Pathophysiology and Management of Mild to Moderate Pediatric Atopic Dermatitis.

Atopic dermatitis (AD), or eczema, is a chronic inflammatory skin condition characterized by relapsing pruritic and dry, scaly lesions. AD affects 10% to 20% of children in the United States and significantly affects the quality of life of patients and their families. Primary care providers (PCPs) are often the first point of contact for the management of AD symptoms. As many as 70% of patients with mild to moderate disease can be managed by a PCP, underscoring the need for these providers to understand basic AD pathophysiology and current standards of care. This article will discuss the basic

The impact of atopic dermatitis on work life - a systematic review.

Atopic dermatitis (AD) has considerable multidimensional personal and societal costs. However, the extend to which the patient's work life is affected due to AD is more sparsely described in the literature. The objective of this review was to examine the impact on work life for patients with AD, with a specific focus on choice of education and occupation, sick leave, social compensations and change of job due to AD. A systematic literature search was performed in PubMed, EMBASE and Web og Science up to 7 February 2017 for articles on the impact on work life for patients with AD. Results were s

Association of Atopic Dermatitis with Cardiovascular Risk Factors and Diseases.

Epidemiological studies suggested an association between atopic dermatitis (AD) and cardiovascular disease. Therefore, we investigate associations and potential underlying pathways of AD and cardiovascular disease in large cohort studies: the AOK PLUS cohort (n = 1.2 Mio), the GINIplus/LISAplus birth cohorts (n = 2,286), and the Cooperative Health Research in the Region of Augsburg (KORA) F4 cohort (n = 2,990). In addition, metabolomics in KORA F4 and established cardiovascular risk loci in genome-wide data on 10,788 AD cases and 30,047 controls were analyzed. Longitudinal analysis of patients

Environmental risk factors and their role in the management of atopic dermatitis.

The etiology of atopic dermatitis (AD) is multifactorial with interaction between genetics, immune and environmental factors. Areas covered: We review the role of prenatal exposures, irritants and pruritogens, pathogens, climate factors, including temperature, humidity, ultraviolet radiation, outdoor and indoor air pollutants, tobacco smoke exposure, water hardness, urban vs. rural living, diet, breastfeeding, probiotics and prebiotics on AD. Expert commentary: The increased global prevalence of AD cannot be attributed to genetics alone, suggesting that evolving environmental exposures may tri

Efficacy of Astaxanthin for the Treatment of Atopic Dermatitis in a Murine Model.

Atopic dermatitis (AD) is a common chronic inflammatory skin disease associated with various factors, including immunological abnormalities and exposure to allergens. Astaxanthin (AST) is a xanthophyll carotenoid that has recently been demonstrated to have anti-inflammatory effects and to regulate the expression of inflammatory cytokines. Thus, we investigated whether AST could improve the dermatitis and pruritus in a murine model of AD using NC/Nga mice. In addition to a behavioral evaluation, the effects of AST on the AD were determined by the clinical skin severity score, serum IgE level, h

Feasibility of actigraphy wristband monitoring of atopic dermatitis in children.

BACKGROUND/PURPOSE: Actigraphy monitors are used to monitor sleep and scratching. Previous studies have implemented these monitors to evaluate behavior in adult patients with atopic dermatitis. However, such monitoring devices have been implemented in a paucity of studies involving pediatric patients with atopic dermatitis. The purpose of this study was to assess the feasibility of actigraphy monitoring in children with mild-to-severe atopic dermatitis. METHODS: A total of six pediatric subjects were recruited. The severity of atopic dermatitis at the wrist area was assessed prior to placemen

Risk factors for the development of atopic dermatitis and early wheeze.

A global assessment of allergic diseases and prenatal and postnatal exposure to various environmental risk factors is needed to enable early prevention of allergic diseases. This study was designed to evaluate an inner-city urban birth cohort to identify early environmental factors associated with atopic dermatitis and food allergy, as well as the incidence of wheezing during the 1st year of life. We evaluated 501 children from the Polish Mother and Child Cohort Study (2007-2011). The children's health, socioeconomic status, and housing conditions were assessed using a questionnaire. Exposure

Profiling of serum and urinary microRNAs in children with atopic dermatitis.

BACKGROUND: Atopic dermatitis (AD) is the most prevalent chronic inflammatory skin disease in children characterized by dermatitis and pruritus. MicroRNAs (miRNAs) have been shown as great potential biomarkers for disease fingerprints to predict prognostics. We aimed to identify miRNA signature from serum and urine for the prognosis of AD patient by genome-wide miRNA profiling analysis. METHODS: Serum and urine from 30 children with AD and 28 healthy children were collected and their genome-wide miRNA expression profiles were measured by TaqMan-based array and confirmed by quantitative real-t

Atopic dermatitis is not a protective factor for melanoma but asthma may be.

BACKGROUND: There is evidence from cohort studies for an inverse association between atopic dermatitis and asthma and cutaneous melanoma. However, these studies have been too heterogeneous and did not show statistically significant results. Also, this association has not been compared to traditional melanoma risk factors. OBJECTIVES: To test for associations between history of atopic disorders and melanoma life-time prevalence, and for associations between atopic disorders and melanoma prognosis. METHODS: Validated questionnaires from the European Community Respiratory Health Survey and Inte

Sphingolipids and antimicrobial peptides: function and roles in atopic dermatitis.

Inflammatory skin diseases such as atopic dermatitis (AD) and rosacea were complicated by barrier abrogation and deficiency in innate immunity. The first defender of epidermal innate immune response is the antimicrobial peptides (AMPs) that exhibit a broad-spectrum antimicrobial activity against multiple pathogens, including Gram-positive and Gram-negative bacteria, viruses, and fungi. The deficiency of these AMPs in the skin of AD fails to protect our body against virulent pathogen infections. In contrast to AD where there is a suppression of AMPs, rosacea is characterized by overexpression o

[Atopic dermatitis: a modern view of pediatricians and pediatric allergologist].

The article presents the views of pediatric allergologist on the problem of atopic dermatitis/ atopic eczema in children. Atopic dermatitis (AD) is considered from a modern viewpoint of allergic 'march', which is characteristic (typical) for children with atopy. These data indicate to systemic nature of atopic 'march', the first step of which is atopic eczema. Further evolution of atopic dermatitis leads to a transformation of it in other atopic diseases--allergic rhinitis and bronchial asthma; this fact indicates that immunopathological disorders are united in these diseases and it conditions

An update on the genetics of atopic dermatitis: scratching the surface in 2009.

A genetic basis for atopic dermatitis (AD) has long been recognized. Historic documents allude to family history of disease as a risk factor. Before characterization of the human genome, heritability studies combined with family-based linkage studies supported the definition of AD as a complex trait in that interactions between genes and environmental factors and the interplay between multiple genes contribute to disease manifestation. A summary of more than 100 published reports on genetic association studies through mid-2009 implicates 81 genes, in 46 of which at least 1 positive association

Role of topical calcineurin inhibitors on atopic dermatitis of children.

Besides the traditional topical treatment of mild and moderate forms of atopic dermatitis (AD), which includes the daily use of emollients and the intermittent use of topical corticosteroids (TCSs) as anti-inflammatory drugs, a new group of drugs has recently been introduced to control the inflammatory phase of the disease: topical calcineurin inhibitors (TCIs). Although the efficacy of TCSs is evident, prolonged unrestricted use is limited by local and systemic side effects. The major risk in children is the hypothalamic-pituitary-adrenal gland suppression, due to the higher percutaneous abso
