

Decision-making in operative vaginal delivery: when to intervene, where to deliver and which instrument to use? Qualitative analysis of expert clinical practice.

OBJECTIVE: To identify the decision-making process involved in determining when to intervene, where to deliver and the optimal choice of instrument for operative vaginal deliveries in the second stage of labour. STUDY DESIGN: A qualitative study using interviews and video recordings took place at two university teaching hospitals (St. Michael's Hospital Bristol and Ninewells Hospital, Dundee). Ten obstetricians and eight midwives were identified as experts in conducting or supporting operative vaginal deliveries. Semi-structured interviews were carried out using routine clinical scenarios. Surgical decision-making affected by clinical and genetic screening of a novel kindred with von Hippel-Lindau disease and pancreatic islet cell tumors.

OBJECTIVE: We report a unique, previously undescribed multigeneration kindred with von Hippel-Lindau (VHL) disease in whom clinical or genetic screening led to the detection of surgically resectable neoplastic disease in several family members. SUMMARY BACKGROUND DATA: Patients with VHL disease have a propensity to develop neoplasms of several different organ sites. Retinal angiomas, cerebellar and spinal hemangioblastomas, solid organ cysts, and renal carcinoma are common lesions; pheochromocytomas and pancreatic islet cell tumors occur less frequently but are important causes of morbidity a

Comparison of educational outcomes and factors associated with clinical decision-making ability between virtual reality and simulation-based training among undergraduate nursing students.

BACKGROUND: Clinical decision-making is a core competency in nursing practice, yet opportunities for direct patient care during clinical placements have become increasingly limited. Although virtual reality (VR) and simulation-based learning (SBL) are widely used in nursing education, comparative evidence regarding their educational outcomes and predictive factors remains limited. This study compared educational outcomes between VR training and simulation-based training and identified factors associated with clinical decision-making ability among undergraduate nursing students. METHODS: This …

4D Flow Yields Similar Clinical Results Compared To 2D Phase Contrast for Decision Making Regarding Pulmonary Valve Replacement in Repaired Tetralogy of Fallot.

2D phase contrast (PC) CMR is critical for monitoring pulmonary regurgitant fraction (PRF) in patients with a history of repaired tetralogy of Fallot (rTOF) and guides decisions about pulmonary valve replacement (PVR). However, newer techniques such as 3D time-resolved phase contrast (4D flow) MRI are gaining momentum due to the added clinical value beyond conventional flow metrics. While agreement between 2D PC CMR and 4D flow is well-established, the real-world implications for clinical decision-making have not been fully studied and are thus the primary aim of this study. All patients with …

A 40-Year-Old Man With 2-Year Chronic Epigastric Pain From a Retained 18-cm Toothbrush: Endoscopic Management and Clinical Decision-Making.

BACKGROUND Foreign body ingestion is common in children, whereas ingestion of non-food objects in adults is uncommon. Toothbrush ingestion is particularly rare. Because of their length and rigidity, toothbrushes are unlikely to pass spontaneously through the gastrointestinal tract and can cause pressure injury, ulceration, perforation, or fistula if retained. We report a case of chronic epigastric pain caused by long-term retention of a swallowed toothbrush and highlight the value of endoscopy when radiologic findings appear more severe than the clinical presentation. CASE REPORT A 40-year-old

Psychopathology, psychopharmacological properties, decision-making capacity to consent to clinical research and the willingness to participate among long-term hospitalized patients with schizophrenia.

Many studies discuss factors related to the decision-making capacity to consent to clinical research (DMC) of patients with schizophrenia. However, these studies rarely approached willingness to participate and the association between psychopharmacological properties (e.g., antipsychotic-induced side effects) and DMC. This study aimed to explore factors related to DMC and willingness to participate in patients with schizophrenia. All 139 patients with schizophrenia were assessed with the MacArthur Competence Assessment Tool for Clinical Research (MacCAT-CR) and other measures. A linear regress

Improving the primary care physicians' decision making for fibromyalgia in clinical practice: development and validation of the Fibromyalgia Detection (FibroDetect®) screening tool.

