We describe a novel technique in which a guide wire is used for the placement of a decompression tube in patients with large odontogenic cysts involving crucial structures. This technique facilitates precise placement of the tube, rather than blindly positioning it within the space.

The objective of this paper is to contrast the use of a longitudinal incision in surgical decompression of De Quervain disease with a transverse incision. The advantages are ease in recognition of compartment variations and superficial branches of radial nerve and prevention of palmar tendon subluxation by permitting a more dorsal release of the compartment sheath. Since 2002, we have used a longitudinal skin incision instead of the classic transverse incision to release the first dorsal compartment.

BACKGROUND: Nerve root decompression with instrumented spondylodesis is the most frequently performed surgical procedure in the treatment of patients with symptomatic low-grade spondylolytic spondylolisthesis. Nerve root decompression without instrumented fusion, i.e. Gill's procedure, is an alternative and less invasive approach. A comparative cost-effectiveness study has not been performed yet. We present the design of a randomised controlled trial on cost-effectiveness of decompression according to Gill versus instrumented spondylodesis.
METHODS/DESIGN: All patients (age between 18 and 70

BACKGROUND AND STUDY AIM: Reoperation for lumbar spinal stenosis (LSS) is technically challenging. Studies comparing preoperative risk factors and reoperation outcomes between spinal fusion and spinal decompression are limited. Thus this study compared fusion and decompression with respect to reoperation rates, preoperative factors related to re-surgery, and clinical outcomes.
PATIENTS AND METHODS: This retrospective cohort study included prospectively collected data from patients who underwent revision surgeries for degenerative LSS between May 2001 and March 2015. The reoperation rate, risk

BACKGROUND: The management of spinal stenosis by surgery has increased rapidly in the past two decades, however, there is still controversy regarding the efficacy of surgery for this condition. Our aim was to investigate the efficacy and comparative effectiveness of surgery in the management of patients with lumbar spinal stenosis.
METHODS: Electronic searches were performed on MEDLINE, EMBASE, AMED, CINAHL, Web of Science, LILACS and Cochrane Library from inception to November 2014. Hand searches were conducted on included articles and relevant reviews. We included randomised controlled tria

BACKGROUND CONTEXT: Early decompression in spinal tuberculosis (TB) with complete paraplegia has a better prognosis in relation to the neurological recovery and deformity progression. Advanced pregnancy can complicate this picture in view of the various fetomaternal factors. The prevalent literature is inconclusive regarding the time and extent of surgical intervention. Delay in the surgical intervention may adversely affect the prognosis.
PURPOSE: To emphasize the importance of early decompression in spinal TB complicated by neurological deficit in advanced pregnancy.
STUDY DESIGN: A case r

BACKGROUND: Evidence regarding the frequency and timing of treatment for lumbar spinal stenosis (LSS) fails to offer clear consensus. We describe the LSS care journey from initial diagnosis to first surgical intervention.
METHODS: Using Medicare claims database from 2009 through 2020, we identified patients who were diagnosed with LSS. The use and timing of conservative and surgical treatments during the entire follow-up from the initial diagnosis were reported.
RESULTS: Of the 143,849 patients identified, 68% received conservative care within 8.4 months and 25.3% received a surgical or mini

The management of metastatic spine disease is complex, but usually involves radiation therapy and/or surgical treatment. Surgery followed by radiation has a significant role in select patients presenting with metastatic spinal cord compression. Ventral decompression can be achieved through several surgical approaches including posterior, posterolateral, and anterior surgical approaches. Although open thoracotomy is the most common approach for ventral decompression, it is associated with significant spinal access morbidity. This video illustrates a thoracoscopic transdiaphragmatic approach for

To test the efficacy and safety of orbital decompression for Graves' ophthalmopathy, the authors studied the records of 60 consecutive patients who were operated on for dysthyroid optic neuropathy or for rehabilitative purposes. Patients decompressed for neuropathy were older, had less proptosis, and a shorter duration of eye disease than patients operated on for disfigurement. The authors compared the results of three surgical procedures including the inferomedial, the inferomedial plus lateral, and the coronal approach. Regarding improvement of visual function, no difference was found betwee

OBJECTIVE: Stroke is the most prevalent disease involving the central nervous system. Since medical modalities are sometimes ineffective for the acute edema following massive infarction, surgical decompression may be an effective option when medical treatments fail. The present study was undertaken to assess the outcome and prognostic factors of decompressive surgery in life threatening acute, severe, brain infarction.
METHODS: We retrospectively analyzed twenty-six patients (17 males and 9 females; average age, 49.7yrs) who underwent decompressive surgery for severe cerebral or cerebellar in

Hemifacial spasms (HFS) are often triggered by vascular compression of the facial nerve, with the vertebral artery (VA) playing a role in 10-20% of cases. This study retrospectively analyses surgical techniques for personalised treatment strategies in patients with VA-involved HFS to determine the most effective surgical approach based on VA characteristics. This retrospective study analysed 112 patients with VA-involved HFS who underwent microvascular decompression (MVD) from July 2020 to June 2023. Patients were categorised into four subtypes of VA-involved HFS based on VA characteristics an

OBJECTIVE: Chiari I malformation (CM-1), traditionally defined as a descent of the cerebellar tonsils by 5 mm or more, is surgically managed via posterior fossa decompression. However, there is currently no clear radiologic or symptomatic selection criteria for surgical intervention to indicate which patients may see the most improvement following decompression.
METHODS: This retrospective pilot study included 22 patients who had symptomatic CM-1 managed via surgical decompression and 22 patients who had nonoperative management of CM-1. Tonsillar motion was assessed using phase-contrast magne

In this clinical feasibility study, CT-based verification of the efficacy of navigated decompression and pedicle screw placement in patients who had tumor-related posterior surgery was demonstrated. Eighty-six percent of the pedicle screws were positioned centrally in the bone without perforation; in all patients accurate decompression was seen. The accuracy of transpedicle screw implantation postoperatively was investigated with CT. In contrast to other published studies, no postoperative neurologic deterioration was seen in the patients as a result of using computer-aided surgical procedures

PURPOSE: Orbital decompression is an established surgical treatment option for a range of orbital conditions. Traditionally, Ear, Nose and Throat surgeons have adopted the endoscopic route while ophthalmologists operate via an external approach. The authors report the outcomes of endonasal decompression performed by oculoplastic surgeons experienced in endonasal techniques.
METHODS: This was a retrospective case series of patients who underwent endoscopic orbital decompression for proptosis secondary to thyroid eye disease across 2 hospital sites between January 2011 and July 2018. Inclusion

We report two cases of nerve abscesses, one suffering from lepromatous leprosy (LL) and the other from tuberculoid neural leprosy. Neither had any signs of reactions. Both were untreated cases. Surgical nerve decompression and systemic prednisolone had resolved the nerve abscess in the first case, whereas the second one responded only to surgical nerve decompression. The unusual nature of clinical presentation of nerve abscess has been outlined.

OBJECTIVE: Chiari malformation type I (CM-I) is defined by the herniation of the cerebellar tonsils into the spinal canal. When symptomatic, surgical decompression is recommended. Reported CM-I reoperation rates have ranged from 3% to 30%. However, the relationship between patient age at first surgical intervention and the likelihood of reoperation and postoperative complications remains poorly characterized. Therefore, this study aimed to determine whether patient age was associated with reoperation and complication rates.
METHODS: Patients 0-21 years old with a diagnosis of CM-I and surgica
