Associations among MRI-derived glymphatic system impairment, gray matter reserve and white matter integrity in cerebral small-vessel disease patients.

The aim of this study was to analyze glymphatic system function and its relationship with white/gray matter (WM/GM) abnormalities and structural network efficiency in patients with cerebral small vessel disease (CSVD) by structural magnetic resonance imaging (MRI) scanning and diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index. We retrospectively enrolled 42 clinically confirmed patients with CSVD, including 22 with normal cognition (CSVD_CN), 20 with mild cognitive impairment (CSVD_CI), and 20 matched healthy control (HC) volunteers. All the subjects underwent 3.0

Interregional compensatory mechanisms of motor functioning in progressing preclinical neurodegeneration.

Understanding brain reserve in preclinical stages of neurodegenerative disorders allows determination of which brain regions contribute to normal functioning despite accelerated neuronal loss. Besides the recruitment of additional regions, a reorganisation and shift of relevance between normally engaged regions are a suggested key mechanism. Thus, network analysis methods seem critical for investigation of changes in directed causal interactions between such candidate brain regions. To identify core compensatory regions, fifteen preclinical patients carrying the genetic mutation leading to Hun

Pharmacologic Management of Insomnia in Adults.

Insomnia is a common reason for patients to seek medical care. A careful history should be obtained to identify predisposing, precipitating, and perpetuating factors contributing to sleep disturbance. Cognitive behavioral therapy for insomnia is the first-line treatment for chronic insomnia, offering effectiveness with minimal risk, though access may be limited by availability. Pharmacologic therapy should ideally be reserved for short-term use and initiated only after setting clear expectations and considering potential risks. Evidence-based medication options include low-dose doxepin, melato

Cumulative family risk and problematic internet use among adolescents: The mediating roles of basic psychological need satisfaction and maladaptive cognition.

OBJECTIVE: Family risk factors frequently cluster together and play important roles in adolescent problematic internet use (PIU). However, the longitudinal association between cumulative family risk and PIU among adolescents and its potential mediation mechanisms are largely unexplored. Grounded in motivational theories, this study aims to examine the longitudinal association between cumulative family risk and adolescent PIU 2 years later and to determine whether this association is mediated by internal need (i.e., basic psychological need satisfaction) and external incentive (i.e., maladaptiv

What was helpful in today's session? Responses of clients in two different psychotherapies for posttraumatic stress disorder.

The aim of this study was to qualitatively analyze and compare the written responses clients in treatment for posttraumatic stress disorder (PTSD) gave when asked about helpful and negative aspects of the previous session. Data were collected during a randomized quantitative study comparing dialogical exposure therapy (DET, a gestalt-based integrative therapy, n = 58) and cognitive processing therapy (CPT, a cognitive-behavioral therapy, n = 52). We developed a coding manual using Mayring's (2015) qualitative content analysis approach and were able to code responses reliably. Five main categor

Employment intentions of vocational nursing students toward geriatric care in central China.

BACKGROUND: Global population aging has raised worldwide demand for geriatric care practitioners, including in China. Vocational nursing students are key reserve workforce for geriatric care, yet few enter this sector. Existing studies have explored individual factors shaping geriatric care employment intention, but few examine interactive effects of family support, experiential exposure and personal cognitive factors, or extend Social Cognitive Career Theory (SCCT) to Chinese vocational nursing education. PURPOSE: This study explores how family support, practical care experience and personal

Sex difference in brain reserve of working memory among older adults.

Working memory is one of the core cognitive functions that declines in older adulthood. Brain reserve (BR) refers to neurobiological status of the brain that supports cognition, offering a valuable neurostructural framework for understanding working memory decline among older adults. However, most existing studies rely on gross, whole-brain measures of BR which may lack sensitivity to specific cognitive functions, and few studies have examined their sex differences. Based on 131 healthy older adults (aged 52-86) from Australia, this study used multimodal imaging data, including grey matter vol

Management of brain metastases in older adults with non-small cell lung cancer.

