[Confusion, tachypnea, and tachycardia in a 71-year-old man with type 2 diabetes mellitus].

Patients with type 2 diabetes who present with confusion and/or abdominal pains should be screened for sodium-glucose cotransporter 2 (SGLT-2)-induced diabetic ketoacidosis. Severe acidosis was diagnosed despite only moderately increased blood sugar levels. If so, immediate ICU treatment is essential.

Anticoagulation in Patients with End-Stage Renal Disease and Atrial Fibrillation: Confusion, Concerns and Consequences.

End-stage renal disease (ESRD) patients have a higher prevalence of diabetes mellitus, hypertension, congestive heart failure and advanced age, along with an increased incidence of non-valvular atrial fibrillation (AF), thereby increasing the risk for cerebrovascular accidents. Systemic anticoagulation is therefore recommended in patients with ESRD with AF to reduce the risk and complications from thromboembolism. Paradoxically, these patients are at an increased risk of bleeding due to great degree of platelet dysfunction and impaired interaction between platelet and endothelium. Currently, C

Hypergammaglobulinemia, normal serum albumin and hypercalcaemia: a case of systemic sarcoidosis with initial diagnostic confusion.

A 53-year-old Afro-Caribbean woman presented to casualty with a constellation of symptoms pointing to a diagnosis of hypercalcaemia. This was confirmed on laboratory investigation. Findings of a raised serum protein (108 g/l) and normal albumin (35 g/l), lead to an initial working diagnosis of multiple myeloma. However, later serum protein electrophoresis found a polyclonal gammopathy and further investigation lead to a diagnosis of sarcoidosis. The patient responded well to conventional treatment with oral prednisolone. A number of learning points have been highlighted including the pitfalls

State of confusion: sorting out a complex medication regimen for a 79-year-old woman.

Patients with Alzheimer's disease and without caregiver assistance can often have difficulties in managing their medications, which can result in serious consequences. This case study depicts a 79-year-old woman who lives in an assisted living facility and is referred to a geriatric pharmacy specialist in a community pharmacy for a medication review and assistance in medication adherence. During the initial patient visit, the patient was found to have an extremely complex method for managing her medications. She also had multiple medical providers, several duplicate medications, and medication

Battleground: chronic kidney disorders mineral and bone disease--calcium obsession, vitamin d, and binder confusion.

Renal osteodystrophy is a significant complication in chronic kidney disease. This condition is referred to as mineral and bone disorders in chronic kidney disease, mainly because of its wider ranging impact, including an association with increased mortality and non-bone-related morbidity. Because most of the abnormalities that characterize mineral and bone disorders in chronic kidney disease (e.g., hyperphosphatemia, secondary hyperparathyroidism) are amenable to therapeutic interventions, this field has also been in the cross-hairs of many pharmaceutical companies. The advent of a number of

Chronic lymphocytic leukemia with an interfollicular architecture: avoiding diagnostic confusion with monocytoid B-cell lymphoma.

Certain low grade B-cell lymphoproliferative disorders can be mistaken for each other morphologically, particularly when there is partial lymph node involvement. We encountered two cases of chronic lymphocytic leukemia, in which the interfollicular growth pattern and the pale appearance of the neoplastic proliferation in the lymph nodes led to a misclassification as monocytoid B-cell lymphoma. The correct diagnosis was established, however, when the lymph node morphology was carefully reexamined, with the knowledge of the clinical history, peripheral blood findings, and bone marrow data. The i

Wiping out confusion: A toolkit to define roles and improve cleanliness in complex patient care spaces.

BACKGROUND: A clean patient care environment is critical for safety, yet medical technology, unclear cleaning ownership, and diverse teams complicate expectations. Reliable cleaning practices are essential. METHODS: A partnership between Infection Prevention, Nursing, and Environmental Services was created to assess and improve cleaning in clinical care environments. The institutional improvement framework was used to guide this work. RESULTS: A partnership between Infection Prevention, Nursing, and Environmental Services led to sustained improvements in room cleanliness and staff understand

Confusion in evaluating research quality comes from lack of clarity: The case for explicit research questions.

