The use of antithrombotics for acute coronary syndromes in the emergency department: considerations and impact.

Evidence-based guidelines for the management of acute coronary syndromes (ACS) identify a central role for unfractionated heparin (UFH) or low-molecular-weight heparin (LMWH). A recent study has suggested that interchanging between UFH and LMWH during the course of treatment may be associated with a worse outcome than continued therapy with either form of heparin. Because this has important implications for physicians in the emergency room, this review examines the current evidence for the efficacy and safety of heparins in ACS. In patients with acute myocardial infarction, several studies hav

[Technique and diagnostic value of musculoskeletal ultrasonography in rheumatology. Part 6: ultrasonography of the wrist/hand].

Sonography of the hands is especially helpful in the diagnosis of early arthritis. Sonography allows for a very sensitive detection of small joint-effusion, tenosynovitis and small erosive bone lesions earlier than conventional radiography. Musculoskeletal sonography is also helpful in morphological analysis of changes of the median nerve in patients with carpal tunnel syndrome. The following standard scans are suggested for the sonographic evaluation of the wrist: 1. dorsal longitudinal scan along the radio-carpal joint, 2) along the ulno-carpal joint, and 3) dorsal transverse scan along the

SARS-CoV-2 Infection-Induced Alterations in ADAR Editing Patterns Differ Between Patients Who Developed Critical Compared to Non-Critical COVID-19.

COVID-19, caused by the SARS-CoV-2 virus, has a wide spectrum of clinical presentations even among individuals with similar demographics. Disease severity has been linked with viral-mediated expression of interferons and interferon-stimulated genes. Among the interferon-stimulated genes are members of adenosine deaminases acting on the RNA (ADAR) family that contribute to transcriptome diversity and modulate immune response. Previous studies identified altered ADAR expression and editing patterns during SARS-CoV-2 infection, although it remains unclear whether ADAR expression and activity diff

Transcriptomic profiling of severe and critical COVID-19 patients reveals alterations in expression, splicing and polyadenylation.

Coronavirus disease 2019 (COVID-19) is a multi-systemic illness that became a pandemic in March 2020. Although environmental factors and comorbidities can influence disease progression, there is a lack of prognostic markers to predict the severity of COVID-19 illness. Identifying these markers is crucial for improving patient outcomes and appropriately allocating scarce resources. Here, an RNA-sequencing study was conducted on blood samples from unvaccinated, hospitalized patients divided by disease severity; 367 moderate, 173 severe, and 199 critical. Using a bioinformatics approach, we ident

CT Scan Utilization Decreases With Implementation of a Clinical Pathway for Children With Suspected Acute Appendicitis.

OBJECTIVES: The National Surgical Quality Improvement Program-Pediatric (NSQIP-P) recommends reducing computed tomography (CT) scan use to <25% in children with suspected appendicitis. Our multidisciplinary team developed a pediatric appendicitis clinical pathway that emphasizes the use of ultrasound as first-line imaging and reserves CT for patients with both non-diagnostic ultrasounds and agreement from pediatric emergency medicine and surgery teams. METHODS: For this local study, all cases of patients aged younger than 19 years who were diagnosed with appendicitis from 2021 to 2023 were re

Co-designing an interprofessional care pathway for (risk of) malnutrition and sarcopenia in community-dwelling older adults.

BACKGROUND: Integrating care for (risk of) malnutrition and sarcopenia in primary care is challenging, as limited physical proximity among healthcare professionals hinders collaboration. Both health conditions are common in community-dwelling older adults and are associated with significant declines in physical functioning, independence, and quality of life. Healthcare professionals tend to manage malnutrition and sarcopenia separately, leading to missed opportunities for early (risk) identification and coordinated care. An interprofessional care pathway can provide an evidence-based, structur

Aligning with optimal care pathways for people with colon cancer: Clinical, survival and cost implications across socioeconomic and geographic populations in Australia.

BACKGROUND: Optimal Care Pathways (OCPs) are national pathway-based frameworks designed to promote timely, coordinated, evidence-based cancer care through tumour-specific action points. However, population-level evidence on associations between OCP alignment, survival, and healthcare costs, remains limited. OBJECTIVE: To examine factors associated with OCP alignment and its relationship with five-year survival and hospital costs among people with colon cancer in Victoria, Australia. METHODS: A retrospective cohort study used linked administrative data from the Victorian Cancer Registry, Vict

[Expert consensus on clinical pathway for high-conformity total knee arthroplasty through a minimally invasive surgical approach].

With the growing demand for total knee arthroplasty (TKA) in patients with end-stage knee osteoarthritis, the integration of minimally invasive surgical concepts with high-conformity personalized prosthesis design has become an important strategy for improving surgical precision and postoperative outcomes. However, standardized clinical pathway guidance for this combined approach remains lacking. Therefore, based on the current body of evidence and through multicenter expert collaboration, this consensus aims to establish a standardized clinical application protocol. The consensus focuses on t

Safety and efficacy of a surgical ambulatory care pathway for superficial abscesses.

BACKGROUND: Superficial abscess of the skin is a common emergency presentation often requiring surgical management. Ambulatory Care Pathways (ACPs) are a method that can reduce the burden of this disease on hospital bed pressure by treating patients with superficial abscesses as day cases. We conducted a prospective cohort study to assess the safety and effectiveness of an ACP for patients meeting strict criteria with a superficial abscess. METHODS: Data on a new ACP for the management of superficial abscesses was collected for 12 months from June 20th, 2021 and compared to a retrospective co

Using a Clinical Pathway to Safely Reduce Transfers and Admissions for Croup in the Urgent Care.

OBJECTIVE: To evaluate the impact of a croup clinical pathway (CP) on transfer rates from our urgent cares (UCs). METHODS: We revised our croup CP in November 2022 to recommend giving additional doses of racemic epinephrine (RE) in UC before transferring a patient and using shared decision-making with families on post-RE observation of their child at home or in a clinical setting. We conducted a retrospective cohort study of patients with croup in 3 UCs who received RE in the pre-CP revision (November 2021 to October 2022) and post-CP revision (November 2022 to October 2023) periods. A Clinical Audit of the Use of CT Head Scan Post-Inpatient Falls in Hospitalised Older Adults Utilising a Post-Fall Clinical Pathway.

Background: Older adults are at high risk of falls, and head injuries following these events can have devastating consequences. The In-Patient Post Fall Clinical Pathway is a tool utilised in many hospitals in Queensland, Australia, to guide the need for CT brain imaging post-inpatient fall. This audit aimed to assess the use of CT imaging following inpatient falls in older adults, evaluate adherence to the In-Patient Post Fall Clinical Pathway, and explore factors associated with serious head injury. Methods: A retrospective audit was conducted across two regional Queensland hospitals over 2.

Association between symptoms of acute cystitis and findings of bladder cancer in the Swedish standardized pathway for macroscopic haematuria.

Standardized Care Pathway (SCP) for patients presenting with macroscopic haematuria was introduced in Sweden in 2015. In contrast to neighbouring countries, the Swedish model includes all patients with macroscopic haematuria above the age of 50, notwithstanding obvious causes. The aim of this study was to characterize the patients referred within the SCP and the presence of acute cystitis. All patients admitted to two regional hospitals in 2023, under the terms of SCP macroscopic haematuria, were included. Patient data were collected retrospectively regarding age, sex, symptoms of cystitis and
