Transverse colon for continent urinary diversion: experimental study comparing two techniques.

OBJECTIVES: Patients who have undergone pelvic radiotherapy can develop urinary complications such as actinic cystitis or fistulas and ultimately need urinary diversion, for which several techniques have been described. The purpose of this experimental study was to compare 2 different types of continent urinary diversion with the colonic pouch. METHODS: Sixteen dogs of both sexes, weighing 20-25 kg were divided into 3 groups: the pilot group consisted of 6 dogs used to standardize the technique. Group 1 consisted of 5 dogs that underwent the Monti technique to construct the conduit and the Ab

Risk factors for fistulizing disease of the pouch after ileal pouch-Anal anastomosis for ulcerative colitis.

PURPOSE: Long-term data on the risk factors for fistulizing disease (FD) after ileal pouch-anal anastomosis (IPAA) for ulcerative colitis are limited. This study aimed to identify the predictors of FD after ileostomy closure and evaluate the impact of FD on pouch failure. METHODS: We reviewed 374 patients who underwent mucosal restorative proctocolectomy with handsewn IPAA between 2004 and 2022. Cox proportional hazards regression and log-rank tests were used to assess the FD-free survival and risk factors. RESULTS: After excluding 59 patients, 315 patients were analyzed. FD developed in 20

LONG-TERM QUALITY OF LIFE AFTER ILEAL POUCH-ANAL ANASTOMOSIS FOR ULCERATIVE COLITIS IN A BRAZILIAN IBD REFERRAL CENTER.

BACKGROUND: Reports on the long-term impact on quality of life (QoL) after ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC) are scarce in Latin America. OBJECTIVE: To evaluate QoL after IPAA in an inflammatory bowel disease (IBD) referral center in Brazil. METHODS: From March 2023 to August 2023, a standardized questionnaire was administered to patients with UC who had previously undergone IPAA surgery. The questionnaire addressed: (1) daily bowel movement frequency, (2) daytime and nighttime fecal incontinence, (3) need for daily medication such as loperamide, (4) fecal urgen

Textbook Outcome After IPAA for Ulcerative Colitis: A Nationwide Multicenter Study and Its Implications for Long-term Quality of Life.

BACKGROUND: Textbook outcome is a composite quality measure increasingly used in oncologic surgery but has not been applied to ulcerative colitis. Restorative proctocolectomy with IPAA is the standard surgical treatment for ulcerative colitis, yet a comprehensive assessment of surgical quality remains lacking. Pouchitis, the most common long-term complication after IPAA, significantly impairs quality of life and may lead to pouch failure. Linking perioperative success to long-term pouch function is therefore clinically relevant. OBJECTIVE: This study aimed to evaluate the feasibility and clin

Assessing the Value of Histology and Anatomic Segment Evaluation Among Patients Undergoing Pouchoscopy.

BACKGROUND: The value of histologic assessment after ileal pouch-anal anastomosis (IPAA) has not been definitively determined. We evaluated the correlation between histology and endoscopic findings, as well as the proportion of patients with inflammation in areas beyond the pouch body on their initial pouchoscopy after IPAA. METHODS: In a retrospective cohort study, we evaluated patients who underwent IPAA for UC between 2012 and 2020 and subsequently underwent a pouchoscopy with routine biopsies of the pouch body, pre-pouch ileum, and rectal cuff. We compared endoscopic and histologic assess

Types, behaviour and therapeutic requirements of inflammatory pouch disorders: Results from the RESERVO study of GETECCU.

BACKGROUND AND AIMS: Inflammatory pouch disorders exhibit a heterogeneous clinical spectrum and therapeutic requirements have not been properly studied. METHODS: This retrospective, multicentre study included ulcerative colitis patients with ileal pouch construction and were later diagnosed with an inflammatory pouch disorder between 1995 and 2020. Classifications, behaviour and therapies applied were recorded and compared in the long-term. RESULTS: Overall, 338 patients were recruited. The most common disorders were pouchitis (n = 258, 76%), Crohn's disease of the pouch (n = 55, 16%) and cu

Results at Up to 30 Years After Ileal Pouch-Anal Anastomosis for Chronic Ulcerative Colitis.

