Health Management Planning for Landscape Fire Smoke Events: A Literature Review and Suggested Human-Centred Framework for Resource Development.

Poor air quality arising from landscape fire smoke (LFS) events presents a substantial health burden to at-risk individuals and communities. However, there are few dedicated health management resources available for priority populations, despite increasing climate-related landscape fire risks. We conducted a literature review to analyse health management plans and strategies available for at-risk individuals in Australia, Canada and the United States. Our review identified four resources, of which only two were dedicated LFS health action plans, revealing a critical gap in preparedness. Given

Measuring Contraceptive Autonomy at Two Sites in Burkina Faso: A First Attempt to Measure a Novel Family Planning Indicator.

There is growing consensus in the family planning community around the need for novel measures of autonomy. Existing literature highlights the tension between efforts to pursue contraceptive targets and maximize uptake on the one hand, and efforts to promote quality, person-centeredness, and contraceptive autonomy on the other hand. Here, we pilot a novel measure of contraceptive autonomy, measuring it at two Health and Demographic Surveillance System sites in Burkina Faso. We conducted a population-based survey with 3,929 women of reproductive age, testing an array of new survey items within

Successful polio supplementary immunisation activities in a security compromised zone - Experiences from the Southwest region of Cameroon.

BACKGROUND: Supplementary immunisation activities (SIAs) play a central role in polio eradication efforts. Armed conflicts resulting in insecurity negatively affect SIAs. In the Southwest region of Cameroon, armed conflicts persisted in 2018. We present our experiences of conducting a polio SIA in an insecure region. METHODS: The SIA took place from the 2nd to 4th of March 2018 and targeted 307,920 children aged 0-59 months. Bivalent polio vaccine was used. Before the SIA, extensive planning was done under the leadership of a Central Technical Group. Planning included security assessment, adv

Mectizan delivery systems and cost recovery in the Central African Republic.

The Central African Republic (CAR) has a serious onchocerciasis problem. The disease is endemic in three quarters of the country and there is considerable onchocercal blindness in the north-west. The low population density and extreme poverty (the CAR being one of the 20 poorest countries in the world) combine to make mass treatment with Mectizan (ivermectin, MSD) a challenge. Although planned, primary health care (PHC) is not widely developed in the country. Mectizan distribution was carried out in 1993-1994 by mobile teams in order to address the most urgent need, particularly in the north-w

Choosing the best apple: counselling leaflets and technologies of communication in the history of reproduction.

Historians have shown how the establishment of human genetic counselling in West Germany was characterised by several sociohistorical factors, in particular the impact of the legacies of Nazi biopolitics. These accounts have reconstructed continuities on an intellectual level which delayed a turn towards non-directive approaches, emphasising individual (emotional) well-being and voluntariness, and instead have prolonged a discourse that defined disability as an economic and social burden. However, while the distinct legacies of eugenics and racial hygienics are well researched, other factors t

Prevalence of psychological trauma and association with current health and functioning in a sample of HIV-infected and HIV-uninfected Tanzanian adults.

BACKGROUND: In high income nations, traumatic life experiences such as childhood sexual abuse are much more common in people living with HIV/AIDS (PLWHA) than the general population, and trauma is associated with worse current health and functioning. Virtually no data exist on the prevalence or consequences of trauma for PLWHA in low income nations. METHODOLOGY/PRINCIPAL FINDINGS: We recruited four cohorts of Tanzanian patients in established medical care for HIV infection (n = 228), individuals newly testing positive for HIV (n = 267), individuals testing negative for HIV at the same sites (

Exploring influences on community-based case managers' advance care planning practices: facilitators or barriers?

Community-based geriatric case managers work with an increasingly frail older population in need of advance care planning throughout the end of life. However, little is known regarding factors that influence their professional practices in this area. This qualitative study used four focus groups to explore case managers' (N = 27) perceptions of facilitators and barriers to advance care planning practices. Themes from the study suggest that case managers view five key influences that appear to either assist or impede practices, depending on case manager's perceptions and experiences. Themes inc

Responding to chemical, biological, or nuclear terrorism: the indirect and long-term health effects may present the greatest challenge.

The possibility of terrorists employing chemical, biological, or nuclear/ radiological (CBN) materials has been a concern since 1995 when sarin gas was dispersed in a Tokyo subway. Contingency planning almost exclusively involved detection. containment, and emergency health care for mass casualties. However, it is clear that even small-scale CBN incidents--like the recent spread of anthrax spores through the mail--can cause widespread confusion, fear, and psychological stress that have lasting effects on the health of affected communities and on a nation's sense of well-being. More emphasis th

Beyond the patient: Health care providers' insights on partner engagement in contraceptive decision-making.