BACKGROUND: Fibromyalgia diagnosis is a challenging and long process, especially among primary care physicians (PCPs), because of symptom heterogeneity, co-morbidities and clinical overlap with other disorders. The purpose was to develop and validate a screening tool in French (FR), German (DE) and English (UK) to help PCPs identify patients with fibromyalgia. METHODS: The FibroDetect questionnaire was simultaneously developed in FR, DE and UK based on information obtained from a literature review, focus groups conducted with clinicians, and face-to-face interviews with fibromyalgia patients

The Medication Recommendation Tracking Form: a novel tool for tracking changes in prescribed medication, clinical decision making, and use in comparative effectiveness research.

This paper describes the development and use of the Medication Recommendation Tracking Form (MRTF), a novel method for capturing physician prescribing behavior and clinical decision making. The Bipolar Trials Network developed and implemented the MRTF in a comparative effectiveness study for bipolar disorder (LiTMUS). The MRTF was used to assess the frequency, types, and reasons for medication adjustments. Changes in treatment were operationalized by the metric Necessary Clinical Adjustments (NCA), defined as medication adjustments to reduce symptoms, optimize treatment response and functionin

Development of a clinical prediction model to inform clinical decision making for classification of patients with sciatica, based on their clinical characteristics, in the Greek health system: protocol for a prospective predictive exploratory study.

INTRODUCTION: Sciatica is one of the most common reasons for seeking healthcare for musculoskeletal pain. Sciatica is primarily considered as neuropathic in nature when neural tissue in the low back is compromised, but sometimes other non-neural structures may be involved. Appropriate assessment and management are important for patients with sciatica. Therapists use several outcome measures to assess patients to inform selection of the most suitable treatment. There is limited evidence for the best treatment of sciatica, and this is likely contributed to by having no reliable algorithm to cate

Pathogen spectrum of pulmonary infections in kidney transplant recipients and the diagnostic value of mNGS: a sputum and BALF study based on clinical decision-making.

BACKGROUND: Pulmonary infection is a common and severe post-transplant complication in kidney transplant recipients (KTRs). Their long-term immunosuppression results in an extremely complex pathogen spectrum. Compared with conventional etiological detection methods, metagenomic next-generation sequencing (mNGS) enables rapid and broad-spectrum pathogen identification. However, compared with bronchoalveolar lavage fluid (BALF), research on the diagnostic value of sputum - used as a non-invasive sample - for pulmonary infections in KTRs remains limited. METHODS: A retrospective study included 7

Towards an Integrative Framework of Self-Regulation: A Scoping Review on the Interplay Between Emotion Regulation, Executive Functions and Decision-Making in Clinical Populations.

BACKGROUND/OBJECTIVE: Emotion regulation (ER) is closely linked to decision-making (DM). Although executive functions (EF) are often suggested as a key mechanism underlying this relationship, evidence from different clinical conditions remains fragmented. This scoping review aims to map the literature on the relationship between ER and DM in clinical populations, with a specific focus on the interaction between EF and ER in shaping adaptive DM processes, particularly in populations characterised by cognitive impairment and emotional dysregulation. METHODS: A search of electronic databases was

The effect of youth-caregiver discrepancies in reports of internalizing and externalizing behavior in forensic clinical decision making about youth involved in the justice system.

Multi-informant assessments are considered the gold standard in the assessment of youth psychopathology. Although best practice dictates that this approach should extend to the assessment of youth involved in the justice system, few studies have examined the effect of multiple informants in a forensic context. Using a sample of 580 youth-caregiver dyads, we conducted a series of polynomial regressions to test whether statistical interactions between scores on the Youth Self-Report and scores on the Child Behavior Checklist (completed by caregivers) predicted psychiatric diagnoses.
