Venom Anaphylaxis.

Allergy to venoms of stinging insects (bees, yellow jackets, hornets, wasps, and stinging ants) causes large local reactions or systemic reactions, including anaphylaxis. Diagnostic evaluation includes skin testing and/or serum venom-specific IgE and basal serum tryptase (to identify underlying mast cell disorders). Patients with sting anaphylaxis should be prescribed epinephrine and venom immunotherapy which prevents allergic reactions in up to 98% of patients. Most patients may safely discontinue venom immunotherapy after 5 years, although high-risk patients need to continue indefinitely. Fu

Combined omalizumab and desensitization to control IgE-mediated hypersensitivity in enzyme replacement therapy for late-onset Pompe disease.

BACKGROUND: Pompe disease is a rare, progressive lysosomal storage disorder caused by acid α-glucosidase deficiency, leading to glycogen accumulation, proximal muscle weakness, and respiratory decline. Enzyme replacement therapy (ERT) significantly improves survival and stabilizes motor function, but IgE-mediated hypersensitivity reactions (HSRs) can critically compromise treatment, posing a major clinical challenge. Desensitization protocols allow temporary tolerance to ERT, yet breakthrough reactions may occur, necessitating adjunctive strategies such as omalizumab. RESULTS: We report a 40-

Imatinib Desensitization After a Type IV Hypersensitivity Reaction in a Gastrointestinal Stromal Tumor Patient-A Case Report.

BACKGROUND: Imatinib treatment is approved for several indications, including chronic myeloid leukemia (CML) and gastrointestinal stromal tumors (GIST). Although adverse events are common, hypersensitivity reactions are not. Because there is a clear clinical benefit of imatinib treatment, re-introduction of imatinib after a hypersensitivity reaction should be considered. CASE: Here we present a case report of a 68-year-old patient with a GIST diagnosis who was re-introduced to imatinib after a type IV hypersensitivity reaction via desensitization. A desensitization plan, a plan for formulatio

The role of immunoglobulin E in upper respiratory allergies: mechanisms and therapeutic strategies.

This review focuses on the central role of Immunoglobulin E (IgE) in upper respiratory allergic diseases and associated therapeutic strategies. As the core immunoglobulin driving type 2 inflammation, IgE production is meticulously regulated by multiple cytokines and signaling pathways. Current therapeutic approaches primarily involve two strategies: direct inhibition of IgE generation and function, and promotion of Immunoglobulin G4 (IgG4) and Immunoglobulin A (IgA) production to competitively suppress IgE-mediated responses. For direct IgE inhibition, monoclonal antibodies such as omalizumab a

Assessing the Allergenic Relevance of Vespula alascensis Venom: Implications for Venom Immunotherapy.

BACKGROUND AND OBJECTIVE: Venom immunotherapy (VIT) is an effective treatment for yellow jacket venom (YJV) allergy. It is often based on mixtures of venom from different species. Recent taxonomic re-evaluation has revealed that widely used VIT preparations contain venom from Vespula alascensis, a North American species, rather than venom from the Palearctic Vespula vulgaris. Objective: To assess the allergenic relevance of V alascensis venom for European patients and evaluate its suitability for inclusion in VIT preparations. METHODS: The ability of V alascensis and V vulgaris venom to activ

Adrenaline: A Lifeline for Rapid Drug Desensitization in Hypersensitive Patients.

BACKGROUND AND OBJECTIVE: Chemotherapeutic agents (CMTs) and monoclonal antibodies (mAbs) are common causes of drug allergy, which is often managed using rapid drug desensitization (RDD). Despite its effectiveness, RDD can be hampered by severe breakthrough reactions (BTRs), potentially leading to failure of the procedure. Objective: To evaluate the usefulness and safety of adrenaline infusion (AI) as an adjuvant during RDD in patients who experience severe drug hypersensitivity reaction (DHR) during standard desensitization protocols. METHODS: Retrospective observational study, analyzing dat

Epicutaneous immunotherapy for food allergy: a systematic review and meta-analysis.

BACKGROUND: There is ongoing debate about the safety and efficacy of epicutaneous immunotherapy (EPIT) in treating food allergies. The systematic review and meta-analysis aimed to evaluate the safety and efficacy of EPIT. METHODS: We systematically searched international trial registers (ClinicalTrials.gov), PubMed, Embase, the Cochrane Central of Controlled Trials (CENTRAL), and Web of Science from the inception of the database until June 25, 2023. Two authors independently screened potential studies based on the following criteria: food allergy, epidermal immunotherapy, and randomized contr

Epicutaneous immunotherapy: A review of safety and efficacy.