BACKGROUND: Ileal pouch-anal anastomosis (IPAA) has become the surgical procedure of choice for patients with chronic ulcerative colitis. No study to date has examined functional and quality-of-life outcomes 30 years after pouch construction. METHODS: Using data from a prospectively maintained database with annually distributed questionnaires, functional outcomes, pouch complications, and quality of life after IPAA were determined. RESULTS: Overall, 93.3% of patients had a functioning pouch at 30 years. Stool frequency during the day increased slightly from a mean of 5.7 (SD, 2.3) at 1 year

Outcomes after ileal pouch anal anastomosis in patients with primary sclerosing cholangitis.

BACKGROUND AND AIMS: Outcomes after ileal pouch anal anastomosis (IPAA) are not well established in patients with primary sclerosing cholangitis (PSC). We conducted a comprehensive outcomes assessment in these patients. METHODS: A retrospective case note review of complications in all PSC-IPAA (n=21) and matched ulcerative colitis patients with IPAA (UC-IPAA; n=79) after surgery in Oxford (1983-2012) was conducted, and functional outcomes (Öresland score) were evaluated (2012). Quality of life [Cleveland Global Quality of Life Questionnaire, Short Form-36 (SF-36)], and sexual function were al

Relapse of Crohn's disease following ileal pouch-anal anastomosis: MDCT and MR features.

OBJECTIVE: The purpose of our study is to use MDCT and MR imaging to describe the normal ileal pouch and to identify features of Crohn's disease (CD) relapse in patients after ileal pouch-anal anastomosis (IPAA). CONCLUSIONS: After total colectomy followed by IPAA, features, optimally evaluated with pelvic MRI, such as fistulas, abscesses, pouch inflammation, and stenoses, indicate CD relapse. Although uncommon, radiologists should be aware that these imaging features strongly favor this diagnosis.

Is omitting pouchography before ileostomy takedown safe after negative clinical examination in asymptomatic patients with pelvic ileal pouch? An observational study.

BACKGROUND: When restorative proctocolectomy (RPC) is performed, a temporary diverting loop ileostomy is often fashioned and usually closed 2-3 months later. Pouchography is used to assess pouch integrity, although its benefits have been questioned and no definitive data support its routine use. Our aim was to assess the utility of pouchography before ileostomy closure in patients with a negative clinical examination. METHODS: We retrospectively reviewed our database of patients who underwent ileostomy takedown between 1987 and 2010. Two hundred and thirty-two patients were identified who und

The clinical impact of primary sclerosing cholangitis in patients with an ileal pouch-anal anastomosis for ulcerative colitis.

INTRODUCTION: Primary sclerosing cholangitis (PSC), a chronic inflammatory disease affecting the liver, is associated with ulcerative colitis (UC) in up to 75% of Northern European patients. These patients are at increased risk for the development of colorectal cancer, and the operation of choice is restorative proctocolectomy with an ileal pouch-anal anastomosis. However, complications such as pouchitis can occur, and studies have suggested that PSC is an independent risk factor for the development of pouchitis. AIM: The aim of this study is to review and discuss the available literature on

Review article: restorative proctocolectomy, indications, management of complications and follow-up--a guide for gastroenterologists.

BACKGROUND: Restorative proctocolectomy with ileal pouch-anal anastomosis is the procedure of choice for the majority of patients with ulcerative colitis who require surgery. Over 2500 patients in the UK have undergone restorative proctocolectomy. It is now increasingly being performed in district general hospitals as well as in specialist inflammatory bowel disease units. Gastroenterologists are increasingly involved in the management of patients following restorative proctocolectomy. AIM: To provide gastroenterologists with a clear understanding of the investigation and evidence-based manag

Short-pouch and low-anastomosis Duhamel procedure results in better fecal control and normal defecation pattern.

PURPOSE: The purpose of this study was to analyze the long-term outcome of the short-pouch and low-anastomosis Duhamel procedure and to evaluate the quality of life after pull-through. We also tried to answer the questions: Does the modified Duhamel procedure produce fecal continence? Is the quality of life correlated to normal bowel function (1-3 per day) without the use of laxatives? MATERIALS AND METHODS: Between January 1993 and January 2002, 53 patients, 44 (83 percent) males and 9 (17 percent) females, who underwent a Duhamel procedure were assessed retrospectively. Of the 53 children,
