Atopic Dermatitis Disease Complications.

This chapter will describe infectious complications of atopic dermatitis, including bacterial, viral, and fungal infections and the evolving understanding of the relationship between atopic dermatitis and infectious disease. The underlying immunological dysregulation and poor skin barrier function associated with atopic dermatitis not only increase the likelihood of infectious complications but also lend atopic dermatitis skin vulnerable to flares induced by environmental triggers. Thus, this chapter will also highlight the impact of common external environmental agents on precipitating flares…

Pediatric Allergic Contact Dermatitis.

Allergic contact dermatitis (ACD) affects up to 20% of adults and children, although children are infrequently patch tested. Available data suggest that children and adults, with or without atopic dermatitis, have the same prevalence of ACD. Patch testing is the gold standard for evaluation of ACD. The Pediatric Baseline Series was recently published by expert consensus for use in pediatric patch testing, with additional allergens tested as guided by history. This article examines methods of patch testing and up-to-date data on pediatric ACD. The top allergens are reviewed, and avoidance strat…

[Airborne allergic contact dermatitis].

In this case report, a 25-year-old woman presented with contact dermatitis caused by airborne exposure to isothiazolinones in paints. Allergic contact dermatitis is most frequently caused by direct contact with a product containing an allergen. However, in rare cases, allergic contact dermatitis can be caused by airborne exposure to an allergen. This report highlights the importance of considering airborne exposure in patients with unexplained dermatitis.

Adult-Onset Atopic Dermatitis.

One in 4 adults with atopic dermatitis (AD) report adult-onset disease. Adult-onset AD appears to be associated with a different disease phenotype compared with childhood-onset AD. A broad differential diagnosis must be considered in a patient presenting with an adult-onset eczematous eruption, including allergic contact dermatitis, mycosis fungoides/cutaneous T-cell lymphoma, psoriasis, scabies, and so forth. This review will specifically address the diagnosis, workup, and management of adult-onset AD. In adults presenting a new-onset chronic eczematous eruption, consideration should be given…

[Pitfall cryothermic dermatitis artefacta].

We present four clinicopathological correlated cases of young patients with cryothermic dermatitis artefacta. They were initially misdiagnosed as primary bullous dermatoses or fixed drug eruptions. Cryothermic dermatitis artefacta can imitate authentic dermatoses such as linear IgA bullous dermatosis, herpes virus infection, bullous pemphigoid or fixed drug eruption. It should be considered as differential diagnosis in uncommon cases of recurrent bullae in adolescent and young adult patients. We summarize helpful clinical and histopathological criteria for correct diagnosis and therewith causa…

Laxative-Induced Contact Dermatitis.

A 5-year-old female with Charcot-Marie-Tooth neuropathy and a history of constipation presented to the emergency department with a new blistering buttocks rash, which was initially concerning for nonaccidental burn. Upon further investigation, it was found that Ex-Lax had been given to the patient for constipation. This had resulted in a bowel movement, which led to an irritant dermatitis. The patient was eventually diagnosed with senna-induced erosive diaper dermatitis. This case report highlights the importance of a thorough history and physical examination to prevent an unnecessary child ab…

Fibrillar-type dermatitis herpetiformis.

Dermatitis herpetiformis (DH) is a pruritic papulovesicular disease that is relatively common in Caucasian populations, and was first reported by Louis Duhring. It is characterized by granular IgA deposition in the dermal-epidermal junction and gluten-sensitive enteropathy (GSE) associated with human leukocyte antigen-DQ2/DQ8 haplotype. In contrast, DH is rare in Japan, and Japanese DH patients occasionally show unique features, including a high frequency of fibrillar IgA deposition in the papillary dermis, and rare occurrence of GSE. We refer to this condition in Japanese patients as "fibrill…

Genital Allergic Contact Dermatitis.

Genital allergic contact dermatitis (ACD) is an uncommon disorder, yet it severely impairs the quality of life for both men and women. Because of cultural taboos, many patients self-treat and delay proper diagnosis before presenting to a provider. Diagnosis is further confounded by irritant contact dermatitis and other genital dermatoses, which can predispose to skin barrier dysfunction and allergen penetration. Genital ACD can present acutely with erythematous erosions and pruritus or chronically with lichenification. Patch testing helps determine the diagnosis and provide relief for the pati…

Atopic dermatitis: emerging therapies.

Crisaborole and dupilumab represent the first 2 Food and Drug Administration (FDA)-approved therapies for atopic dermatitis (AD) in more than 15 years, and there are many promising drugs currently in development. This new wave of therapeutics capitalizes on the large body of work clarifying the pathogenesis of AD over the last several decades. In particular, type 2 cytokine-driven inflammation and skin barrier dysfunction are key processes underlying AD pathogenesis.
