Prevalence and Health Care Disparities of Retinal Conditions: A Meta-Analysis.

IMPORTANCE: Age-related macular degeneration (AMD), diabetic retinopathy (DR), diabetic macular edema (DME), and retinal vein occlusion (RVO) are leading causes of vision impairment and can lead to permanent, irreversible blindness. OBJECTIVE: To update age-standardized prevalence estimates for AMD, DR, DME, and RVO by gender, race and ethnicity, and US state or county for 2022. DATA SOURCES: The National Health and Nutrition Examination Survey (NHANES; 2005-2008 and 2017-March 2020), Medicare fee-for-service claims (2017-2019), IBM MarketScan commercial insurance claims (2016), and eligible

Stimulant Use Associated With Psychosocial Factors, HIV Risk, and Concurrent Hazardous Alcohol Use Among US Adults: Exploratory Cross-Sectional Questionnaire Study.

BACKGROUND: Stimulant use is a major public health problem that contributes to morbidity and mortality among men who have sex with men (MSM) in the United States. To reduce the harms associated with stimulant use, there is a need to identify the factors associated with stimulant use to inform interventions. Additionally, there is a need to use large crowdsourcing platforms like Amazon Mechanical Turk (MTurk) to engage more individuals who use substances across the United States. OBJECTIVE: We identified the correlates of stimulant use among people who use alcohol or stimulants in the United S

Core state preconception health indicators - pregnancy risk assessment monitoring system and behavioral risk factor surveillance system, 2009.

PROBLEM/CONDITION: Promoting preconception health can potentially improve women's health and pregnancy outcomes. Evidence-based interventions exist to reduce many maternal behaviors and chronic conditions that are associated with adverse pregnancy outcomes such as tobacco use, alcohol use, inadequate folic acid intake, obesity, hypertension, and diabetes. The 2006 national recommendations to improve preconception health included monitoring improvements in preconception health by maximizing public health surveillance (CDC. Toward the development of a vibrant, super-aged society: The future of medicine and society in Japan.

BACKGROUND: As Japan's population continues to age, it is estimated that the number of people aged ≥75 years will exceed 20 million by 2025. Furthermore, over the past 10 years, we have not reduced the difference between life expectancy and healthy life expectancy. Therefore, the extension of healthy life expectancy and the development of a healthy society are the most urgent issues. In terms of medical care, the changing times have inevitably led to changes in disease structures and medical demands; therefore, the medical delivery system has had to be changed to meet these demands. As dementi

Evaluating the effects of community-based programs on viral rebound and viral suppression among HIV-positive orphaned and vulnerable children receiving antiretroviral treatment: Findings from the ACHIEVE project in Tanzania.

BACKGROUND: Achieving and sustaining viral load suppression (VLS) among children living with HIV remains challenging despite the availability of antiretroviral therapy (ART), primarily due to adherence difficulties. This study evaluated the effects of three interventions under the ACHIEVE project on viral load outcomes (viral rebound and viral suppression) among HIV-positive orphaned and vulnerable children aged 0-17 years (hereafter referred to as CLHIV) receiving ART in Tanzania. METHODS: This is a longitudinal analysis of 26,257 CLHIV who are beneficiaries of the ACHIEVE project in Tanzani

Immunization coverage levels among 19- to 35-month-old children in 4 diverse, medically underserved areas of the United States.

BACKGROUND: The National Immunization Survey demonstrates that national immunization coverage in 2002 remained near the all-time highs achieved in 2000. However, that survey cannot detect whether coverage is uniformly high within relatively small areas or populations. The measles resurgence in the early 1990s revealed that coverage was low in some areas, particularly among inner-city children from racial and ethnic minority groups. Today, identifying areas with low childhood-vaccination coverage remains important, particularly if these areas are at risk for the introduction of disease. In 1995

Antithrombotic Medications and Intraocular Hemorrhage Risk in Exudative Age-Related Macular Degeneration.

IMPORTANCE: Although the use of anticoagulants or antiplatelets is known to increase bleeding risk, there is limited evidence on whether they contribute to a higher incidence of intraocular hemorrhage in patients with age-related macular degeneration (AMD). OBJECTIVE: To examine the association between anticoagulant or antiplatelet use and clinically important intraocular hemorrhage requiring vitrectomy in patients with exudative AMD. DESIGN, SETTING, AND PARTICIPANTS: In this nationwide, population-based, retrospective cohort study using the Korean Health Insurance Review and Assessment Ser

Analysis of online user discussions on Reddit associated with the transition of use between HIV PrEP therapy.

