A temporal analysis of opioid use, patient satisfaction, and pain scores in colorectal surgery patients.

BACKGROUND: Recent health care policy changes promote objective measurements of patient satisfaction with care provided during hospitalization. Acute postsurgical pain is a significant medical problem and strongly impacts patient experience and patient satisfaction. Multimodal analgesic pathways are used for acute pain management, but opioid medications remain a mainstay of treatment. Opioid use is increasing in the outpatient setting, but opioid use trends in the inpatient postsurgical setting are not well known. We hypothesized that use of opioid medications has increased over time along wit

The impact of mechanical bowel preparation in elective colorectal surgery: a propensity score matching analysis.

PURPOSE: To evaluate the influence of preoperative mechanical bowel preparation (MBP) based on the occurrence of anastomosis leakage, surgical site infection (SSI), and severity of surgical complication when performing elective colorectal surgery. MATERIALS AND METHODS: MBP and non-MBP patients were matched using propensity score. The outcomes were evaluated according to tumor location such as right- (n=84) and left-sided colon (n=50) and rectum (n=100). In the non-MBP group, patients with right-sided colon cancer did not receive any preparation, and patients with both left-sided colon and re…

No-scar transanal total mesorectal excision: the last step to pure NOTES for colorectal surgery.

HYPOTHESIS: Because of the concerns over the operative platform, accidental organ injury, and viscerotomy closure, natural orifice transluminal endoscopic surgery (NOTES) currently remains an experimental technique. Transanal NOTES for colorectal surgery overcomes all of these issues; however, all of the reports to date have used hybrid laparoscopic techniques. We demonstrate herein the first case, to our knowledge, of pure transanal NOTES colorectal surgery. DESIGN: Case report. SETTING: University hospital. PATIENT: The patient was a 56-year-old woman with a midrectal neoplasia. INTERVEN…

Evaluation of inflammatory markers as predictors of hospital stay and unplanned readmission after colorectal surgery.

UNLABELLED: Hospital length of stay (LOS) and readmissions continue to be expensive and unexpected events following colorectal surgery (CRS) whether patients follow enhanced recovery pathways or traditional care. Predictors of these adverse events could facilitate identification and optimization of CRS patients. THE AIM OF THE STUDY: To examine the impact of white blood cell count (WBC) and C-reactive protein (CRP) levels as predictors of delayed recovery or hospital readmission following CRS. MATERIAL AND METHODS: Patients undergoing laparoscopic or open abdominal colorectal surgery by a si…

Self-expanding metallic stents for palliation and as a bridge to minimally invasive surgery in colorectal obstruction.

BACKGROUND AND OBJECTIVES: Acute colorectal obstruction is a potentially life-threatening emergency that requires immediate surgical treatment. Emergency procedures had an associated mortality rate of 10% to 30%. This encouraged development of other options, most notably self-expanding metallic stents. The primary endpoint of this study to is to report our group's experience. METHODS: We performed a retrospective review of 37 patients who underwent self-expanding metallic stent placement for colorectal obstruction between July 2000 and May 2012. Data collected were age, comorbidities, diagnos

[Liver and pulmonary metastases of the colorectal carcinoma - the experience of the Department of Surgery, University Hospital in Pilsen].

INTRODUCTION: Liver and pulmonary metastases of the colorectal carcinoma develop in about 20-70%, resp. in 10-22% patients with the colorectal carcinoma. Their treatment is still a matter of discussion. The aim of our study was to evaluate the results of surgical treatment as a part of the multimodal approach. MATERIAL AND METHODS: 30 patients with liver and pulmonary colorectal metastases were operated on between 2002 and 4/2013. The average age was 62.8 ± 10.7 years. 19 patients had metachronous and 11 synchronous metastases. The liver metastases preceded pulmonary in 14 cases, and vice ver

[Interdisciplinary cooperation as a prerequisite for precise tumor documentation. Illustrated by an example of surgery for colorectal carcinoma].

Current tumor management is increasingly founded on interdisciplinary cooperation. The main partners in cases of solid tumors are oncologic surgery, medical oncology, and radiotherapy, guided by pathology. The cooperative concept, particularly the individual strategy and selection of the most adequate approach, oriented on guidelines and therapeutic standards, depends on the quality of the involved components as well as personal abilities of the "actors." In addition to the personal qualification, decision making depends on both tumor stage and completeness of tumor removal. In this point, the

Trends in preparation for colorectal surgery: survey of the members of the American Society of Colon and Rectal Surgeons.

The utility of antibiotic and mechanical preparation for colorectal surgery is controversial, and numerous different regimens are used. The aim of this study was to detect trends in preparation for surgery among American colon and rectal surgeons. Members of the American Society of Colon and Rectal Surgeons practicing in the United States were surveyed with a postal questionnaire regarding their routine preparations for colon and rectal surgery. Five hundred fifteen (40%) of the 1295 questionnaires sent were returned. Eighty-one per cent of the respondents had completed an accredited colorecta…

Prospective comparative study of cefotetan with piperacillin for prophylaxis against infection in elective colorectal surgery.