Epcutaneous immunotherapy (EPIT) is a novel, non-oral route of allergen immunotherapy, utilizing the skin and its robust density of epidermal Langerhans cells (LC) for antigen presentation. This space is non-vascularized and impermeable, which limits allergen exposure into the bloodstream but preserves antigen presentation to regional lymph nodes to generate gut-homing regulatory T cells. The EPIT patch utilizes natural water loss from the skin to absorb electrosprayed allergen through condensation. EPIT represents an alternative, non-oral route of immunotherapy for food allergy, with good eff

Should Biologics Be Used Before Aspirin Desensitization in Aspirin-Exacerbated Respiratory Disease?

There has been a paradigm shift in the management of aspirin-exacerbated respiratory disease (AERD). It started in 2015 when the first biologic was Food and Drug Administration (FDA) approved for severe eosinophilic asthma. Thus, there emerged a new era in the treatment of patients with type 2-mediated airway diseases. This has led to an increasing number of options for patients, undoubtably a great thing, but has left clinicians without a clear answer for how to balance the therapies that exist for AERD, what to recommend for treatment, and how to best assess the benefits and risks of each th

Probing the conjugation of epigallocatechin gallate with β-lactoglobulin and its in vivo desensitization efficiency.

Epigallocatechin gallate (EGCG) and β-lactoglobulin (βLg) were conjugated by covalent bonds to form EGCG-βLg conjugates. This conjugation causes structural and bioactivity changes in βLg, which in turn can be used as a possible approach for desensitization to allergens. In this study, the desensitization mechanism was investigated by monitoring βLg secondary structure and immunoglobulin E (IgE) combining capacity changes on the basis of the conjugation mechanism. Furthermore, the desensitization efficiency in vivo was evaluated through animal experiments. The results show that temperature infl

Individually dosed omalizumab facilitates peanut oral immunotherapy in peanut allergic adolescents.

BACKGROUND: Peanut oral immunotherapy (pOIT) has showed good short-term outcomes, but allergic reactions may prevent effective up-dosing and is a major cause of stopping OIT. In placebo-controlled trials, omalizumab has been shown to facilitate allergen immunotherapy and increase tolerance to peanut. OBJECTIVE: We hypothesized that by combining omalizumab with pOIT, and monitor treatment effects with basophil allergen threshold sensitivity tests (CD-sens), peanut allergic patients could safely initiate pOIT and thereafter slowly withdraw omalizumab. METHODS: This is the 2nd part of a one-arm

Conjunctival provocation tests: prediction of seasonal allergy.

PURPOSE OF REVIEW: The conjunctival provocation test (CPT) is often used to clearly identify the specific allergen causing the symptoms of allergic rhinoconjunctivitis but also to assess the clinical efficacy of an allergen immunotherapy (AIT). As there is no consensus about its predictive value, the aim of this publication is to evaluate under which conditions the CPT can predict the symptom severity during the allergy season after previous AIT. RECENT FINDINGS: Three out of four randomized controlled trials (RCTs) showed a correlation between CPT reactivity and symptoms occurring under natu

Aspirin-exacerbated respiratory disease: Prevalence, diagnosis, treatment, and considerations for the future.

Aspirin-exacerbated respiratory disease (AERD) is a late onset condition characterized by the Samter triad (aspirin sensitivity [as well as sensitivity to any nonselective cyclooxygenase inhibitor], nasal polyps, asthma) and additional features, including eosinophilic chronic rhinosinusitis, hypereosinophilia, anosmia, frequent absence of atopy, and, intolerance to ingestion of red wine and other alcoholic beverages. The diagnosis is rare, and, because of this, it is also often missed by physicians. However, it is highly overexpressed in patients with severe asthma (and severe chronic rhinosin

Elevated IL-5 and IL-13 responses to egg proteins predate the introduction of egg in solid foods in infants with eczema.

BACKGROUND: Egg allergy is a leading cause of food allergy in young infants; however, little is known about early allergen-specific T-cell responses which predate the presentation of egg allergy, and if these are altered by early egg exposure. OBJECTIVE: To investigate the early T-cell responses to multiple egg proteins in relation to patterns of egg exposure and subsequent IgE-mediated egg allergy. METHODS: Egg-specific T-cell cytokine responses (IL-5, IL-13, IL-10, IFNγ and TNFα) to ovomucoid (OM), ovalbumin (OVA), conalbumin (CON) and lysozyme (LYS) were measured in infants with eczema at