A recent news article suggested observational studies confuse correlation and causation. Such confusion often arises from unclear research aims rather than inherent limitations of study design. We use this recent public critique of epidemiologic research to illustrate how ambiguity in study questions contributes to misinterpretation of findings. We argue that epidemiologic studies generally fall into three distinct categories (descriptive, predictive, and causal) and that each has different goals, assumptions, analytic approaches, and criteria for evaluation. Descriptive studies characterize t…

Unexplained portal hypertension and confusion in an elderly patient: a late presentation of congenital hepatic fibrosis.

A 71-year old male patient with myelodysplastic syndrome presented with severe signs of portal hypertension. There was no underlying cirrhosis nor portal vein thrombosis. Despite liver stiffness being high, hepatic vein catheterisation failed to show an increased hepatic venous pressure gradient compatible with clinically significant portal hypertension. Finally, a liver biopsy showed enlarged fibrotic portal tracts with multiple dilated immature bile ducts compatible with congenital hepatic fibrosis - there was no macroscopic biliary disease. Mostly presenting in childhood or adulthood, conge

GCSTG: Generating Class-confusion-aware Samples with a Tree-structure Graph for Few-shot Object Detection.

Few-Shot Object Detection (FSOD) aims to detect the objects of novel classes using only a few manually annotated samples. With the few novel class samples, learning the inter-class relationships among foreground and constructing the corresponding class hierarchy in FSOD is a challenging task. The poor construction of the class hierarchy will result in the inter-class confusion problem, which has been identified as a primary cause of inferior performance in novel classes by recent FSOD methods. In this work, we further find that the intra-super-class confusion, where samples are misclassified a…

A lightweight multi round confusion-diffusion cryptosystem for securing images using a modified 5D chaotic system.

In recent years, technological advancements have made the transmission of confidential information spooky. This research proposes a modified 5D chaotic map and a new image encryption algorithm based on an integrated chaotic system developed with SHA-512 hashing and a confusion-diffusion architecture. The modified 5D chaotic map provides randomness, and its performance is evaluated through a bifurcation diagram and Lyapunov exponent. The randomness of chaotic sequences is validated through the NIST test. The multi-round diffusion and permutation incorporating the proposed chaotic sequences sign…

The dilemma of sweet temptation: How sugar perception confusion in sweetened beverages shapes consumer avoidance behavior.

Despite widespread consumption of sugar-sweetened beverages, consumers face contradictory information from health authorities, marketing, and social media, yet limited research examines how this information conflict affects purchasing decisions. This study investigates how sugar perception confusion influences purchasing avoidance through ambivalent attitudes. Based on cognitive dissonance and information processing theories, we developed a cognitive-affective-behavioral model examining relationships among sugar perception confusion, ambivalent attitudes, and purchasing avoidance behaviors. Us…

Confusion With Presentations of Calcium Pyrophosphate Dihydrate Disease: A Report of Two Cases Mistaken for Cellulitis.

Both pseudogout and cellulitis are diseases that may mimic one another in clinical practice. We discuss two cases of acute calcium pyrophosphate dihydrate (CPPD) arthritis mistaken for cellulitis in the emergency department. Both patients experienced significant improvement after management was changed to treat CPPD. These cases highlight how it is essential for physicians to consider CPPD as a differential diagnosis for a patient that is presenting with signs of inflammation in any joint.

Uncertainty and Confusion Regarding Transgender Non-discrimination Policies: Implications for the Mental Health of Transgender Americans.

INTRODUCTION: Recent policies have restricted the rights of the US transgender people; there is a need to explore transgender people's knowledge and attitudes regarding such policies and related mental health. METHODS: In 2019, 580 transgender adults living in the Northeastern US completed a survey assessing demographics, knowledge/attitudes toward transgender-related policies, and mental health. Multivariable logistic regression models, adjusted for age, gender, race, and prior abuse, were fit to examine the association of concerns about the implementation of discriminatory policies and depr