This study compared one dose of cefotetan with three doses of piperacillin as prophylaxis against wound infection in 153 patients undergoing elective colorectal surgery. The patients were randomized into two groups: the first received 2 g cefotetan intravenously with induction of anaesthesia (n = 75), and the second received three doses of 2 g piperacillin (n = 78). Wound infection was defined as the presence of an abscess or discharging pus from the wound. In the cefotetan group there were 14 (19%) wound infections and 13 (17%) in the piperacillin group. There were three septic deaths, one in…

Epithelial cell kinetics in the remaining colorectal mucosa after surgery for cancer of the large bowel.

We used microautoradiography in order to evaluate cell replication of the remaining colorectal mucosa in 20 patients previously operated on for cancer of the large bowel. The results were compared to those of 24 controls without neoplasms or other relevant colorectal disease. Samples of colorectal mucosa were taken during endoscopy. At histological examination each labeled intestinal hemicrypt was divided into 5 longitudinal compartments, from the base to the surface, and S-phase cells in each compartment were counted. Total labeling index (LI, ratio of labeled to total cells x 100) and labeli

Oral prophylaxis with neomycin and erythromycin in colorectal surgery. More proof for efficacy than failure.

In an open, prospective, and randomized investigation on the prophylactic efficacy of peroral neomycin sulfate-erythromycin base vs intravenous ceftriaxone-metronidazole preparation in colorectal surgery, no significantly diverging results between regimens were recorded (1/27 [3.7%] and 2/27 [7.4%] wound infections, respectively). Commentary is made about the diverging results from earlier studies on antimicrobial prophylaxis and on the multifactorial causality of surgical infection. We believe that variables such as physical condition of the patients, virulence and local resistance patterns o…

A comparison of three different prophylactic parenteral antibiotic regimens in colorectal surgery: a prospective study.

A double-blind, prospective, randomized study was performed with 90 patients undergoing colorectal surgery, to ascertain the prophylactic effect of three different parenteral antibiotic programs. The patients were divided into three groups. The patients in group 1 received 80 mg Gentamycin and 600 mg Lincomycin intramuscularly, two hours preoperatively, and every eight hours postoperatively for three days. In group 2, the treatment was similar to group 1 but the Lincomycin was replaced by 600 mg Clindamycin. In Group 3, the treatment was also similar to group 1, but 500 mg Metronidazole given …

Comparison between oral and systemic antibiotics and their combined use for the prevention of complications in colorectal surgery.

Ninety patients were included in this prospective randomized trial. Each required electric colorectal surgery and was prepared for operation with oral preoperative antibiotic therapy, systemic peroperative therapy, or by a combination of both. The number of each type of septic postoperative complication and their total did not differ between the group treated by oral antibiotics prior to operation and the group treated peroperatively with systemic antibiotic therapy. The total number of septic complications (wall abscesses, fistulas, subdiaphragmatic abscesses, septicemia, peritonitis), howeve…

A systematic literature review of colorectal robotic surgery with the Hugo robotic-assisted surgery (RAS) system: feasibility, safety, and clinical applications.

PURPOSE: The Hugo robotic-assisted surgery (RAS) system is a novel modular robotic platform that has recently been introduced in colorectal surgery. Although robotic approaches have improved surgical ergonomics and precision, evidence specific to the Hugo RAS system remains limited. This systematic review synthesizes the current literature on colorectal procedures performed with the Hugo RAS system and evaluates feasibility, safety, and early oncologic outcomes. METHODS: A systematic literature search of PubMed, Scopus, Embase, and Ovid was conducted through September 2025 in accordance with …

Robotic-assisted versus laparoscopic surgery for colorectal resection in oncologic surgery: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Robotic-assisted surgery (RAS) is increasingly used for colorectal cancer (CRC), but its clinical and oncologic advantages over conventional laparoscopy (LS) remain uncertain. Prior meta-analyses (e.g., Huang et al., Front Oncol 13;1273378, 2023; Zou et al., BMC Surg 25;86, 2025) have included overlapping RCTs but vary in methodology, scope, and analytical transparency. This review aims to provide an updated, independently re-analyzed synthesis of RCTs published from 2015 to 2025, with full PRISMA compliance, explicit analytic reproducibility, and expanded evaluation of bias and ev

Influence of the Enhanced Recovery After Surgery Protocol on Postoperative Inflammation and Short-term Postoperative Surgical Outcomes After Colorectal Cancer Surgery.

PURPOSE: Many studies have shown that the enhanced recovery after surgery (ERAS) protocols improve postoperative surgical outcomes. The purpose of this study was to observe the effects on postoperative inflammatory markers and to explore the effects of a high degree of compliance and the use of epidural anesthesia on inflammation and surgical outcomes. METHODS: Four hundred patients underwent colorectal cancer surgery at 2 hospitals during 2 different periods, namely, from January 2006 to December 2009 and from January 2017 to July 2017. Identifying Patients with Colorectal Cancer Likely to Benefit from a Trimodal Prehabilitation Prior to Surgery.

Introduction: While enhanced recovery after surgery (ERAS) programs are widely implemented to reduce postoperative complications of colorectal cancer surgery, evidence for trimodal prehabilitation is inconsistent. We compared prehabilitation+ERAS versus ERAS alone, and explored patient subgroups most likely to benefit from targeted prehabilitation. Methods: A prospective, single-center parallel cohort study was conducted from October 2017 to August 2022. Consecutive adults undergoing elective colorectal surgery received ERAS alone or a 3-6-week trimodal prehabilitation programme (nutritional o…