Brain metastases (BrM) are a major cause of morbidity in patients with non-small-cell lung cancer (NSCLC). As global populations age and systemic therapies improve survival, the number of older individuals presenting with BrM continues to rise. However, older adults remain underrepresented in clinical trials. Thus, treatment decisions in this population are often based on limited evidence from randomized prospective studies. Beyond chronological age, older adults with cancer frequently present with multiple comorbidities, frailty, baseline cognitive impairment, and reduced physiologic reserve.

Impaired neuromodulator crosstalk delays arousal-dependent astroglia Ca2+ activation in mouse models of Alzheimer's disease.

Cortical astrocyte Ca2+ activation is associated with control of neuronal network activity and is altered in mouse models of Alzheimer's disease (AD). Applying enforced running, a well-controlled behavioral paradigm triggering arousal-mediated widespread Ca2+ activation in astroglia throughout the brain, to two mouse models of AD (APPswe/PSEN1dE9 and App NL-F KI), we consistently observed delayed and less coordinated astrocyte Ca2+ elevations. Combining pharmacological and genetic manipulations, we found that direct noradrenergic signaling to astrocytes was facilitated by cholinergic signaling

Examining Sex Differences in Markers of Cognition and Neurodegeneration in Autosomal Dominant Alzheimer's Disease: Preliminary Findings from the Colombian Alzheimer's Prevention Initiative Biomarker Study.

BACKGROUND: Growing evidence suggests that there may be a sex-specific biological risk for Alzheimer's disease (AD). Individuals with autosomal dominant AD due to a mutation (E280A) in Presenilin-1 (PSEN1) are genetically determined to develop early-onset dementia and thus, have few age-related risk factors for AD that are known to vary by sex (i.e., cardiovascular disease, menopause, life expectancy). OBJECTIVE: Investigate sex differences in markers of cognition and neurodegeneration in autosomal dominant AD. METHODS: We conducted a retrospective study in 19 cognitively-unimpaired PSEN1 mu

The relationship between psychological stress and ovulatory disorders and its molecular mechanisms: a narrative review.

This narrative review explores the relationship between psychological stress and ovulatory disorders, focusing on the molecular mechanisms involved. Ovulation is regulated by the hypothalamus-pituitary-ovarian (HPO) axis, and disruptions in this axis can lead to ovulatory dysfunction. Chronic psychological stress affects the HPO axis, resulting in abnormalities in hypothalamus hormone secretion, pituitary hormone release, and ovarian function. These disruptions cause ovulation disorders and menstrual irregularities. The mechanisms by which psychological stress affects ovulation involve alterat

The impact of bilingualism on brain structure and function in Huntington's disease.

INTRODUCTION: Bilingualism exerts neuroprotective effects against neurodegeneration. In Huntington's disease (HD), the systems involved in bilingual control show early compromise, but the effect of bilingualism on the course of HD is unknown. METHODS: We addressed the impact of livelong use of bilingualism on the clinical features, brain structure and function in 30 early-mild stage HD patients. Using voxel-wise regression analysis, we explored the effect of levels of use of bilingualism on grey-matter volume (GMV) and 18F-FDG metabolism. RESULTS: Higher use of bilingualism was associated wi

From Cognitive Impairment to Uremic Encephalopathy in Chronic Kidney Disease: A Proposed Pathophysiological Continuum.

Chronic kidney disease (CKD) is associated with neurological abnormalities ranging from subtle cognitive deficits to acute or subacute uremic encephalopathy. Although clinically distinct, these syndromes may arise from overlapping biological processes, including retained uremic solutes, cerebrovascular and blood-brain barrier dysfunction, systemic and neuroinflammation, oxidative stress, mitochondrial impairment, and altered neurotransmission. This narrative review synthesizes clinical, experimental, and cellular evidence supporting the contribution of these interconnected pathways to CKD-asso
