Allergic rhinitis: diagnosis and management.

Allergic rhinitis affects 20% of the population of the UK. It confers a significant health burden upon the individual as it affects the patient's quality of life and is associated with serious comorbidities including asthma, sinusitis and conjunctivitis. Owing to its prevalence, it has a significant economic impact through its effects on education, productivity and use of healthcare resources. This review focuses on the management of allergic rhinitis and potential future treatments, because of the lack of clear national guidelines and because this illness is often misdiagnosed and mismanaged.

Topical Percutaneous Drug Delivery for Allergic Diseases: A Novel Strategy for Site-Directed Pharmacologic Modulation.

Topical percutaneous drug delivery has recently emerged as a novel strategy for the treatment of allergic diseases, offering targeted drug delivery to mucosal tissues adjacent to the skin. Unlike conventional topical approaches that act on the skin surface or mucosal membranes, topical percutaneous drug delivery enables non-invasive pharmacologic modulation of deeper structures such as the conjunctiva, nasal mucosa, and trachea. This review explores the rationale, pharmacokinetic foundation, clinical data, and future prospects of transdermal therapy in allergic conjunctivitis, allergic rhiniti…

Exploring the relationship between serum 25-hydroxyvitamin D levels and allergic rhinitis severity in the pediatric population.

OBJECTIVES: Recent studies have suggested that serum 25-hydroxyvitamin D3 (s25-OHD3) may modulate immune responses in allergic diseases. However, the relationship between s25-OHD3 levels, allergic rhinitis (AR) severity, and allergen sensitization remains unclear. This study aimed to investigate the association between s25-OHD3 levels and AR severity, including the potential role of allergen sensitization. METHODS: This retrospective study, conducted at Ankara Bilkent City Hospital between 2019 and 2024, included 343 children with AR aged 2 to 18 years. Patients were evaluated for s25-OHD3 le

Dominance of IL-5 and CCL17 in nasal cytokine profiles of children with type 2 inflammation.

BACKGROUND: Type 2 (T2) inflammation, driven by TH2 cells and associated cytokines, has been linked with asthma and allergic rhinitis. Early detection and immunophenotyping of T2-related diseases in childhood are crucial for effective targeted therapies. OBJECTIVE: We evaluated the usefulness of a noninvasive method to measure nasal cytokine profiles for airway-specific immunophenotyping of T2 inflammation in children. METHODS: Nasal epithelial lining fluid was analyzed for levels of 24 cytokines in 612 children at age 6 years from the Danish COPSAC2010 mother-child cohort with prospective d

Dermatophagoides Pteronyssinus 1 (DerP1) May Trigger NLRP3-Mediated Corneal Epithelial Cell Pyroptosis by Elevating Interleukin-33 Expression Levels.

PURPOSE: To characterize the in vivo effects of Dermatophagoides pteronyssinus 1 (DerP1) in mice and determine the underlying NLRP3 inflammasome-mediated pyroptosis signaling mechanisms in the human corneal epithelial cells (HCECs). METHODS: DerP1 was used to induce allergic conjunctivitis in C57 mice. HCECs were sensitized with DerP1 in vitro to mimic their condition observed in allergic conjunctivitis in vivo. Transmission electron microscopy was used to evaluate pyroptosis in the HCECs, enzyme-linked immunosorbent assays to assess interleukin (IL)-33, IL-1β and IL-4 levels, flow cytometry

Rhinitis phenotypes and multimorbidities in the general population: the CONSTANCES cohort.

BACKGROUND: Scarce epidemiological studies have characterised allergic rhinitis (AR) and non-allergic rhinitis (NAR) in adults. In a population-based cohort, our aims were to 1) describe rhinitis, AR and NAR, and 2) explore how asthma and conjunctivitis may lead to the identification of novel rhinitis phenotypes. METHODS: In this cross-sectional analysis, current rhinitis was defined as present in the last 12 months using a questionnaire from the French CONSTANCES cohort. Participants with current rhinitis reporting nasal allergies were considered as AR, otherwise as NAR. We described AR and

Beneficial pharmacological effects of selective glucocorticoid receptor agonist in external eye diseases.

