Classics in Spine. Surgery literature revisited.

STUDY DESIGN: This article reviews the criteria for evaluating the quality of clinical trials. OBJECTIVES: To outline the current methodologic standards by which the validity of controlled trials need to be evaluated. SUMMARY OF BACKGROUND DATA: Weber's study, published in 1983 in Spine, is the only randomized trial comparing surgery and conservative management of sciatica in herniated lumbar discs. METHODS: Weber's article is revisited to illustrate basic principles in the design of clinical trials. RESULTS: Weber's study is a classic in spine surgery and has changed thinking regarding th

Individual therapy-dependent prognosis based on data from controlled clinical trials in chronic liver disease.

In a given disease the manifestations and course of disease may vary markedly between the patients. This complicates prediction of the prognosis and treatment effect in individual patients. Most controlled clinical trials present only the "average" effect e.g. the therapy-dependent survival in the studied patient group. To estimate the therapy-dependent prognosis in individual patients it is necessary to utilize the covariation between survival time and variables characterizing each patient including the therapy given. This paper describes current methods for identification of variables which

Topical treatment for moccasin-type tinea pedis.

Sixty patients were treated for tinea pedis with 2% topical ketoconazole cream either once or twice daily. All diagnoses were confirmed mycologically. They were randomly assigned in a double-blind study into three groups on the basis of their clinical lesions, that is, vesicular pedis, interdigital pedis, and hyperkeratotic "moccasin" pedis. Patients were treated for a period of 1 month and were evaluated weekly. A 4-week posttreatment evaluation permitted an assessment of recurrence or improvement. The follow-up visits showed 93% of the vesicular patients, 86% of the interdigital patients, an

Treatment of Bell's Palsy. An analysis of the available studies.

In this paper the basic requirements for a clinical therapeutic trial in Bell's palsy are discussed. The available therapeutic studies are analysed in view of these requirements. The following conclusions seem justified: (1) Galvanic stimulation has not been proved to lead to a better recovery. (2) There is no evidence that decompression of the mastoid part of the facial nerve in the second or third week has a positive effect. Whether surgery at an earlier stage or decompression of the complete intratemporal part of the nerve is effective, has not yet been studied. (3) There is no convincing p

Protocol of a prospective, monocentric phase I/II feasibility study investigating the safety of multimodality treatment with a combination of intraoperative chemotherapy and surgical resection in locally confined or borderline resectable pancreatic cancer: the combiCaRe study.

INTRODUCTION: Pancreatic cancer is a devastating disease with an exceptionally poor prognosis. Complete resection of the primary tumour followed by adjuvant chemotherapy is the current standard treatment for patients with resectable disease and the only curative treatment option. However, long-term survival remains rare. Tumour cell dissemination due to manipulation during surgery may increase the rate of future metastases and local recurrence, and perioperative chemotherapy might diminish local, distant and circulating minimal residual disease. Yet, safety and feasibility of systemic chemothe

Rugby and Shoulder Trauma: A Systematic Review.

Rugby is a popular contact sport worldwide. Collisions and tackles during matches and practices often lead to traumatic injuries of the shoulder. This review reports on the epidemiology of injuries, type of lesions and treatment of shoulder injuries, risk factors, such as player position, and return to sport activities. Electronic searches through PubMed (Medline), EMBASE, and Cochrane Library retrieved studies concerning shoulder injuries in rugby players. Data regarding incidence, type and mechanisms of lesion, risk factors and return to sport were extracted and analyzed. The main reported d

Obstructive sleep apnoea and glaucoma: a systematic review and meta-analysis.

INTRODUCTION: Obstructive sleep apnoea (OSA) has been thought to be associated with glaucoma, however there are many conflicting studies on this topic. With many new studies having been published since the previous meta-analysis, we believe it is important to clarify this association. Hence, in this study we meta-analyse the recent literature regarding the association between OSA and glaucoma. METHODS: Pubmed, Embase, Scopus and Cochrane Library were searched from inception till the 28th February 2022 for observational as well as cross-sectional studies examining the association between OSA a

How phytochemicals influence reproductive outcomes in women receiving assisted reproductive techniques: a systematic review.

CONTEXT: Over the past few years, there has been an increasing amount of scholarly literature suggesting a connection between the nutritional status of pregnant mothers and early fetal development, as well as the long-term health consequences of their offspring. Multiple studies have documented that alterations in dietary patterns prior to conception have the potential to affect the initial stages of embryonic development. OBJECTIVES: The aim of this study was to perform a comprehensive review of the research pertaining to the correlation between phytochemicals ( specifically, polyphenols, ca

AI-Driven Clinical Decision Support to Reduce Hospital-Acquired Venous Thromboembolism: A Trial Protocol.

IMPORTANCE: Hospital-acquired venous thromboembolism (HA-VTE) remains a leading cause of preventable death among hospitalized adults in the US. Despite numerous attempts to prognosticate HA-VTE with risk models, no single model has outperformed the rest, and the effectiveness of such models to drive prophylaxis decisions is unknown. Testing such systems in urban and rural settings may inform their generalizability. OBJECTIVE: To conduct a randomized clinical trial to assess the effectiveness of artificial intelligence (AI)-driven clinical decision support (CDS) in reducing HA-VTE incidence in

Clinical study of cinacalcet in Japan.

For the approval of cinacalcet hydrochloride, five clinical trials have been performed excluding pharmacokinetic studies for healthy volunteers and hemodialysis patients. An early phase II dose-finding study was started for regular hemodialysis patients with secondary hyperparathyroidism from a daily dose of 12.5 mg cinacalcet and increasing 25 mg up to 50 mg for 9 weeks. A placebo-controlled double-blind three dose-finding study (late phase II study) was then conducted. By this trial, an adequate initial dose of cinacalcet was determined as 25 mg daily, and the placebo-controlled double-blind

[Translated article] Profile of clinical trials with drugs for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in Spain.

BACKGROUND: On 11 March 2020, World Health Organisation declared COVID-19 a pandemic. During the early stages, treatments lacking scientific evidence, such as hydroxychloroquine and later remdesivir, were used; meanwhile, clinical trials were being conducted in Spain to evaluate new therapies with greater scientific rigor. This article analyses the study profile, publication rate and completion of studies during the COVID-19 pandemic in Spain. METHODS: Meta-epidemiological, analytical, and retrospective study analyzing the characteristics and rate of completion of clinical trials with SARS-Co

Radiation-before-pathology approach in the palliative oncology setting: a pragmatic clinical trial protocol (RT-NOW).

BACKGROUND: Patients with incurable but not-yet-biopsied cancers sometimes require urgent palliative radiation. However, wait-times for biopsy procedures and pathologic results can delay treatment, with significant consequences to patient quality of life and/or the chance of irreversible cancer complications. There is no prospective data to guide empirical decision-making in these urgent, palliative contexts. METHODS: In this prospective single-arm pragmatic clinical trial, we will enrol 48 patients with incurable cancer where a biopsy is delaying urgent palliative radiation. Patients will re
