The prognosis of endometrial cancers stratified with conventional risk factors and modified molecular classification.

This study aimed to validate the Proactive Molecular Risk Classifier for Endometrial Cancer, a modified version of The Cancer Genome Atlas, using data from 184 patients with endometrial cancer (median age: 57.5 years; median follow-up period: 109 months) who had undergone radical surgery (including systemic lymphadenectomy) and subsequent adjuvant chemotherapy (patients with intermediate or high recurrence risk) from 2003 to 2015. Tissue microarrays were prepared from surgical specimens and classified using the conventional clinical risk classifier. Immunohistochemistry was used to detect mism

Dostarlimab for recurrent mismatch repair-deficient endometrial cancer: A cost-effectiveness study.

OBJECTIVE: Patients with recurrent endometrial cancer treated with carboplatin and paclitaxel whose disease progresses have few effective treatment options. Based on promising clinical trial data, the anti-programmed cell death 1 (anti-PD-1) antibody dostarlimab was recently granted accelerated approval for endometrial cancer by the US Food and Drug Administration. We developed a decision model to examine the cost-effectiveness of dostarlimab for patients with progressive/recurrent deficient mismatch repair (dMMR) endometrial cancer whose disease has progressed with first-line chemotherapy. BACKGROUND: Lynch syndrome is a highly penetrant cancer predisposition syndrome caused by germline mutations in DNA mismatch repair (MMR) genes. We estimated the risks of primary cancers other than colorectal cancer following a diagnosis of colorectal cancer in mutation carriers. METHODS: We obtained data from the Colon Cancer Family Registry for 764 carriers of an MMR gene mutation (316 MLH1, 357 MSH2, 49 MSH6, and 42 PMS2), who had a previous diagnosis of colorectal cancer. The Kaplan-Meier method was used to estimate their cumulative risk of cancers 10 and 20 years after colorectal cancer.…

Familial colorectal cancer type X: polyp burden and cancer risk stratification via a family history score.

BACKGROUND: Patients fulfilling Amsterdam-1 criteria without mismatch repair deficiency (termed familial colorectal cancer type X (FCC type X)) were reported to have lower cancer risks than classic Lynch syndrome. This study investigates the polyp and cancer burden of this population and demonstrates relationships with a family history score (FHS). METHODS: The Jagelman Registry was queried for patients meeting Amsterdam criteria with microsatellite stable/low colorectal cancers. The risk of colorectal neoplasia was ascertained using a published FHS. Polyp distribution, histology and cumulati

[Familial tumor syndrome and ectopic hormone-producing tumors].

It has been thought that endocrine tumors occurred through interactions of multiple environments factors and a personal genetic factor. A normal somatic cell having an intrinsic function is able to obtain the character of a malignant cell by influence of many factors. Several percent of all tumors have obvious familial aggregation. These entity are called familial cancer. Familial cancer syndrome is well defined for colorectal cancer and breast cancer, but an endocrine neoplasia is the one, too. Ectopic hormone producing tumors are kinds of endocrine tumors, and have the characteristics, which

Fanca deficiency reduces A/T transitions in somatic hypermutation and alters class switch recombination junctions in mouse B cells.

Fanconi anemia is a rare genetic disorder that can lead to bone marrow failure, congenital abnormalities, and increased risk for leukemia and cancer. Cells with loss-of-function mutations in the FANC pathway are characterized by chromosome fragility, altered mutability, and abnormal regulation of the nonhomologous end-joining (NHEJ) pathway. Somatic hypermutation (SHM) and immunoglobulin (Ig) class switch recombination (CSR) enable B cells to produce high-affinity antibodies of various isotypes. Both processes are initiated after the generation of dG:dU mismatches by activation-induced cytidin

Pathological complete response following immunotherapy in dMMR/MSI-H ascending colon primary squamous cell carcinoma: a case report.

Primary colon squamous cell carcinoma (SCC) is an extremely rare malignancy and associated with a poor prognosis. This case report describes a patient with deficient mismatch repair/microsatellite instability-high (dMMR/MSI-H) ascending colon SCC, who was treated at our institution. After receiving four cycles of programmed cell death -1 (PD-1) blockade monotherapy, the tumor exhibited significant regression, and pathological complete response (pCR) was achieved following surgical resection. This case demonstrates that Anti-PD-1 therapy can induce clinically meaningful tumor regression even in

Contrasting responses to neoadjuvant immunotherapy in synchronous transverse colon cancers with discordant mismatch repair status.

Mismatch repair (MMR) status is a critical predictive biomarker for immune checkpoint inhibitor (ICI) efficacy in colorectal cancer (CRC); however, direct in vivo comparisons of ICI responsiveness between MMR-deficient (dMMR) and MMR-proficient (pMMR) tumors within the same patient are exceedingly rare. We report an exceptional case of synchronous transverse colon cancer with discordant MMR status that demonstrated strikingly divergent responses to neoadjuvant immunotherapy. A 75-year-old woman with a history of dyslipidemia presented with anemia. She was diagnosed with two synchronous transve

Abnormal Nuclear Membranous Staining Pattern by MLH1 Immunohistochemistry in Endometrial Cancer: A Diagnostic Pitfall and Clone-dependent Artifact.