INTRODUCTION: Family planning is a dyadic decision influenced by social, cultural, and structural factors. However, contraceptive counseling in perinatal care often centers on pregnant individuals alone, potentially overlooking the critical role of partner involvement. This qualitative study explored health care providers' (HCPs) perceptions of involving partners in perinatal contraceptive education to understand the potential value and challenges of dyadic approaches, guided by the biobehavioral family model. METHOD: Ten HCPs (including physicians, nurse practitioners, and community health d

Abortion and its association with antiretroviral therapy among young women living with HIV in northern Uganda: a cross-sectional study.

INTRODUCTION: Abortion is a critical reproductive health issue among young women living with HIV (YWLHIV). Despite the widespread use of the antiretroviral therapy (ART) for women of reproductive age, its impact on abortion prevalence remains unclear. We set out to determine the prevalence of abortion among the YWLHIV receiving TLD-based ART regimen in northern Uganda and its association with the ART regimen and duration alongside other key socio-demographic, reproductive health, lifestyle and facility access- related factors. METHODS: A cross-sectional study of YWLHIV who reported at least o

Digital storytelling as a tool for community education and stigma reduction in tuberculosis.

This study compared digital storytelling with standard community education to reduce tuberculosis (TB) stigma. Prior work on digital storytelling in TB control often focuses on uptake, so effects measured during follow-up are less clear. A prospective mixed-methods evaluation compared community clusters. Surveys were completed before exposure and again at 3 months (90 days), alongside interviews and 30-day engagement records. The between-group difference in mean change in stigma score was -1.4 [-2.3, -0.4] with p = 0.006, which is consistent with larger average stigma reductions with digital s

Using community-based participatory approaches to improve access to mass drug administration for trachoma elimination in a pastoral conflict area of Kenya.

In Baringo County, Kenya, trachoma remains endemic despite repeated mass drug administration (MDA) efforts, with coverage in one of the wards consistently falling short of world health organization (WHO) targets. The disease is endemic in 12 out of the 47 counties in Kenya. Baringo county is a pastoral conflict, hard to reach area where eight rounds of mass drug administration (MDA) for trachoma have been implemented. In Loyamorok ward, treatment coverage has been below 68% against the WHO recommended threshold of 80%. Community engagements that promote participatory approaches are key to MDA

Perspectives on implementing Individual Placement and Support (IPS) within primary health care settings for adults living in British Columbia, Canada.

BACKGROUND: Individual placement and support (IPS) is an evidence-based practice (EBP) designed to help people with severe mental illness re-enter the labour market. Implementing an IPS program within a new context (e.g., primary health care setting) to support populations that are complex and multi-barriered presents a set of unique challenges and considerations. This paper provides community-based perspectives that identify implementation strengths and challenges and highlights potential strategies aimed at addressing emergent barriers. METHODS: A case study was conducted across three commu

Family planning self-care: From global frameworks to local meaning, perceptions, experiences and opportunities in Niger.

Family planning (FP) self-care is a strategic pillar for advancing Universal Health Coverage (UHC) and mitigating health workforce shortages. However, a significant disconnect persists between global normative frameworks and local implementation realities. This study examines the local meanings, perceptions, and experiences of FP self-care in Niger to inform contextualized scale-up of self-care interventions. We employed a sequential mixed-methods design in the Niamey (urban) and Zinder (rural) regions of Niger. A quantitative household survey was conducted with 510 women and 357 men to assess

"Because Even the Person Living With HIV/AIDS Might Need to Make Babies" - Perspectives on the Drivers of Feasibility and Acceptability of an Integrated Community Health Worker Model in Iringa, Tanzania.

BACKGROUND: Countries with health workforce shortages are increasingly turning to multipurpose community health workers (CHWs) to extend integrated services to the community-level. However, there may be tradeoffs with the number of tasks a CHW can effectively perform before quality and/or productivity decline. This qualitative study was conducted within an existing program in Iringa, Tanzania where HIV-focused CHWs working as volunteers received additional training on maternal, newborn, and child health (MNCH) promotion, thereby establishing a dual role CHW model. METHODS: To evaluate the fea

Content Analysis of Social Determinants of Health Accelerator Plans Using Artificial Intelligence: A Use Case for Public Health Practitioners.

CONTEXT: Public health practice involves the development of reports and plans, including funding progress reports, strategic plans, and community needs assessments. These documents are valuable data sources for program monitoring and evaluation. However, practitioners rarely have the bandwidth to thoroughly and rapidly review large amounts of primarily qualitative data to support real-time and continuous program improvement. Systematically examining and categorizing qualitative data through content analysis is labor-intensive. Large language models (LLMs), a type of generative artificial intel

A process for developing community consensus regarding the diagnosis and management of attention-deficit/hyperactivity disorder.

There remain large discrepancies between pediatricians' practice patterns and the American Academy of Pediatrics (AAP) guidelines for the assessment and treatment of children with attention-deficit/hyperactivity disorder (ADHD). Several studies raise additional concerns about access to ADHD treatment for girls, blacks, and poorer individuals. Barriers may occur at multiple levels, including identification and referral by school personnel, parents' help-seeking behavior, diagnosis by the medical provider, treatment decisions, and acceptance of treatment. Such findings confirm the importance of