In 2019, the U.S. Food and Drug Administration (FDA) approved emtricitabine and tenofovir alafenamide (Descovy) as another option for HIV pre-exposure prophylaxis (PrEP) prevention for high-risk adults and adolescents. With the introduction of this new PrEP, millions of current users on emtricitabine and tenofovir disoproxil fumarate (Truvada), another PrEP medication currently used to prevent HIV transmission, have options of whether to continue their current treatment regime or transition to new treatment options. The objective of this study was to conduct a descriptive analysis to character

Assessing the impact of pediatric-based development services on infants, families, and clinicians: challenges to evaluating the Health Steps Program.

BACKGROUND: Begun in 1996, the Healthy Steps for Young Children Program (HS) is a new model of pediatric practice that incorporates child development specialists and enhanced developmental services for families of young children. HS is for all families, not just those at high-risk. It is expected to strengthen parents' knowledge, attitudes, and behaviors in ways that promote child health and development, and in turn, to lead to improved child outcomes, such as improved language development, increased utilization of well child care, and decreased problem behaviors, hospitalizations, and injurie

Protocol for a multi-centre, definitive randomised controlled trial of the effectiveness of Individual Placement and Support for employment support among people with alcohol and drug dependence.

BACKGROUND: Unemployment is highly prevalent in populations with alcohol and drug dependence and the employment support offered in addiction-treatment programmes is ineffective. Individual Placement and Support (IPS) is an evidence-based intervention for competitive employment. IPS has been extensively studied in severe mental illness and physical disabilities, but there have been no formal randomised controlled trials (RCTs) in alcohol and drug dependence. The Individual Placement and Support for Alcohol and Drug Dependence (IPS-AD) study should determine whether IPS for patients with alcohol

The effects of the Rx Kids unconditional cash prescription programme during pregnancy and infancy on birth outcomes in the USA: a population-based, quasi-experimental study.

BACKGROUND: Rates of preterm birth and low birthweight remain high in the USA, in part due to perinatal poverty and societal underinvestment during a crucial window of human development. In 2024, Rx Kids was launched in Flint, Michigan, providing every expectant mother with unconditional cash transfers during pregnancy and infancy. It is the first cash transfer programme in the USA with universal eligibility (ie, community wide and no means testing) that targets the perinatal period. This study aimed to estimate the impact of Rx Kids on birth outcomes. METHODS: In this population-based study,

[Prolonged Weaning - S2k-Guideline Published by the German Respiratory Society].

Mechanical ventilation (MV) is an essential part of modern intensive care medicine. MV is performed in patients with severe respiratory failure caused by insufficiency of respiratory muscles and/or lung parenchymal disease when/after other treatments, (i. e. medication, oxygen, secretion management, continuous positive airway pressure or nasal highflow) have failed.MV is required to maintain gas exchange and to buy time for curative therapy of the underlying cause of respiratory failure. In the majority of patients weaning from MV is routine and causes no special problems. However, about 20 %

Is There a Relationship Between Social Health and Musculoskeletal Discomfort and Incapability? A Systematic Review.

BACKGROUND: The notable and consistent evidence that musculoskeletal symptom intensity varies according to thoughts and feelings as much or more so than pathophysiologic factors has firmly established the importance of comprehensive, biopsychosocial, whole-person health strategies in orthopaedic surgery. A systematic review of existing knowledge of the role of social health factors (defined as security in relationships, roles, finances, housing, and sustenance) as sources of variation in levels of musculoskeletal discomfort and incapability could further inform the development and disseminatio

Baseline characteristics and secondary medication adherence among Medicare patients diagnosed with transthyretin amyloid cardiomyopathy and/or receiving tafamidis prescriptions: A retrospective analysis of a Medicare cohort.

BACKGROUND: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an underdiagnosed, life-threatening condition that mostly affects older persons. In May 2019, regulatory approval of tafamidis provided the first pharmacologic treatment of ATTR-CM. In the pivotal phase 3 Transthyretin Amyloidosis Cardiomyopathy Clinical Trial (ATTR-ACT), 97.2% of patients were classified as adherent (defined as taking ≥ 80% of scheduled doses). Given its recent approval, there is limited real-world evidence examining patient adherence to tafamidis. OBJECTIVE: To evaluate adherence patterns, demographics, and clinic