PURPOSE: Glucocorticoids exert their actions via the glucocorticoid receptor through at least 2 intracellular mechanisms, known as transrepression and transactivation. It has been hypothesized that transrepression is the basis of their anti-inflammatory effects, whereas transactivation has been assumed to cause their side effects. ZK209614, a recently identified, novel selective glucocorticoid receptor agonist, exerts strong transrepression and weak transactivation. The objective of this study was to determine whether its pharmacological effects can be dissociated from its side effects. For th

Exposure to ambient air pollution during trimesters of pregnancy and childhood allergic diseases in Wuhan, China.

The study explored the associations between maternal exposure to air pollution during different trimesters and allergic diseases including asthma, allergic rhinitis, allergic conjunctivitis or/and eczema. Individual exposure to air pollutants was assessed by an inverse distance weighted (IDW) method using daily concentrations of SO2, NO2, PM10, and PM2.5 from air quality monitoring stations. Multiple logistic regression model was performed to estimate the associations between air pollution during each trimester of pregnancy and childhood allergic diseases. A total of 332 children (51.3%) were

Allergy and infections: long-term improvement of vernal keratoconjunctivitis following viral conjunctivitis.

PURPOSE: Vernal keratoconjunctivitis (VKC) is a severe, chronic allergic inflammatory disease of the ocular surface poorly responsive to antiallergic treatments and possibly leading to permanent visual impairment. VKC, because of mast cell, eosinophil, and Th2-type inflammation, polyclonal IgE activation, and tissue remodeling, is considered to be a typical Th2- driven disease. Viral infection stimulates a Th1 type immune response, potentially attenuating allergen-induced inflammation. The purpose of this report is to describe the effect of viral keratoconjunctivitis in a patient with VKC. ME

Animal models of ocular allergy and their clinical correlations.

Ocular allergic diseases represent a wide spectrum of disorders, from the acute self-limited, mild form of seasonal allergic conjunctivitis to the chronic, severe, sight-threatening atopic keratoconjunctivitis. The least problematic forms are the most prevalent, and several animal models have contributed to elucidate their etiopathogenetic mechanisms and have served to test numerous anti-allergic compounds. The most severe and chronic, although less prevalent, ocular allergic problems have not benefited from a similar advance, with the subsequent lack of full understanding and a limited therap

Allergen immunotherapy, routes of administration and cytokine networks: an update.

Allergen immunotherapy is a disease-modifying therapy, effective for the treatment of allergic rhinitis, allergic asthma, conjunctivitis or stinging insect allergy. Allergen immunotherapy involves the administration of increasing doses of allergens with the aim of ameliorating the allergic response. Although precise underlying mechanisms of the induction of immune tolerance remain unclear, immunotherapy has been associated with the induction of distinct subsets of Tregs that eventually lead to peripheral tolerance by inducing a deviation from Th2 to Th1 immune responses. This review focuses on

Allergic rhinoconjunctivitis: pathophysiological mechanism and new therapeutic approach.

Allergic rhinoconjunctivitis (AR) is the most common IgE-mediated disease. A type2 immune response is involved in AR pathogenesis. Allergic inflammation is characterized by eosinophilic infiltrate and mediators release. AR treatment is usually based on medication prescription, including antihistamines and intranasal corticosteroids. However, medications may be prescribed for long periods and sometimes may be scarcely effective, thus aggressive strategy should be used. Therefore, complementary medicine is becoming attractive for patients at present. Nutraceuticals represent interesting therapeu

Hypersensitivity responses and contact lens wear.

Hypersensitivity diseases that involve the eye are common reasons why patients present to eyecare practitioners. Common ocular hypersensitivity disorders include allergic conjunctivitis, giant or contact lens induced papillary conjunctivitis and atopic keratoconjunctivitis. The diagnosis and management of ocular hypersensitivity can present a challenge to eyecare practitioners and an understanding of the mechanisms that underlie the signs and symptoms of such conditions is necessary for their appropriate management. This article reviews the mechanisms by which the eye responds to antigenic cha
