An unusual form of duplicate bladder exstrophy.

Duplicate bladder exstrophy is a rare variant of the exstrophy/epispadic lesions in which an external patch of exstrophic bladder presents in addition to a closed bladder in a more normal anatomical position, diastasis of the pubis and rectus muscles, and often a form of epispadias. We describe an anomaly of this variant with a fistula to the closed bladder. Careful examination with fluoroscopy and cystoscopy is helpful in defining the anatomy in these variants.

[Secondary obstructive uropathy to inverted papilloma of the ureter].

We report a new case of inverted papilloma of the ureter, with an uncommon clinic manifestation as a obstructive uropathy. In spite of the bening nature the inverted papilloma of the urinary tract, it can be multifocal and relapsing and it may be associated with low rate urothelial carcinomas. A precise pre-operative diagnosis is necessary for a conservative or endourologic treatment. The followup of these cases with periodic urography and cystoscopy is mandatory.

Partial perforation of bladder by multiload.

A rare case of partial perforation of the wall of the bladder by an intrauterine contraceptive device (IUCD) in a puerperal woman and its subsequent laparoscopic removal is described. Cystoscopy revealed an intact bladder mucosa with the IUCD lying submucosally. This case demonstrates that an IUCD embedded in the wall of the bladder, whose mucosa is intact, can be safely removed through a laparoscope without resorting to laparotomy.

Bilateral cecoureteroceles causing urinary retention in the newborn.

The cecoureterocele differs from the usual ectopic ureterocele because it extends outside the bladder and may obstruct the urethra. The differentiation requires careful examination of the urethra by a voiding cystourethrogram and cystoscopy. We describe the management of a girl with bilateral cecoureteroceles who presented as a newborn with urinary retention and sepsis. Surgical correction of the cecoureterocele may differ from that required for the usual ectopic ureterocele.

Hernia mesh migration into bladder presenting as malignancy: potential source of diagnostic errors.

Hernia mesh erosion into bladder is a rare complication of hernioplasty. We report two cases of patients complained of recurrent frank hematuria; based on clinical and radiological evaluation, malignant tumor was considered. Cystoscopy and open surgery were performed subsequently which found the migration of mesh into bladder. History reviewed showed that they underwent hernioplasty 3 and 4 years ago, respectively.

[Ureteral inverted papilloma: a case report].

A 43-year-old man presented with left hydronephrosis, incidentally found by ultrasonography. He had undergone transurethral bladder tumor resection when he was 29 years old. Cystoscopy revealed a tumor protruding from the left ureteral orifice. Left partial ureterectomy was performed, and its histopathological diagnosis was ureteral inverted papilloma. He has remained free of disease for 18 months after the surgery.

Colovesical fistulas.

Colovesical fistula often presents with recurrent or persistent urinary tract infection, especially in men. The commonest cause is diverticular disease. Other causes include carcinoma of the colon, Crohn's disease, radiotherapy and trauma. Barium enema may suggest the pathology but cystoscopy is the best investigation to confirm the presence of a fistula. One-stage resection and anastomosis is suitable for most patients with diverticular disease.

Urethral recurrence of bladder tumor occurring as a perineal mass 2 years after transurethral resection: a case report.

Transitional cell carcinoma can develop as a multifocal tumor in the urinary system, especially in the bladder. Here, we report a 69-year-old man who had undergone transurethral resection for bladder tumor and had urethral recurrence that presented as a perineal mass 2 years after treatment. However, he had obtained normal cystoscopy, ultrasonography and computed tomography results at follow-up examinations.

Recurrent multifocal primary amyloidosis of urinary bladder.

Primary localized amyloidosis of bladder is rare. We report a case of recurrence of multifocal primary amyloidosis of urinary bladder. Cystoscopy revealed a diffuse left lateral wall lesion with normal surrounding mucosa. Histopathological examination of the specimen revealed urinary bladder amyloidosis with negative surgical margins. Recurrent urinary bladder amyloidosis was confirmed 3 months after the first resection. Close follow-up is recommended.

Nephrogenic adenoma of the bladder in children.

Nephrogenic adenoma is reported in 3 young girls, whose bladders had all previously been operated on. Nephrogenic adenoma is most likely a metaplastic reaction of the bladder epithelium to irritative stimuli. Malignant degeneration has not been proven. Treatment consists of transurethral resection with fulguration of the tumor base. A follow-up as for other bladder tumors--cystoscopy at regular intervals--is recommended.

Optimal Duration of Hydrodistension for Symptomatic Treatment of Interstitial Cystitis: A Systematic Review.

INTRODUCTION: Interstitial cystitis (IC) is a chronic pelvic condition in the absence of other pathology leading to significant morbidity with limited available treatment options. Cystoscopic hydrodistension (HD) remains an offered intervention for diagnosis and management, providing temporary relief in some patients. There is no overall consensus regarding the optimal duration of HD. This systematic review aims to comprehensively assess the existing literature to identify if there is a relationship between HD time and treatment efficacy. MATERIALS AND METHODS: A systematic search in MEDLINE,

Phenotyping of interstitial cystitis/bladder pain syndrome.

Interstitial cystitis/bladder pain syndrome is a chronic, potentially debilitating condition characterized by pain perceived to be related to the bladder in conjunction with lower urinary tract symptoms, and includes a wide variety of clinical phenotypes with diverse etiologies. Currently the only clinically relevant proven phenotype of interstitial cystitis/bladder pain syndrome is the Hunner lesion. Whether the presence of Hunner lesions is a hallmark of a distinct disease cohort or a potentially transient feature of non-Hunner lesion phenotype has been debated but remains controversial. The

How can we improve the diagnosis and management of bladder pain syndrome? Part 1: ICI-RS 2018.

BACKGROUND: This paper summarizes the discussion in a think tank at the ICI-RS 2018 about the diagnosis of bladder pain syndrome (BPS). AIMS: To review the guidelines, investigations and subtypes of BPS. MATERIALS AND METHODS: Review of literature in the light of the think tank discussion. RESULTS: All guidelines recommend completing history, physical examination, urine analysis, urine culture, and urine cytology to define the BPS phenotype but there are differences on further investigations. In those guidelines which recommend cystoscopy, the identification of Hunner's lesions (HLs) is rec…
