Teaching delirium to undergraduate medical students: Exploring the effects of a cross-professional group objective structured clinical examination compared to standard education.

OBJECTIVES: To compare a cross-professional facilitated delirium group objective structured clinical examination (GOSCE) educational intervention with standard delirium education for medical students during clinical placement, and explore the differences in the examiner's written feedback between the two groups. METHODS: A non-randomised clustered controlled designed study at a single metropolitan university across several campuses in Sydney, Australia. A convenience sample of third-year medical students was recruited. Students' knowledge, communication, and clinical reasoning skills were ass…

Clinical Ethics in Name Only? The Case for Moral Realism at the Bedside.

As Clinical Ethics Consultation (CEC) becomes more institutionalized, concerns about its legitimacy and efficacy have grown. We defend a realist orientation, according to which CEC has normative force only if its recommendations answer to stance-independent reasons, not to individual preferences and institutional policies. Public justification can and should proceed through familiar means (e.g., shared mid-level norms and transparent procedures) so that stakeholders can scrutinize the reasoning. We show how neutrality, admirable as a procedural virtue, hollows CEC when treated as a master idea

Beyond Single Systems: How Multi-Agent AI Is Reshaping Ethics in Radiology.

Radiology is undergoing a paradigm shift from traditional single-function AI systems to sophisticated multi-agent networks capable of autonomous reasoning, coordinated decision-making, and adaptive workflow management. These agentic AI systems move beyond simple pattern recognition to encompass complex radiological workflows including image analysis, report generation, clinical communication, and care coordination. While multi-agent radiological AI promises enhanced diagnostic accuracy, improved workflow efficiency, and reduced physician burden, it simultaneously amplifies the long-standing "b

"A Transformational redesign of the global clinical workforce decision-making processes: Novel cognitive schemes embracing modern healthcare paradigms".

BackgroundGlobal healthcare systems continue to face a persistent quality gap driven by fragmented and inconsistent clinical decision-making … sWe employed a conceptual development methodology to synthesize evidence from cognitive psychology, clinical reasoning research, CBME frameworks, and healthcare quality paradigms. Concepts were mapped to sequential milestones and refined into cognitive schemes.ResultsThe roadmap comprises seven milestones supported by structured decision aids in the form of cognitiv …

Generative AI and Large Language Models in Rehabilitation: A Scoping Review.

Generative artificial intelligence (AI) and large language models (LLMs) are increasingly evaluated in rehabilitation, yet their clinical validity, reproducibility, and safety remain uncertain. This scoping review mapped peer-reviewed studies of generative AI/LLMs across rehabilitation assessment, clinical reasoning, decision support, planning, education, and functional classification. Following JBI methodology and PRISMA-ScR, five databases were searched for English-language studies published from 1 January 2015 to 24 July 2026. Two reviewers independently conducted study selection, data extr…

"Nursing home staff's reasoning about providing palliative care following an educational intervention - a qualitative interview study".

BACKGROUND: Palliative care (PC) is increasingly recognised as an essential component of healthcare, yet access remains unequal, particularly for older people. Nursing homes accommodate older people with complex needs and account for a large proportion of deaths, making PC a central component of care in these settings and underscoring the importance of integrating a PC approach into everyday care. However, staff often lack sufficient training and may perceive PC as limited to end-of-life care rather than as an ongoing approach. Education has been proposed to strengthen PC competence, but littl

Discomfort Avoidance and Desire for Normality Are Decision-Making Drivers of Footwear Choice in Underserved Communities at Risk of Diabetic Foot Ulcer.

BACKGROUND: Diabetes-related foot ulceration (DFU) is a serious complication that can lead to lower limb amputation or death. Custom-made footwear is a key preventative strategy, yet adherence to use is often low. The factors influencing decision-making for footwear use remain unclear, particularly among underserved groups such as people from low socioeconomic and South Asian backgrounds. This study aimed to identify the psychological mechanisms underlying footwear choice and adherence in people with diabetes from low socioeconomic and South Asian populations. METHODS: This study used a quali

Pitfalls of reasoning in hospital-based transfusion medicine.

INTRODUCTION: Hospital-based transfusion involves hundreds of daily medical decisions. Medical decision-making under uncertainty is susceptible to cognitive biases which can lead to systematic errors of reasoning and suboptimal patient care. Here we review common cognitive biases that may be relevant for transfusion practice. MATERIALS AND METHODS: Biases were selected based on categorical diversity, evidence from healthcare contexts, and relevance for transfusion medicine. For each bias, we provide background psychology literature, representative clinical examples, considerations for transfu

Effectiveness of a Specialty-Focused, Outpatient-Based Case Teaching Model in Residency Training for Metabolic Dysfunction-Associated Steatotic Liver Disease.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) has become one of the most prevalent chronic liver diseases worldwide and requires comprehensive clinical management involving risk stratification, longitudinal assessment, and integrated decision-making. Developing residents' clinical reasoning and stratified management competence remains a challenge in traditional residency training, which often relies on fragmented case exposure. This study aimed to evaluate the effectiveness of a specialty-focused, outpatient-based case teaching model embedded in a MASLD clinic fo…

Dutch rehabilitation physicians' perspectives on contracture management in children with spinal muscular atrophy: challenges in a changing landscape.

BACKGROUND: Many children with hereditary proximal spinal muscular atrophy (SMA) develop joint contractures. With the introduction of disease-modifying treatments (DMTs) for SMA, the improved functional prognosis may change the focus of (preventive) contracture management. OBJECTIVE: This study aimed to describe current approaches to contracture management among Dutch pediatric rehabilitation physicians caring for children with SMA receiving DMT, and to explore the underlying considerations and clinical reasoning that inform their decisions on contracture management in the evolving therapeuti…

Advancing neuro-ophthalmic diagnostics: a multimodal imaging approach integrating OCT angiography and AI-enhanced MRI for improved visual pathway analysis.

INTRODUCTION: Recent advancements in neuro-ophthalmology necessitate integrative imaging methodologies to address the structural and functional complexities of the visual pathway. Conventional diagnostic tools, including magnetic resonance imaging (MRI) and optical coherence tomography (OCT), are constrained by limitations in spatial resolution, cross-modality integration, and interpretability, often resulting in diagnostic uncertainty in cases involving compressive neuropathies, demyelinating diseases, or unexplained visual field deficits. Deep learning approaches, despite their computational

Generating guideline-concordant and safe recommendations for diabetic kidney disease management via a hierarchical retrieval-augmented large language model.

Managing diabetic kidney disease (DKD) is inherently complex, requiring clinicians to synthesize patient history, fluctuating biomarkers, and evolving treatment guidelines. While large language models (LLMs) show promise in medical decision support, their clinical adoption is hindered by factual inaccuracies and a lack of specific reasoning required for individualized patient management. To address this, we developed a hierarchical multi-agent system that integrates a locally deployed retrieval-augmented generation (RAG) framework with a cloud-based advanced reasoning engine, grounding respons
