Recent insights into the nature of hemorrhoids and anal fissures have led to specific strategies for treatment. Symptomatic hemorrhoids often result from prolapse of submucosal vascular cushions. Appropriate treatments include dietary change, injection therapy, rubber-band ligation, and hemorrhoidectomy. Patients with anal fissures usually have an abnormal contraction of the sphincter. Many fissures heal spontaneously. Treatment often centers on helping the patient avoid constipation, although surgery is indicated in some cases.

Six skin refrigerants were evaluated for maximum cooling temperature. The temperatures produced correlated well with the chemical components and also the gelatin freeze-thaw times. Freon 114 and Freon 114-ethyl chloride mixtures are time-tested, safe skin refrigerants. Some of the newer skin refrigerants are pure sources of Freon 12 or mixtures of Freon 12 and Freon 11. These newer, colder products have the potential to damage the skin and represent a hazard to successful dermabrasion.

The natural history of the hemangioma is spontaneous regression; therefore, most lesions require no treatment. The large occlusive subglottic hemangioma which presents before the age of 6 months, however, may be life-threatening. Two infants with subglottic hemangiomas with complete upper airway obstruction were treated successfully with cryotherapy, after tracheostomy. A group of patients with retinal breaks without detachment were analyzed. They were divided into a treatment and a nontreatment group. The treatment group consisted of cases that the authors felt to be at high risk for the development of retinal detachment. Most of these were cases of horseshoe tears following the onset of an acute posterior vitreous detachment. Although a number of modalities were used in the treatment, a transconjunctival cryotherapy approach with topical anesthetic drops is currently used and was the most frequently employed. Twenty five patients (mostly boys between the ages of nine and 14) suffering from vernal conjunctivitis were treated by cryotherapy. Three cryocycles, each of one to two minutes' duration, consisting in contact freezing at -160 degrees C, were performed under local anesthetic. There was very rapid regression of the acute inflammatory symptoms within a few days. At the same time, the eosinophil count in the smear preparation decreased considerably. The "cobblestones" only flattened out in the course of several weeks, following repeated sessions.

Histopathological changes following cyclocryokoagulation in a group of rabbits pretreated with acetylsalicyclic acid (ASA) were studied and compared with similar changes in a control group. A report is given on cryotherapy of a subconjunctival iris prolapse in a 46-year-old man 23 days after cataract extraction. The procedure is simple, without complications, and was in accordance with descriptions in the literatur also in this case successful.