Immunohistochemistry (IHC) for mismatch repair (MMR) proteins is routinely performed for endometrial cancer (EC). Loss of nuclear staining for MLH1/PMS2 triggers reflex testing for MLH1 promoter hypermethylation, while loss of MSH2/MSH6 or isolated loss of MSH6 and PMS2 prompts germline testing for Lynch syndrome. We observed an unusual nuclear membranous staining pattern of MLH1 (clone G168-15). The goal of the study was to determine its significance and highlight this IHC interpretation pitfall. A total of 52 EC cases with abnormal IHC staining patterns were identified in our database from 2

Neoadjuvant Immunotherapy With Prolgolimab in Patients With Locally Advanced Microsatellite Instability/Defective Mismatch Repair Colorectal Cancer.

PURPOSE: Although adjuvant chemotherapy is standard for locally advanced colon cancer, defective mismatch repair (dMMR)/microsatellite instability (MSI) tumors show high sensitivity to immune checkpoint inhibitors. We aimed to investigate the efficacy of the PD-1 inhibitor prolgolimab in this setting. METHODS: We conducted phase II nonrandomized open-label clinical trial (ClinicalTrials.gov identifier: NCT06428487) in patients with locally advanced colorectal cancer (CRC) and dMMR/MSI. Prolgolimab (1 mg/kg) was administered once every 2 weeks, and surgery was performed after 6 months of immun

Primary cutaneous lymphoma is a microsatellite stable tumor: An analysis of microsatellite instability.

A deficiency in DNA mismatch repair (MMR) leads to microsatellite instability (MSI), which is associated with a favorable response to immune checkpoint inhibitors (ICIs), and the Promega MSI Analysis System is approved as a companion diagnostic tool for it. In this study, we investigated the MMR status in patients with primary cutaneous lymphoma (PCL) diagnosed at our hospital. MSI was found in 1 of the 29 patients (3.4%), an 87-year-old man diagnosed with subcutaneous panniculitis-like T-cell lymphoma. Only the NR-21 marker was present in both tumor and normal tissue, indicating that the MMR …

BRAF-mutant mismatch repair deficient invasive colon cancer regressing to sessile serrated lesion.

A 69-year-old female was presented with a history of sigmoid colon cancer, uterine cancer, and intrahepatic carcinomas. After computed tomography revealed a disseminated nodule located in the peritoneum, colonoscopy demonstrated a rather flat-to-slightly elevated lesion with a depressed area located in the ascending colon. The flat component showed color similar to its surrounding area, and the depressed area showed redness and an expanded appearance. We obtained a biopsy specimen from the depressed area, and microscopic examination revealed well-differentiated adenocarcinoma, which was immuno

Remodeling of the immune and stromal cell compartment by PD-1 blockade in mismatch repair-deficient colorectal cancer.

Immune checkpoint inhibitor (ICI) therapy can induce complete responses in mismatch repair-deficient and microsatellite instability-high (d-MMR/MSI-H) colorectal cancers (CRCs). However, the underlying mechanism for pathological complete response (pCR) to immunotherapy has not been completely understood. We utilize single-cell RNA sequencing (scRNA-seq) to investigate the dynamics of immune and stromal cells in 19 patients with d-MMR/MSI-H CRC who received neoadjuvant PD-1 blockade. We found that in tumors with pCR, there is a concerted decrease in CD8+ Trm-mitotic, CD4+ Tregs, proinflammatory

Improving identification of lynch syndrome patients: a comparison of research data with clinical records.

Current evidence suggests poor identification and referral of Lynch syndrome patients. This study evaluated the strategies by which patients with endometrial cancer were referred to genetics services. Data from clinic-based patients with endometrial cancer enrolled through the Australian National Endometrial Cancer population-based research study with detailed family history information were analyzed. The Amsterdam II criteria, the revised Bethesda guidelines, and criteria adapted for this study was assessed using personal/family history information. The percentages of patients referred and wh

Application of molecular diagnostics for the detection of Lynch syndrome.

Lynch syndrome (LS), or hereditary nonpolyposis colorectal cancer, is the most common hereditary colorectal cancer (CRC) syndrome, accounting for approximately 2-5% of all newly diagnosed cases of CRC. Patients with LS have an increased lifetime risk of colorectal (52.2% in women and 68.7% in men) and endometrial cancer (15-70%), as well as certain extra-colonic cancers. Germline mutations in one of several DNA mismatch repair genes underlie LS. Molecular testing has emerged as an indispensable strategy for the diagnosis of LS. The diagnostic work-up of at-risk individuals includes a careful f…
